1) The Sudbury District Nurse Practitioner Clinics (SDNPC) are a group of NP-led primary care clinics in Sudbury, Ontario that aim to provide access to care for underserved populations. 2) The SDNPC consists of 3 clinics with a multidisciplinary team including 5.5 NPs, 2 part-time physicians, and other health professionals. They provide comprehensive primary care. 3) The SDNPC was established in 2007 in response to thousands of residents lacking access to primary care. It has shown successes in improving access to care and detecting previously undiagnosed health issues, with a 7% reduction in ER visits attributed to increased access.
