Genetic Basis of Pyridoxine-Responsive Neonatal Epilepsy in Consanguineous Fa...Delaina Hawkins
Hilal Al-Shekaili is a PhD student at the University of British Columbia who conducts research in rare, autosomal recessive disorders, specifically pyridoxine-responsive epileptic encephalopathies (PREE). PREE is often characterized by recurrent seizures in the prenatal, neonatal, or postnatal period, which are typically resistant to conventional anticonvulsant treatment but are well-controlled by the administration of pyridoxine (vitamin B6). Hilal and his colleagues at UBC are undertaking a research project to identify novel genetic causes in unexplained forms of pyridoxine-dependent epilepsy (PDE), a special type of PREE with an estimated incidence of 1:20,000 to 1:750,000. In most affected infants, PDE is caused by mutations in the antiquitin gene (ALDH7A1) and subsequent inactivation of α-aminoadipic semialdehyde dehydrogenase (antiquitin, ATQ).
Currently, ALDH7A1 is the only gene for which mutations are known to underlie PDE. However, locus heterogeneity has been reported in some families and other genes seem to be involved. Nearly 5% of children with a typical clinical picture of PDE harbor no detectable mutation of ALDH7A1. Identifying causative genes in such families will likely lead to improved treatment for these patients and help unravel much of the unknown about pyridoxine metabolism in the human body.
In this webinar, Hilal will cover how he and his team used whole-genome SNP genotyping, genome-wide runs of homozygosity (RoH) mapping using SVS, and whole-exome sequencing to characterize the genetic defect underlying PREE in a consanguineous Omani Arab family with two affected children who have a PDE-like clinical picture but negative ATQ biomarkers.
Background: It is often difficult to predict which newborn with HIE will develop neurological sequlae so there is an urgent need for predictors for adverse neurological outcomes in these infants. Aim of Study: To evaluate the serum levels of serum amyloid A (SAA) protein in newborns with HIE during the first week of life and after 3 and 6 months of follow up to assess its correlation with degree of HIE neurological sequlee. Patients and Methods; This case-control study was conducted on 72 infants; group (1) included 36 full term neonates diagnosed as HIE and group (2)included 36 age and sex matched, infants as a control group, Serum amyloid A by ELIZA technique was measured at post natal age of 1 and 7 days, CT scan was done in justified cases .with follow up at age of 3 and 6 months for neurological sequlee. Results: SAA protein level was elevated in the asphyxiated group in comparison to the control group at day 1 and day 7, SAA level was significantly correlated to the Sarnat scoring system of HIE. SAA level significantly differ on follow up of developmental milestone at age of 3 and 6 months. ROC curve for validity of SAA for severity of HIE at cut off point > 25μg/ml at day 1 and at cut off point > 20 μg/ml at day 7 of HIE diagnosis reported sensitivity 100% and specificity 100% .Conclusion: SAA correlates with the severity of HIE and higher SAA expression is a prognostic marker for morbidity in these infants.

Fecal egg count reduction test (FECRT) – eggs/gram at pretreatment levels versus those 10-14 days later. For practical purposes, a 90% reduction suggests successful treatment and <90% reduction may indicate resistant parasites, ineffective dose or route of administration. Egg count concerns – <200 epg are usually considered ok if BCS is also normal. If epg is rising, animals have poor bcs or other signs of ill thrift, it is possible treatment may be helpful. Epg significance is much lower when considering low or intermittently shedding parasites like trichuris, capillaria, nematodirus, or emac (even low numbers may warrant treatment).
Not ideal to blanket treat entire herd or to rotate dewormers regularly as this may lead to the development of resistant parasites. The ideal approach involves selectively treating animals based on fecal counts and clinical signs (20% of animals harbor 80% of the herd parasite burden). Targeted deworming allows one to leave a population of parasites that have not been exposed to specific drugs. Leaving these naïve parasites helps prevent selection for resistant parasites (refugia, as coined by parasitologists).
