A 30-year-old asthmatic presented with worsening dyspnea and wheezing during a chest infection. His oxygen saturation was 88%. While a pneumothorax was expected, his chest X-ray instead showed a collapsed right middle lobe (atelectasis). Atelectasis can occur in asthmatics due to airway narrowing and excess secretions. Treatment for this patient should include intensive chest physiotherapy in addition to usual asthma medications, rather than chest drain insertion.
