Employee Annual Assessment Period
Performance evaluations allow you, as managers,to engage in a dialogue about how well your employees
are performing their duties and to discuss their goals for the coming year. The first step of the assessment
process is for your employees to complete and send you their self-assessment. Employee self-
assessments will help you facilitate a review of your employees’ overall progress and accomplishments.
The second part of the assessment process is for you to evaluate your employees’ overall performance.
The third step is for you to meet with your employee to review their assessment. The fourth step is to
determine if your employees are eligible for a merit increase based on the assessment period and the merit
increase budget.
Please complete your employees’ assessment no later than November 24, 2017! Please refer to the
Employee Annual Assessment Process and Procedure for specific timelines.
Section III: Manager Assessment (Performance Competencies)
Performance Competencies: Please complete the following Manager Assessment. Use the spaces
provided to highlight your employees’ accomplishments, areas where you feel she/he needs improvement,
and provide ratings. The Rating Scale: Exceptional (5); Exceeds Expectations (4); Meet Expectations
(3); Needs Improvement (2); Unsatisfactory (1). Additionally, please complete the optional “Any Special
Considerations to Share” Section.
