Primary care can help reduce chronic disease and lower healthcare costs. A case study in northeast England showed how a primary care network improved access and reduced emergency admissions and A&E visits. They analyzed patient data, identified issues like access and care for patients in nursing homes, then redesigned systems and care pathways. For diabetes care specifically, they streamlined the patient pathway, increased capacity, and eliminated backlogs which led to improved patient care. The key factors in its success were using excellent patient data, focusing on specific problems, incentivizing practices, dynamic quality improvement processes, and multidisciplinary team reviews.
