Peningkatan status kesehatan ibu dan anak merupakan salah satu hal prioritas di Indonesia. Status derajat kesehatan ibu dan anak sendiri dapat dinilai dari jumlah AKI dan AKB. Pemerintah berupaya menerapkan program Sustainable Development Goals (SDGs) dengan harapan dapat menekan AKI dan AKB, tetapi kenyataannya masih tinggi sehingga tujuan dari penyusunan laporan tugas akhir ini untuk memberikan asuhan kebidanan secara komprehensif dari ibu hamil trimester III sampai KB.
Metode penelitian menggunakan Continuity of Care dengan pendokumentasian SOAP Notes. Subjek penelitian Ny. “H” usia 34 tahun masa kehamilan Trimester III hingga KB di PMB E Kecamatan Ngunut Kabupaten Tulungagung.
Hasil asuhan selama masa kehamilan trimester III tidak ada komplikasi pada Ny. “E”. Masa persalinan berjalan lancar meskipun terdapat kesenjangan dimana IMD dilakukan kurang dari 1 jam. Kunjungan neonatus hingga nifas normal tidak ada komplikasi, metode kontrasepsi memilih KB implant.
Kesimpulan asuhan pada Ny. “H” ditemukan kesenjangan antara kenyataan dan teori di penatalaksanaan, tetapi dalam pemberian asuhan ini kesenjangan masih dalam batas normal. Asuhan kebidanan ini diberikan untuk membantu mengurangi kemungkinan terjadi komplikasi pada saat masa kehamilan hingga KB.

PRESENTATION.
Definition
Face presentation occurs when babys spine
extended until the head is shifted back so babys
face comes throught the pelvis first. Baby may
settle in a face presentation before labour. A baby
who is in a face first position often started as an
extended (chin up) occiput posterior or occiput
transverse position.

8. L01: PREDISPOSING FACTORS
FETAL CAUSE
1) Anencephaly
- Disebabkan tiada Vault of the scalp sementara
dibahagian facial, menyebabkan bahagian muka akan
memasuki pelvic brim apabila labour bermula.
9. 2) Tumour dibahagian leher fetus, thyroid neoplasm.
( Boleh menyebabkan extension of the head –
thyroid gland.)
10. 3) SPASM of the posterior neck muscle.
4) Shape of the fetal head-dolichocephaly
– dimana long anterior - posterior diameter of
head.
5) LOOPS OF CORD disekeliling leher yang
menyebabkan extension of the head.
11. Maternal cause
1) Contracted pelvis
Android pelvis -`parietal eminence`kepala fetal
yang deflex terperangkap didalam garis pusat
sacrocotyloid. Kepala fetal tidak boleh menurun
dan ekstend berlaku menyebabkan presentasi
muka.
12. Maternal cause
2) Polyhdramnious
Bila membran ruptured secara spontan kepala
fetus menjadi extensi disebabkan oleh aliran
amniotic fluid ke bahagian lower uterine
segment.
13. 3) Congenital uterine abnormalities.
- Contohnya bicornuate uterus ( irregular
shape of uterus ) dimana fetus tidak dapat
well flex dengan sempurna.
4) Spinal deformities dimana fetal head
tidak masuk dalam pelvic brim. Contohnya
spondylolisthesis.
5) Posterior placenta previa dimana fetal
head akan directed forward.

-Berlaku di mana dagu berpusing 1/8 pusingan
ke kiri pelvis ibu.

dan beritahu setiap progres labor beliau
supaya beliau lebih kuat dan dapat beri
kerjasama.

kepada doktor .
46. Pengurusan selepas kelahiran
1. Perhatikan tanda-tanda post partum haemorrhage.
2. Kekalkan posisi extension of the head setelah bayi
dilahirkan untuk mengelakkan masalah
pernafasan. Informed paeds untuk mengesan
keabnormalan.
3. Pemerhatian ke atas bayi adalah penting kerana
muka bayi bruising dan edema.
47. 4. Beritahu ibu bahawa bengkak dan
bruising akan kembali normal selepas
beberapa minggu.
5. Sekiranya tanda bengkak dan bruising
tidak hilang, nasihat ibu bawa bayi
berjumpa doktor untuk rawatan
selanjutnya.
6. Bantu ibu semasa feeding bayi.
