2. What is Home Visit?
A visit made by a health professional (HP) to a patient's home.
3. Why do home visit?
- To assess the environment that the client will return to home
- To observe the client’s ability to function in his/her natural
environment
4. Who participates?
- Therapist, to identify potential areas of difficulty
- Client, to discuss and demonstrate potential areas of difficulty
- Family, to discuss their concerns
- Other staff (such as social welfare) who might be able to assist
the therapist and family in identifying and resolving problem
areas
5. Types of Home Visit
There are many types of home visits and different reasons for
doing them. Some common types are:
1. Pre-discharge home visit
2. Discharge home visit
3. Post-discharge home visit
4. Community home visit
6. Preparing for the visit
Before a visit, therapist should also ensure
- Client/family permission is obtained to conduct the visit
- Address is known
- Transport has been arranged
- S/he has a tape-measure, note book and pen
- S/he has thought about and obtained any necessary equipment
or aids that are specially required (remember, thought the
therapist will always emphasize construction and design of
locally make equipment using local skills and materials).
