This document discusses consumer perspectives and concerns regarding personal health information and health IT. It notes that consumers want health information that supports informed decisions, and they have concerns about privacy, security, and how information may be used and shared without their consent. The document also outlines recommendations from a consumer forum to ensure community understanding and support for appropriate electronic health information use.
The document summarizes Dougal McKechnie's presentation on health IT in New Zealand. It discusses the New Zealand Health IT Cluster alliance and its role in facilitating collaboration. It also outlines challenges facing New Zealand's health system and opportunities for health IT, including the draft National Health IT Plan and developing New Zealand as a global eHealth research and development laboratory.
The document discusses making clinical terminology work by driving in-situ use of complex classification schemes using commoditized software. It notes that clinical classification involves many "wicked" influences that make the problem difficult to solve. The author proposes embedding standardized clinical terminologies into commonly used software like Microsoft Word to facilitate electronic documentation and data extraction in a way that is usable for both humans and machines without requiring technical expertise. A demonstration is provided of how standardized terminologies and other services could be integrated into word processing to support clinical documentation and decision-making.
The National Health IT Board provided an update on the status of New Zealand's national health IT plan. The plan is being implemented in two phases over 7 years, with the goal of enabling an integrated healthcare model through shared clinical data and decision support tools. Consumer forums showed general support for greater information sharing across the healthcare sector. Key areas like e-prescriptions and clinical standards are progressing, with regional implementation plans in development. The update concluded that continued work on interoperability standards and regional rollouts will help realize the plan's vision of coordinated care.
- An e-referrals pilot project was conducted in the Waikato region of New Zealand to evaluate the benefits of electronic referrals between general practices and the regional health board's referral centers.
- The pilot involved 5 GP practices transmitting referrals electronically using a standardized template, which provided benefits like reduced processing time and requests for additional information.
- The pilot demonstrated benefits for GPs, practices, referral centers, patients and the health system. However, scaling the solution would be challenging and specialty-specific templates were desired.
This document discusses a collaborative service network (CSN) being developed by Smart Internet CRC to facilitate communication between commercial vendors, universities, and government bodies. The CSN aims to stimulate market production of a next generation IP network meeting healthcare requirements within 18 months. Smart Internet CRC will provide commercial requirements analysis, R&D assistance, market stimulation, and focal point for university research. The initial target market is home telecare which could improve case management, assisted self management, and better use of human resources while reducing costs. Expected benefits of home telecare include decreased visits to GPs and hospitals as well as reduced time off work.
The document discusses the evolution of information systems to better support healthcare in New Zealand. It outlines several key areas such as meeting the needs of patients, providers, and the population's health as a whole. It also addresses the importance of innovation, privacy, infrastructure investment, and balancing the perspectives of individuals, providers, and policymakers. The response is to bring a more complete health picture for individuals into consultations to enable informed treatment decisions.
The document outlines four key lessons learned from the Health Roundtable experience in improving health care service delivery in New Zealand:
1. Deeper analysis is needed to uncover real performance differences between hospitals that are otherwise hidden, as casemix averages can hide big variations.
2. Financial incentives strongly influence practices and behaviors, as demonstrated by changes made in response to casemix payment systems.
3. Necessity drives innovation, as health systems have developed many strategies to address issues like bed shortages and growing elderly populations.
4. Peer discussions are valuable for sharing problems and innovations, and monitoring joint progress on key initiatives.
This document outlines eight priorities for improving New Zealand's health system in 2010/11. The priorities include creating a more unified system for service planning, funding, and provision. It also discusses establishing national services, regional service plans, improving DHB planning and monitoring, allocating ministry funding, workforce and facility capacity planning, reducing bureaucracy, and encouraging clinical leadership.
Accessing patient information from primary care providers allows ambulances to provide more complete care and ensure the patient's GP is informed, helping to participate in care plans and enhance clinical training. However, challenges include accessing primary care information, maintaining patient privacy in a multi-operator system, and ensuring effective communications and usability of any new system.
The document summarizes a presentation on organizing health information in an eHealth environment according to citizen-centric principles and concepts. It outlines the background, methods, preliminary findings, conceptual information model, information management principles, and roadmap presented. The presentation calls for an iterative development process to involve stakeholders and promote a citizen-centric model for integrating new eHealth possibilities in New Zealand's health systems.
The document discusses challenges with e-health and patient safety in New Zealand. It notes that patients want appropriate, safe, private care with good information and convenience, but often receive records that are disorganized and not integrated between providers. The document presents four cases: 1) a patient receiving suboptimal primary care despite quality standards, 2) a doctor failing to properly notify a patient of test results, 3) a medication error where an elderly patient received another's morphine in the hospital, and 4) a hospital failing to send a discharge summary to a patient's GP after multiple admissions. Overall it finds that while standards and accreditation aim to ensure quality, more work is still needed to translate this into good patient care in practice
1) The document discusses licensing and ownership of SNOMED CT content including the Core, Extensions, Derivatives, and National Releases.
2) It describes the roles of Members, Affiliates, and End Users in licensing SNOMED CT content from the IHTSDO and Members.
3) Ownership and obligations around transferring intellectual property rights in Extensions and Derivatives to the IHTSDO are also covered.
Alzheimers disease medications
Several prescription drugs are currently approved by the U.S. Food and Drug Administration
(FDA) to treat people who have been diagnosed with Alzheimer’s disease. Treating the symptoms of Alzheimer’s can provide patients with comfort, dignity, and independence for a longer period of time and can encourage and assist their caregivers as well. It is important to understand that none of these medications stops the disease itself.

Breast cancer: Post menopausal endocrine therapyDr. Sumit KUMAR
Breast cancer in postmenopausal women with hormone receptor-positive (HR+) status is a common and complex condition that necessitates a multifaceted approach to management. HR+ breast cancer means that the cancer cells grow in response to hormones such as estrogen and progesterone. This subtype is prevalent among postmenopausal women and typically exhibits a more indolent course compared to other forms of breast cancer, which allows for a variety of treatment options.
Diagnosis and Staging
The diagnosis of HR+ breast cancer begins with clinical evaluation, imaging, and biopsy. Imaging modalities such as mammography, ultrasound, and MRI help in assessing the extent of the disease. Histopathological examination and immunohistochemical staining of the biopsy sample confirm the diagnosis and hormone receptor status by identifying the presence of estrogen receptors (ER) and progesterone receptors (PR) on the tumor cells.
Staging involves determining the size of the tumor (T), the involvement of regional lymph nodes (N), and the presence of distant metastasis (M). The American Joint Committee on Cancer (AJCC) staging system is commonly used. Accurate staging is critical as it guides treatment decisions.
Treatment Options
Endocrine Therapy
Endocrine therapy is the cornerstone of treatment for HR+ breast cancer in postmenopausal women. The primary goal is to reduce the levels of estrogen or block its effects on cancer cells. Commonly used agents include:
Selective Estrogen Receptor Modulators (SERMs): Tamoxifen is a SERM that binds to estrogen receptors, blocking estrogen from stimulating breast cancer cells. It is effective but may have side effects such as increased risk of endometrial cancer and thromboembolic events.
Aromatase Inhibitors (AIs): These drugs, including anastrozole, letrozole, and exemestane, lower estrogen levels by inhibiting the aromatase enzyme, which converts androgens to estrogen in peripheral tissues. AIs are generally preferred in postmenopausal women due to their efficacy and safety profile compared to tamoxifen.
Selective Estrogen Receptor Downregulators (SERDs): Fulvestrant is a SERD that degrades estrogen receptors and is used in cases where resistance to other endocrine therapies develops.
Combination Therapies
Combining endocrine therapy with other treatments enhances efficacy. Examples include:
Endocrine Therapy with CDK4/6 Inhibitors: Palbociclib, ribociclib, and abemaciclib are CDK4/6 inhibitors that, when combined with endocrine therapy, significantly improve progression-free survival in advanced HR+ breast cancer.
Endocrine Therapy with mTOR Inhibitors: Everolimus, an mTOR inhibitor, can be added to endocrine therapy for patients who have developed resistance to aromatase inhibitors.
Chemotherapy
Chemotherapy is generally reserved for patients with high-risk features, such as large tumor size, high-grade histology, or extensive lymph node involvement. Regimens often include anthracyclines and taxanes.
5-hydroxytryptamine or 5-HT or Serotonin is a neurotransmitter that serves a range of roles in the human body. It is sometimes referred to as the happy chemical since it promotes overall well-being and happiness.
It is mostly found in the brain, intestines, and blood platelets.
5-HT is utilised to transport messages between nerve cells, is known to be involved in smooth muscle contraction, and adds to overall well-being and pleasure, among other benefits. 5-HT regulates the body's sleep-wake cycles and internal clock by acting as a precursor to melatonin.
It is hypothesised to regulate hunger, emotions, motor, cognitive, and autonomic processes.
