Malaria is still considered globally as a leading cause of morbidity with Nigeria carrying the highest burden of 19%. Coinfection of malaria and Human Immunodeficiency Virus (HIV) accelerate disease progression of HIV/AIDS subjects. This study investigated the prevalence and predictors of malaria among HIV infected subjects attending the antiretroviral therapy Clinic at Federal the Medical Centre, Keffi, Nigeria. After ethical clearance, 200 whole blood specimens were collected from patients who gave informed consent and completed a self-structured questionnaire. The specimens were examined for malarial parasite using rapid kits and microscopy. The overall prevalence of the infection was 78/200 (39.0%). The prevalence was higher in male (44.7%) than female (34.0%) subjects. Those subjects aged < 20 years (54.5), male gender (44.7%), non-formal education holders (61.5%), farmers (62.5%), stream water users (48.1%), those that lives in rural setting (43.6%), those that do not use Insecticides Treated Nets (ITNs) (39.4%) and swampy environment dwellers (41.7%) were identified predictors for malaria infection in the area. All the predictors studied did not show any statistically significant difference with the infection but some arithmetic difference exists (P > 0.05). The 39.0% prevalence of malaria in HIV infected subjects is a public health concern. Therefore, Public health surveillance and health education among HIV population should be advocated to help eradicate malaria comes 2030. Further study that will characterize the genes of the parasite should be carried out.

ABSTRACT
Objective: To determine the frequency of hepatitis C virus infection in patients with type 2 diabetes mellitus and to look for the common risk factors leading to this infection in diabetics. Study Design: Descriptive cross sectional study design. Place and Duration of Study: Department of Medicine, Combined Military Hospital (CMH) Kharian, from Jan 2015 to Jun 2015. Patients and Methods: This study was conducted in the department of Medicine, Combined Military Hospital Kharian. Through a descriptive cross sectional study design, a total of 140 patients with type 2 diabetes mellitus, admitted through casualty, OPD or private clinics were selected and tested for Hepatitis C virus infection. The common risk factors leading to such infection among positive cases were also scrutinized. Results: The mean age of patients was 48.82 ± 10.14 with 60.7% female gender predominating the overall sample of diabetics. Using 3rd generation ELISA method, hepatitis C virus was found in 45 (32.1%) of patients with 41-50 years of age group most commonly affected age group (34.7%) and female (57.8%) commonly affected gender. The distribution of risk factors leading to hepatitis C virus in diabetics are: 21 (46.7%) had history of surgery in the past, 13 (28.9%) had history of blood transfusion in the past, 7 (15.55%) had history of hemodialysis while only 4 (8.9%) had history of tattooing in the past. Conclusion: Hepatitis C virus infection is still a common problem in diabetic patients of our local population and we recommend further research work over its risk factors so that the guidelines for its control may be formulated. ABSTRACT
This study on the prevalence of TB among HIV sero-positive was carried at the HIV CLINIC of Kampala International University Teaching Hospital (KIUTH), Ishaka Bushenyi district. A retrospective cross-sectional study design was used to conduct this research. The study targeted all patients attending KIUTH HIV/TB clinic. A standard structured and semi-structured questionnaires were designed and pre-tested for validity and reliability at Kampala International University Teaching Hospital HIV/Tuberculosis clinic before being used for data collection. Data collection started by recruitment of qualified research assistants, appropriate training and orientation of the interviewers before the survey for example when reading the questions. Quantitative methods of data analysis was used in which data was presented in form of bar charts, graphs and tables. The prevalence of TB among HIV sero-positive patients attending HIV clinic at KIUTH stands at 8.06 per 100 participants. The study found that generally, people are aware about the modes of transmission of TB but there is still need for more awareness. Many patients are still not certain whether TB is curable in HIV patients. As seen from the above study, most of the people are not yet aware whether HIV goes hand in hand with tuberculosis. The prevalence of TB in HIV sero-positive attending HIV clinic at KIUTH is high. Generally, TB is affecting patients of all ages and most patients are still not aware if TB in HIV is curable. Most patients have a perception that all TB patients have HIV. Health workers in HIV clinic of KIU-TH should teach patients the modes of transmission and prevention of TB. KIUTH also need to provide easy access to TB screening services to patients. There is need for financial support by the government to the unemployed patients and low-income earners in order to curb TB infections.

ABSTRACT- INTRODUCTION- HIV/AIDS pandemic is responsible for the resurgence of Tuberculosis worldwide,
resulting in increased morbidity and mortality. Co-infection with HIV infection leads to difficulty in both the diagnosis
and treatment of Tuberculosis, increased risk of death, treatment failure and relapse.
OBJECTIVE- The present study highlights the correlation of Pulmonary Tuberculosis in HIV positive cases and its
association with CD4 count.
MATERIAL & METHODS- A total of 72 known case of HIV were screened for tuberculosis infection by clinical
examination, radiology & ZN staining.
RESULTS AND CONCLUSIONS- From our study 60 (83.33%) were diagnosed as tuberculosis and 12 (16.67%) were
negative. More common HIV infection in case of male 48 (66.67%). Out of 60 tuberculosis infection 53 (88.33%) were
diagnosed as Pulmonary Tuberculosis and 7 (11.67%) were diagnosed as Extrapulmonary Tuberculosis. The result of
study emphasizes that co-infection of tuberculosis in HIV/AIDS patient is a concern. There is direct correlation between
CD4 counts depletion and Pulmonary Tuberculosis in HIV/AIDS patients.

COVID-19 is a new infectious disease with an unclear incidence and an unknown rate of progression to severe disease. The Gibraltar COVID-19 Cohort utilises two distinct cohorts - a clinical cohort and a random population based cohort -, to provide an accurate assessment of case severity rate. Design: Retrospective analysis of a SARS-CoV2 RT-PCR point prevalence study and a RT-PCR confirmed positive clinical case cohort to calculate case severity rates. Settings and Participants: Over a three day period nasopharyngeal swabs were sampled from a randomly selected 1.2% of the population of Gibraltar and then analysed via RT-PCR to determine the background incidence of COVID-19 infection. The results were then analysed and compared to the clinical case cohort. The rate of progression to severe COVID-19 disease in those with COVID-19 infection was then calculated.

Background: Tuberculosis is one of the most ancient infectious diseases caused by Mycobacterium tuberculosis. The population most affected is the young and economically productive one. The social factors include poor quality of life, poor housing, overcrowding, population explosion, under nutrition, lack of education, and last but not the least lack of awareness of cause of illness.
Aims and Objectives:
1. To study the clinical presentation of tuberculosis in patients.
2. To study various risk factors of tuberculosis.
Material and Methods: This study was conducted at selected designated microscopic centre (DMCs) Kanpur Nagar district has a population of 45.73lakh ( Census 2011).All the patients who were registered in the selected DMCs in the last one month of the year 2016 ( between April and May) were taken into consideration for the present study. Data was collected on predesigned and pretested questionnaire using direct personal interview method of patients at DMCs on the DOTS days of the week i.e Monday, Wednesday and Friday. Informed consent of the study subjects was taken before interview. A total of 105 registered patients were interviewed personally and also the treatment card of patients was obtained from their respective DMCs.
Results: Out of 105 cases of tuberculosis which reported at DMCs maximum no. of patients belongs to age group between 21-40 yrs of age group (58%). Majority of cases were married (65.7%) cases. (62%) cases were Hindu by religion and (58%) belongs to other backward caste. In the study we found majority of patient was illiterate (34.3%). Most common clinical presentation was cough, fever and cough with expectoration, anorexia was reported in (61.9 %) of cases (77%) were cigarette/bidi smokers, 60% were tobacco chewer. Diabetes was reported in (12.4%) cases and (3.8%) cases were HIV positive.

Despite overwhelming news coverage, significant knowledge gaps regarding COVID-19 persist. The current uncertainties regarding incidence and the case fatality rate can only be addressed by widespread testing. But the paucity of testing, and diversity of approaches implemented in different countries, particularly among the general asymptomatic public, perpetuates a lack of understanding about spread and infectivity. The essential indicators that would describe the pandemic more accurately can be obtained using real-world data (RWD). To that purpose, we designed a data collection tool to collect data from hospitals that treat COVID-19 patients. The captured data will enhance our understanding of the COVID-19 pandemic, identify risk factors relevant for triage, relate to other similar seasonal infections and gain insight into the safety and efficacy of experimental and off-label therapies. Knowledge derived from a focused data collection effort will enable clinicians to adjust rapidly clinical protocols and discontinue interventions that turn out to be ineffective or harmful. By deploying our elegantly designed survey to capture routine clinical indicators, we avoid placing an additional burden on practitioners. Systematically generating real-world evidence can decrease the time to insight compared to randomized clinical trials, improving the odds for patients in rapidly changing conditions.

The current pandemic of Corona Virus Disease-2019 (COVID-19) which is caused by Severe Acute Respiratory Syndrome Corona Virus-2 (SARS-CoV-2) has resulted in lockdown in many countries culminating in a major socio-economic crisis globally. COVID-19 can remain asymptomatic and so is crucial for early diagnosis to prevent further spread of this pandemic. Here we highlight the importance of screening asymptomatic patients prior to elective surgery, procedure or scheduled hospital admission. This analysis was done for the month of July 2020 during which 261 asymptomatic people were screened for COVID-19. Out of this, 6 patients (2.29%) were diagnosed to have COVID-19 on nasopharyngeal/ oropharyngeal swabs and subsequently had to delay their elective procedure or surgery. This clearly shows how important it is to screen this cohort of asymptomatic people who could potentially have spread the virus to other patients as well as healthcare professionals.
The prevalence of tuberculosis and Rifampicin-resistant tuberculosis (RMP-TB) among patients showing symptoms of tuberculosis that visited Federal Medical Centre, Yenagoa, Bayelsa state, Nigeria was determined from June 2015 to December 2015. A total of 456 patients comprising 218(47.8%) males and 238(52.2%) females were examined using their sputum and gastric lavage samples. GeneXpert System was used to determine the TB and RMF-TB. Results showed that out of the 456 patients, overall tuberculosis prevalence was 88(19.3%), males recorded 48(10.5%) while females had 40(8.8%). The highest tuberculosis prevalence was recorded amongst 21-30 years and 31-40 years age groups (5.5%). Out of the 456 patients, total prevalence for Rifampicin resistance was 11(2.4%). Of these, females and male prevalence was 6(1.3%) and 5(1.1%) respectively. There was no significant difference (P>0.05) in prevalence between age and gender. The treatment and follow-up of existing cases is a key to preventing the spread of multi drug-resistant tuberculosis.

RESULTS: Overall life span (LS) was 2252.1±1742.5 days and cumulative 5-year survival (5YS) reached 73.2%, 10 years – 64.8%, 20 years – 42.5%. 513 LCP lived more than 5 years (LS=3124.6±1525.6 days), 148 LCP – more than 10 years (LS=5054.4±1504.1 days).199 LCP died because of LC (LS=562.7±374.5 days). 5YS of LCP after bi/lobectomies was significantly superior in comparison with LCP after pneumonectomies (78.1% vs.63.7%, P=0.00001 by log-rank test). AT significantly improved 5YS (66.3% vs. 34.8%) (P=0.00000 by log-rank test) only for LCP with N1-2. Cox modeling displayed that 5YS of LCP significantly depended on: phase transition (PT) early-invasive LC in terms of synergetics, PT N0—N12, cell ratio factors (ratio between cancer cells- CC and blood cells subpopulations), G1-3, histology, glucose, AT, blood cell circuit, prothrombin index, heparin tolerance, recalcification time (P=0.000-0.038). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and PT early-invasive LC (rank=1), PT N0—N12 (rank=2), thrombocytes/CC (3), erythrocytes/CC (4), eosinophils/CC (5), healthy cells/CC (6), lymphocytes/CC (7), segmented neutrophils/CC (8), stick neutrophils/CC (9), monocytes/CC (10); leucocytes/CC (11). Correct prediction of 5YS was 100% by neural networks computing (area under ROC curve=1.0; error=0.0).
CONCLUSIONS: 5YS of LCP after radical procedures significantly depended on: 1) PT early-invasive cancer; 2) PT N0--N12; 3) cell ratio factors; 4) blood cell circuit; 5) biochemical factors; 6) hemostasis system; 7) AT; 8) LC characteristics; 9) LC cell dynamics; 10) surgery type: lobectomy/pneumonectomy; 11) anthropometric data. Optimal diagnosis and treatment strategies for LC are: 1) screening and early detection of LC; 2) availability of experienced thoracic surgeons because of complexity of radical procedures; 3) aggressive en block surgery and adequate lymph node dissection for completeness; 4) precise prediction; 5) adjuvant chemoimmunoradiotherapy for LCP with unfavorable prognosis.

-----------------------------------------------------------ABSTRACT---------------------------------------------------------Epidemiological studies were carried out among 1320 patients admitted at Aminu Kano Teaching Hospital (AKTH)
concerning the prevalence of hepatitis B & C and Co – infections with TB over a period of four years (2008 – 2012) .The
grand diagnostic kits were used to analyze blood samples for hepatitis B virus (HBV) and hepatitis C virus (HCV) while
sputum samples were analyzed for TB using the Zel Nielsen (ZN) staining technique. The aim of the present research is to
carry out epidemiological studies concerning the prevalence of Hepatitis B & C co – infection with Tuberculosis (TB) in a
well defined hospital based population. From the study, the overall sero-prevalence of hepatitis B and C were found to be
8.0% and 4.7% respectively. The relative risk of infection was found to be higher in males than in females (p>0.05) for the
two hepatitis markers. Out of 1320 subjects screened 196 (14.8%) were smear positive for TB out of which 17(8.7%) were
co-infected with HBV while 29(14.8%) were co-infected with HCV. Viral hepatitis constitutes a major public health problem
worldwide. The consequences of chronic liver diseases put a considerable economic burden upon the communities afflicted
with the diseases.

INRODUCTION
Hepatitis is an inflammation of the liver caused by virus, bacterial infections, or continuous exposure to alcohol, drugs, or toxic
chemicals, such as those found in aerosol sprays and paint thinners [1]. Hepatitis can also result from an autoimmune disorder in which the
body mistakenly sends disease fighting cells to attack its own healthy tissue i.e. the liver. The signs and symptoms of acute viral hepatitis are
similar regardless of the hepatitis virus responsible. Patients may experience a flu-like illness, and general symptoms include nausea,
vomiting, abdominal pain, fever and fatigue, loss of appetite, and less commonly rashes and joint pain. Sometimes a condition called
jaundice, a yellowing of the skin and eyes-develop [2]. These acute symptoms result from the damage the virus inflicts on the liver cells.
The acute symptomatic phase of viral hepatitis usually lasts from a few days to several weeks; the period of jaundice that may follow can
persist from one to three weeks. Complications of acute viral hepatitis include fulminate hepatitis, which is a very severe, rapidly developing
form of the diseases, and chronic hepatitis, which is characterized by liver cell death and inflammation that lasts longer than six months.
Hepatitis B (HBV) and hepatitis C (HCV) infections are common global health problems and they are spreading rapidly in developing
countries due to lack of health education, poverty and illiteracy. Both of these infections can be transmitted through blood or body fluid,
tattooing, through infected instruments, unsafe shave by barbers and sexual contacts [3].The development of serological tests for HBsAg and
their respective antibodies led to the recognition that HBV has a worldwide distribution and that infection rates in some parts of the world
such as Africa, Asia and Oceania are extremely high. Serum hepatitis cases were associated with HBV infections [4]. A high incidence
(16.0%) of HBV infection was reported in a rural community in Kano [5]. However, figures reported from various regions of Nigeria
indicated that prevalence rate of the diseases seems to be on the increases. [6] reported 8.9% from Lagos, [7] reported 17.0% from Ibadan
while 22.0% was reported from Maiduguri. The Hepatitis C virus (HCV) like HBV, also causes a life threaten infection. Approximately 170
million people worldwide are chronically infected with the hepatitis C virus (HCV) and the infection is often described as “silent” because
people may be infected for 10 – 30 years with no exhibited symptoms [8]. The prevalence of HCV is not well documented in many countries
partly due to the expense and practical difficulties involved in the detection of HCV RNA in the serum of infected individual [9]. According
to the Centre for Disease Control and Prevention (CDC) anyone who received a blood transfusion prior to 1992 before accurate routine
blood screening was established, may be infected with the virus [2]. Available data showed that the prevalence of HCV among local
commercial blood donors in Nigeria ranged from 12.3 – 14% [10]. Although more recent studies among patients with sickle cell anemia in
Lagos indicated 5.0% anti HCV prevalence [11]. Nigeria belongs to the group of countries highly endemic for viral hepatitis. Infant, about
75% of the Nigerian population is likely to have been exposed to hepatitis viruses at one time or the other in their life [12]. Viral hepatitis
constitutes a major public health problem. The consequences of chronic liver disease put a considerable burden upon the affected
communities and because there is no therapy for acute viral hepatitis, emphasis is placed on prevention through various control measures
[13]. The development of successful control measures has depended to a large extent on the growing understanding of the epidemiology and
biology of these viruses and on the availability of serologic assays to identify infected and immune individuals. The present study will
greatly enhance our understanding of the epidemiology of these two important diseases.

MATERIALS AND METHODS

2.1. Study area
Aminu Kano Teaching Hospital (A.K.T.H.) was selected for the study, is one of the reference hospitals in the state
where people from various parts of the state and neighboring states of various occupations attend. It serves as a Federal
Government Tertiary Health Center and gives more than 60% of health care delivery in the state at large.
2.2. Study population
The studied population was a heterogeneous of different age groups, ethnicities and educational status. Biodata and
other information were collected after obtaining written informed consent from each patient with the assurance that all
information obtained would be treated with utmost confidentiality.
2.3. Study design
General descriptive epidemiological study was used to design out the survey. Cross-sectional (Prevalence) survey was
carried out according to the ethical standards for human experimentation.
2.4. Sample size
A total of 1320 patients were recruited for the study. Three (3) ml of blood were collected from each patient by veinpuncture using sterile Syringes and needles into a sterile container with a screw cap. The blood samples collected were
centrifuged at 2000rpm for 5 minutes in order to separate the sera. After centrifugation, the sera were labeled and stored in
plastic tubes at 20C – 80C for analysis within 24 hours.
2.5. Virological Examination
Virological analysis for HBV and HCV was done on Patients admitted in the Aminu Kano Teaching Hospital over the
period of four years (2008- 2012) in an attempt to assess the situation in the Hospital. All procedures for the test were based
on recommendation of the manufacturers. The steps for the procedure are as follows:
Test strip, serum were allowed to equilibrate to room temperature 25-300C Prior to testing. Test strip was removed
from the sealed paunch and used as soon as possible with arrows pointing towards the serum. The test strip was immersed
vertically in to the serum for at least 10-15 seconds and placed on a non-absorbent flat surface with the timer on and then
observed for the red line (s) to appear. The results were read after 15 minutes. No result was interpreted after 30 minutes.
Two distinct red lines indicate positive, one line in the control region (C) and another line in the test region (T). One red line
in the control region (C) and no apparent red line or pink line in the test region (T) indicates negative. Results were invalid
when Control line fails to appear. Insufficient specimen volume or incorrect procedural techniques were the most likely
reasons for control line failure.
2.6. Tuberculosis screening procedure:
In order to detect cases of TB Co-infections with HBV and HCV, three Sputum samples were collected from each subjects and
examined for the presence of Acid fast bacilli using the Ziehl’s Neelsen staining techniques. Sputum collection was done in the open air
with the patients facing away from the wind and also away from others during expectoration [14]. Three sputum samples were usually
collected within 24 hours for diagnosis, while two samples were collected for follow-up cases. Sputum samples were smeared using
applicator stick in biosafety cabinets. The smears were air dried in the biosafety cabinet and the slides were then fixed by passing them
through flame about 2-3 times. The slides were then stained by pouring strong carbol-fuschin solution to cover the whole surface and then
heated until steam rose without boiling and allowed to stand for 5 minutes. The slides were then tilted and washed with distilled water and
decolorized with 3% acid alcohol for 3-5 minutes. The slides were then washed with distilled water and tilted to drained excess water.
Counter staining was done with 0.1% methylene blue for 1 minute and then washed with distilled water. The slides were then placed on the
slide rack and allowed to air dry. The smears were examined under X100 objective. Positive showed pink rod shape bacteria while the other
substances appeared blue.
2.7. Statistical analysis of data
Results were expressed as percentages by Cross-tabulation using SPSS 14 Version Soft ware. ANOVA was used to
compare categorical data at 95% confidence interval and significance level were taken at P ≤0.05

RESULTS
The results of the studies on TB Co-infections with HBV and HCV are presented in Table 1- 4 and Fig. 1- 2. Out of total 1320
subjects screened for TB, 196 were sputum smear positive, giving a prevalence rate of 14.8% (Table 1). Of the 196 subjects that were
sputum smear positive for TB, 17(8.7%) were co-infected with HBV while 29 (14.8%) were co-infected with HCV. The rate of HBV/TB
Co-infection was highest (4.2%) among the 21-30 years bracket and higher among females subjects (9.1%) than in males (8.4%) as shown in
tables 3 and 4 respectively .Fig 1 and 2 Illustrates the age and sex related distribution pattern for HCV/TB Co-infection among the subjects
respectively. Cases of HCV/TB Co-infection occurred most frequently in the 41-50 years age group and the rate was higher among males
(65.5%) than in females (34.6%) subjects.
