What is a Form
 A form is a document (electronic or
 printed) with spaces for on which
 to write or enter data.
REASONS FOR FORMS
 Standardization – for collecting
  data in standard procedures.
 Ensure that all data needed is
  collected
 Data is sequenced in preferred
  priority
