NAME OF ARTIFACT : INDIVIDUAL LEARNING MONITORING PLAN
PROFESSIONAL STANDARDS/STRAND:
3.1. Learners’ gender, needs, strengths, interest and experiences
ANNOTATION:
I contacted the parents of my students and let them know the status of their children. I
made sure to record in the ILMP the intervention that I am planning. That is to give them
Learning Activity Sheet.
NAME OF ARTIFACT : LESSON PLANS (DLP, DLL, WHLP, WLL, LESSON
EXEMPLARS)
PROFESSIONAL STANDARDS/STRAND:
3.4. Learners in difficult circumstances
ANNOTATION:
Some of the parents find it hard to make sure that their children is working on their
module specially here that some students are helping their parents in their livelihood. To
make sure that students have time to work on their modules, I made WHLP and give it to
their parents together with the modules to help them keep on track on their modules.

ANNOTATION:
To assue of giving proper delivery of my lessons, I always make sure that I had the DLL
before going to the class

I made a more simple learning activity sheets with mini lesson in order for the students to
easily understand and answer the said activity.

7.4. Professional reflection & learning to improve practice
ANNOTATION:
In this reflection, I stated the importance of the LDM course on me as a teacher.
This LDM course helps us realize our strengths and weaknesses specially now
that we are in pandemic. It helps us determine on where we need to develop
more and let us improve more on what we already know. And let us see where
we fail as a teacher. It also help us realize that we need to keep our artifacts so
that we can be ready anytime it is needed.

Learners’ gender, needs, strengths, interest and experiences ANNOTATION: I contacted the parents of my students and let them know the status of their children. I made sure to record in the ILMP the intervention that I am planning. That is to give them Learning Activity Sheet.

Learners in difficult circumstances ANNOTATION: Some of the parents find it hard to make sure that their children is working on their module specially here that some students are helping their parents in their livelihood. To make sure that students have time to work on their modules, I made WHLP and give it to their parents together with the modules to help them keep on track on their modules.

Design, selection, organization and utilization of assessment strategies ANNOTATION: I made a more simple learning activity sheets with mini lesson in order for the students to easily understand and answer the said activity.

Professional links with Colleagues ANNOTATION: I attended different trainings together with my colleagues like LAC sessions and INSET for me to improve my knowledge on different skills in teaching specially now that we are on the new normal.

Professional reflection & learning to improve practice ANNOTATION: In this reflection, I stated the importance of the LDM course on me as a teacher. This LDM course helps us realize our strengths and weaknesses specially now that we are in pandemic. It helps us determine on where we need to develop more and let us improve more on what we already know. And let us see where we fail as a teacher. It also help us realize that we need to keep our artifacts so that we can be ready anytime it is needed.

Monitoring & evaluation of learner progress & achievement ANNOTATION: I checked the students activity sheets every after submission to ensure that I am giving them timely feedbacks on their achievements.
