Epidemiologic studies are either observational or experimental. Observational studies, including ecologic,
cross-sectional, cohort, and case-control designs, are considered “natural” experiments, but experimental
studies are considered true experiments. We will spend the next 2 modules discussing these designs.
Before we begin to discuss study designs, we need a brief introduction to a concept that we will spend more
time discussing in later modules -- bias. The definition of bias is:
“Deviation of results or inferences from the truth, or processes leading to such deviation. Any trend in the
collection, analysis, interpretation, publication, or review of data that can lead to conclusions that are
systematically different from the truth.” (Last, J.M., A Dictionary of Epidemiology, 4th ed.)
Epidemiologists are naturally concerned whether the results of an epidemiologic study are biased, since many
important public health decisions are often drawn from epidemiologic research. The severity of the bias, that
is - how much it influences or distorts the results, is related to the study design as well as how information is
analyzed.
Experimental Studies
The defining feature of experimental studies is that the investigator assigns exposure to the study subjects.
Experimental studies most closely resemble controlled laboratory experiments and serve as models for the
conduct of observational studies, thus they are the “gold standard” of epidemiologic research. Experimental
studies have high validity (i.e., less bias), and can identify even very small effects. The most well known type of
experimental study is a randomized trial (sometimes referred to as a randomized controlled trial), where the
investigator randomly assigns exposure to the study subjects. In this type of study, the only expected
difference between the experimental and control groups is the outcome variable being studied.
Experimental designs like the randomized trial can assess both preventive interventions, where a prophylactic
agent is given to healthy or high-risk individual to prevent disease, or can assess effects of therapeutic
treatment, such as those given to diseased individuals to reduce their risk of disease recurrence, or to improve
their survival or quality of life.
Preventive intervention: Does tamoxifen lower the incidence of breast cancer in women with high risk profile
compared to high risk women not given tamoxifen?
Therapeutic intervention: Do combinations of two or three antiretroviral drugs prolong survival of AIDS
patients as well as regimens of single drugs?
The investigator can assign exposures (or allocate interventions) to either individuals or to an entire
community.

Defecation
Normal defecation begins with movement in the left colon, moving stool toward the anus. When stool reaches the rectum, the distention causes relaxation of the internal sphincter and an awareness of the need to defecate. At the time of defecation, the external sphincter relaxes, and abdominal muscles contract, increasing intrarectal pressure and forcing the stool out
The Valsalva maneuver exerts pressure to expel faeces through a voluntary contraction of the abdominal muscles while maintaining forced expiration against a closed airway. Patients with cardiovascular disease, glaucoma, increased intracranial pressure, or a new surgical wound are at greater risk for cardiac dysrhythmias and elevated blood pressure with the Valsalva maneuver and need to avoid straining to pass the stool.
Normal defecation is painless, resulting in passage of soft, formed stool
CONSTIPATION
Constipation is a symptom, not a disease. Improper diet, reduced fluid intake, lack of exercise, and certain medications can cause constipation. For example, patients receiving opiates for pain after surgery often require a stool softener or laxative to prevent constipation. The signs of constipation include infrequent bowel movements (less than every 3 days), difficulty passing stools, excessive straining, inability to defecate at will, and hard feaces
IMPACTION
Fecal impaction results from unrelieved constipation. It is a collection of hardened feces wedged in the rectum that a person cannot expel. In cases of severe impaction the mass extends up into the sigmoid colon.
DIARRHEA
Diarrhea is an increase in the number of stools and the passage of liquid, unformed feces. It is associated with disorders affecting digestion, absorption, and secretion in the GI tract. Intestinal contents pass through the small and large intestine too quickly to allow for the usual absorption of fluid and nutrients. Irritation within the colon results in increased mucus secretion. As a result, feces become watery, and the patient is unable to control the urge to defecate. Normally an anal bag is safe and effective in long-term treatment of patients with fecal incontinence at home, in hospice, or in the hospital. Fecal incontinence is expensive and a potentially dangerous condition in terms of contamination and risk of skin ulceration
HEMORRHOIDS
Hemorrhoids are dilated, engorged veins in the lining of the rectum. They are either external or internal.
FLATULENCE
As gas accumulates in the lumen of the intestines, the bowel wall stretches and distends (flatulence). It is a common cause of abdominal fullness, pain, and cramping. Normally intestinal gas escapes through the mouth (belching) or the anus (passing of flatus)
FECAL INCONTINENCE
Fecal incontinence is the inability to control passage of feces and gas from the anus. Incontinence harms a patient’s body image

5. NATURALISTIC OBSERVATION
Sometimes all a researcher needs to know is what
is happening to a group of animals or people. The
best way to look at the behavior of animals or
people is to watch them behave in their normal
environment.
6. In many cases, animals or people who know they
are being watched will not behave normally—a
process called the observer effect.
Observer bias. That happens when the person
doing the observing has a particular opinion about
what he or she expects to see.
7. LABORATORY OBSERVATION
Sometimes observing behavior in animals or people is
just not practical in a natural setting.
laboratory settings have the disadvantage of being an
artificial situation that might result in artificial
behavior—both animals and people often react
differently in the laboratory than they would in the real
world.
The main advantage of this method is the degree of
control that it gives to the observer.
8. CASE STUDIES
“One individual is studied in great detail.”
In a case study, researchers try to learn everything they
can about that individual.
Sigmund Freud based his entire theory of
psychoanalysis on case studies.
The advantage of the case study is the tremendous
amount of detail it provides.
The disadvantage of the case study is that researchers
can’t really apply the results to other similar people.
9. SURVEY
In the survey method, researchers will ask a series of
questions about the topic they are studying. Surveys
can be conducted in person in the form of interviews or
on the telephone, the Internet, or with a questionnaire.
That is the big advantage of surveys, aside from their
ability to get at private information. Researchers can get
a tremendous amount of data on a very large group of
people.
Of course, there are disadvantages. For one,
researchers have to be very careful about the group of
people they survey.
10. THE EXPERIMENT
The only method that will allow researchers to determine
the cause of a behavior is the experiment.
In an experiment, researchers deliberately manipulate
(change in some purposeful way) the variable they think
is causing some behavior while holding all the other
variables that might interfere with the experiment’s
results constant and unchanging.
11. STEPS IN EXPERIMENT METHOD:
Selection: First, selection of the participant.
Variables: Second, decide on the variable the researchers
want to manipulate (Independent Variable) and the variable
they want to measure to see if there are any
changes.(Dependent Variable)
Groups: The group that is exposed to the independent
variable (Experimental group). The other group that gets
either no treatment or some kind of treatment that should have
no effect (Control group).
A variable is anything that can change or vary—scores on a test,
temperature in a room, gender, and so on.
12. EXPERIMENTAL HAZARDS:
Confounding variables: Variables that interfere with
each other and their possible effects on some other
variable of interest.
The improvement may have had more to do with
participants’ belief in the drug than the drug itself, a
phenomenon known as the placebo effect.
Another way that expectations about the outcome of the
experiment can influence the results, even when the
participants are animals rather than people, is called the
experimenter effect.
13. CONTROL THESE EFFECTS
Single-Blind: Even if the control group improves a little,
the drug group should improve significantly more if the
drug is working. Because the participants are “blind” to
the treatment they receive.
Double-Blind Studies: neither the participants nor the
experimenter know who got what.
14. CORRELATIONS METHOD:
“a measure of the relationship between two or more
variables.”
The strength of the relationship between the
variables will be determined by the actual number
itself. That number will always range between +1.00
and −1.00.
