 The Engineer being relieved will prepare this report in duplicate. One copy ro be sent to
respective Superintendent In-Charged, and one to be retained on board.
 This report shall be submitted via e-mail attachment (pdf file) at least one week prior
signing-off to respective Superintendent In-Charged.
 Whenever there is insufficient space available, supplemental sheet to be added, In case
items to be accounted for are not onboard, explanation is required.
 The information on this report will be filled out within reasonable times prior to change
of Chief Engineer so that this report will require minimum updating during the day of
handing over.
 The document will be duly signed with date and time after it has been reviewed by the
incoming Engineer in the presence of outgoing Engineer and witness by Chief Engineer
on board.
