•To be included in logbook signed by
preceptor/faculty & HOD.

8. ASSESMENT OF ACTIVITIES
•Included in logbook.
•Formative assessment – by preceptor

9. •Logbook is student
responsibility.
•It is required for the student to
appear for the examination

10. •Issue of certification to be ( a copy
) kept in department.
•The candidate signature to be
kept as evidence.

11. •A copy of documentation of
certification of all the students
who have attended the electives
postings should be preserved by
the HOD of the department.
