New Directions in Targeted Therapeutic Approaches for Older Adults With Mantl...i3 Health
i3 Health is pleased to make the speaker slides from this activity available for use as a non-accredited self-study or teaching resource.
This slide deck presented by Dr. Kami Maddocks, Professor-Clinical in the Division of Hematology and
Associate Division Director for Ambulatory Operations
The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.
Target Audience
Hematology/oncology fellows, attending faculty, and other health care professionals involved in the treatment of patients with mantle cell lymphoma (MCL).
Learning Objectives
1.) Identify clinical and biological prognostic factors that can guide treatment decision making for older adults with MCL
2.) Evaluate emerging data on targeted therapeutic approaches for treatment-naive and relapsed/refractory MCL and their applicability to older adults
3.) Assess mechanisms of resistance to targeted therapies for MCL and their implications for treatment selection

Ethanol (CH3CH2OH), or beverage alcohol, is a two-carbon alcohol
that is rapidly distributed in the body and brain. Ethanol alters many
neurochemical systems and has rewarding and addictive properties. It
is the oldest recreational drug and likely contributes to more morbidity,
mortality, and public health costs than all illicit drugs combined. The
5th edition of the Diagnostic and Statistical Manual of Mental Disorders
(DSM-5) integrates alcohol abuse and alcohol dependence into a single
disorder called alcohol use disorder (AUD), with mild, moderate,
and severe subclassifications (American Psychiatric Association, 2013).
In the DSM-5, all types of substance abuse and dependence have been
combined into a single substance use disorder (SUD) on a continuum
from mild to severe. A diagnosis of AUD requires that at least two of
the 11 DSM-5 behaviors be present within a 12-month period (mild
AUD: 2–3 criteria; moderate AUD: 4–5 criteria; severe AUD: 6–11 criteria).
The four main behavioral effects of AUD are impaired control over
drinking, negative social consequences, risky use, and altered physiological
effects (tolerance, withdrawal). This chapter presents an overview
of the prevalence and harmful consequences of AUD in the U.S.,
the systemic nature of the disease, neurocircuitry and stages of AUD,
comorbidities, fetal alcohol spectrum disorders, genetic risk factors, and
pharmacotherapies for AUD.

How you can cure pcos naturally
1. What is Polycystic Ovarian Syndrome (PCOS) and How You Can Cure PCOS Naturally?
PCOS is the number one reproductive disorder diagnosed in women and affectsapproximately five percent inthe United States.
Characterized by hormonal imbalance, many symptomsincludeobesity, abnormal hair growth and absenceof or irregular menstrua l periods.
Affected women may have multipleovarian cystsand an increased risk of Type-2 diabetesand infertility. Witha number of possible factors
contributing to thisdisease including genetics, diet, environment, vitamindeficiencies, metabolic issues, and stress, there is no known Cure
PCOS Naturally. Thiscan seem overwhelming at first however, there isa growing community of womenwho have lowered their sym ptomsor
even Cure PCOS Naturally by adoptinga plant-based diet, exercising andcutting outtoxinsfound in their day to day lives. Get on the pathto
healingby followingthese restorative tips:
Diet Overhaul
Thismay sound intimidating but thehealingmust beginwith your diet. Many women are managing their symptomsor curing PCOS
completely by first purgingtheir kitchensof meat, dairy, eggs, processed foods, caffeine, alcohol and soy and switching to a low fat, high
fiber, plant-based diet that promotesweight lossand hormone balance.
Avoid Processed and Refined Foods
The preservativesand chemicalsfound in processed and refinedfoodscan disrupt hormone production. From the ingredientsto the
packaging, processed foodsare a must to avoid. These can includehot dogs, bacon, sausage, beef jerky, lunch meat, salami, chicken
nuggets, cheese, savory snacks such as chips, microwave or ready meals, bread, canned food, breakfast cerealsand soda.
Switch to Plant-Based
Roughly half of women withPCOS are obese and a majority suffersfrom rapid weight gainand difficulty losing weight. Often, losingaslittle
as five to ten percent of body weight can restore a woman’snatural menstrual cycle. Followinga low fat, high fiber, plant -based diet isthe
easiest way to lose weight sustainably and start healing your PCOS.
Eliminating foodshigh infat and saturatedfat such asmeat and eggspromotesnatural and sustainableweight lossthat can last a lifetime.
Processed meat isoften packed with hormonesand antibioticsthat can disrupt the metabolism and hormonelevelsin women. Bre ast and
ovarian cancershave further been linked to dairy consumption andDr. T. Colin Campbell, Professor Emeritusof nutritional biochemistry at
Cornell University and author of The ChinaStudy, found that casein, the mainprotein inmilk, isone of the most significantcancer promoters
ever discovered. Cows are also injected withsynthetic hormonessuch as recombinant bovinegrowth hormone(rBGH) to increase th e
production of milkwhich can cause hormoneimbalancesand weight gain inhumans.
A diet low in fat, highin fiber and whole grainscan helpreverse Type-2 diabetes, reduce body weight and addressunhealthy cholesterol
levels. The additionof fruitsand vegetablesare also excellent at managing insulin levels.
Avoid Caffeine and Alcohol
2. Caffeine increasesyour body’s normal estrogen levelswhich are already highin women withPCOS. Thishighly addictivedrug blocksthe
absorption of magnesium, a key mineral that isessential to the colon’sregulation of normal, healthy, bowel movements. Coffe e, inparticular,
compoundsthisproblem by acting asa laxative, causingthe bowelsto move prior to the absorptionof water and mineral nutrientswhich
reinforcesmalnutritionand dehydration. These negative effectscan disrupt digestion andthe metabolism leading to difficult y losing weight in
the long run.
Alcohol inhibitsyour liver from metabolizing moleculesproperly.Women withPCOS already haveproblemsfilteringout excess insulin and
androgen hormoneswhich cause rampant hormonal imbalancesin the body.
Avoid Soy
Soy containsnutrientsknown as Phytoestrogensand substancesknown as Goitrogens. Phytoestrogensmimic the hormoneEstrogen, which
causes Endocrine disruption inwomenwith PCOS. Goitrogensblockyour Thyroid from takingin Iodine, which causesThyroid disruption and
affectsvariousaspects of PCOS.
Supplement with Probiotics
Probioticsare important for womenwith PCOS, butmostly overlooked. According to Nava Health andVitality Center, “…benefici al bacteria in
your digestive system have the capability of affecting your body’s vitamin andmineral absorbency, hormone regulation, digestion, vitamin
production, immune response, and ability to eliminatetoxins, not to mentionyour overall mental health”. Kefir, Kimchi, Kombucha, and
probiotic supplementsare great optionsto add to your diet.
Environmental Factors
These factorsare less obviousbut important to consider when discovering new waysof managingor curing your PCOS naturally.
Avoid BPA Products
BPA is as an endocrine disruptor which interfereswith your natural hormonal system. Studiesfind that women with PCOS havea higher
amount of BPA in their blood, which cannotbe metabolized by the liver. Avoid drinking out of cans, do not heat up foodsin p lastic containers
(limit/eliminate all plastic productsaltogether if possible by using glassproducts), drinkout BPA free water bottles, and limit/decrease using
canned foods.
Start Exercising
3. You don’t need to start trainingfor a marathonor be a pro athletebut 30 minutesa day for at least 5 daysa week can make a huge
difference. Not only doesexercise aid inweight loss, it restores natural hormone levels, actsas a stress reliever (reducing cortisol andthe
production of androgens/insulin resistance), and fightsanxiety/depression.
Avoid Toxic Chemicals
These can be found in every day householdproductsand eliminatingthem can make a big difference.According to a study published inthe
Journal of Environmental andPublic health, many every day productshave endocrine-disruptingproperties, including “cleaningproducts, air
fresheners, hair dyes, cosmetics, and sunscreens”. Since PCOS isassociated with endocrine functioning, itisbest to choose productsthat
are as natural aspossible. Use natural cleaning products, eat organically whenever possible, limit deodorants/p erfumes/toxic skincare
products, and take your shoes off at the door before entering your home to avoid bringing inoutdoor pollutants.
Conclusion
Being diagnosed with any healthdisorder isdifficult to tolerateand PCOS can feel debilitating. However, thisdisorder doesnot have to
dampen your outlookon life andbirth control isnot the only way to manage your symptoms. Many womenhave found naturally re storative
solutionsthat have increased their long term healthand evenCure PCOS Naturally.
