A 35-year-old woman was in a palm oil lorry accident that caused her left femoral shaft fracture and arterial injury. She presented with a cold, painful left leg with absent pulses. Exploration revealed an oblique femoral shaft fracture with partial femoral artery injury. The bone was plated and artery repaired with end-to-end anastomosis. Post-operatively, pulses returned and she showed great improvement. The case highlights that vascular injury can occur with femoral fractures and should be monitored for even in closed fractures.
