A Study on Delay in Discharge Process, in One of Multispeciality Hospital in ...ijtsrd
Discharge delays are one of those problems that spoil the overall pleasant experience inside the hospital. The study was conducted to identify the reasons and determinants of discharge delay in acute patients care. Delayed discharge is usually associated with a patient's medical conditions, delayed health care or medical advice, delayed diagnostic services, and delayed related health services. This paper deals with the discharge delay of inpatients in a selected hospital. An annexure was prepared to see the time taken by patients from the time of discharge till they actually leave the hospital premises. The outcome that is expected from this study was to identify the reasons for the delay of discharge and to come up with suggestions to reduce them. K. Revathi | Mrs. U. Research indicates that individuals with severe mental illness are overrepresented in jails and prisons in the U.S. The prevalence of severe mental disorders in correctional facilities range between 6 and 9%, and are significantly higher than the rate of 2.8% in the general population. Individuals who exhibit symptoms of a mental illness at the time of an encounter with the criminal justice system are more likely to be arrested. Persons with serious mental illness are jailed more often than hospitalized.People with serious mental illness spend longer in jail for similar convictions than those without serious mental illness. Many offenses that lead to incarceration for individuals with SMI are minor. Individuals with serious mental illness who experience things not directly related to their illness (i.e. homelessness) are more likely to return to jail. Forensic Case management can lead to a reduction in arrests and incarceration.

GeneCIS Communicating the Patient JourneyDMF-Systems
GeneCIS is DMF Systems suite of Clinical Information Systems. GeneCIS communicates the patient journey from the referral right through to discharge. The suite consists of eReferrals, eScheduler, MediViewer, Clinical Summaries and eDischarge.
All of the solutions within the GeneCIS suite can be implemented on a modular basis to ensure you get exactly what you require. Each additional module can be bolted on seamlessly as required. This enables organisations to utilise scarce resources to meet pertinent needs and then grow from there as resources become available.
GeneCIS improves patient outcomes, significantly reduces costs and leads to superior use of scarce hospital resources.
All of the modules can be integrated with the hospital PAS to reduce data entry.
Integration services can also be utilised to revive legacy systems.

discharge planning
1. Care providers are under increasing pressure to reduce patient care costs and at the
same time improve the quality of care. CSC’s Coordinated Care Delivery (CCD) Effective
Discharge Planning solution links the systems of multiple service providers, to help plan
patient discharges in a more efficient way and free up clinicians’ time so they have more
time to provide hands on patient care.
EFFECTIVEDISCHARGE PLANNING
COORDINATED CARE DELIVERY
WHY ACT NOW?
Care providers continue to be under
increasing pressure to collaborate to
reduce patient care costs.
Effective discharge was raised as a
national priority in the report, ‘Ready
to go? Planning the discharge and the
transfer of patients from hospital and
intermediate care’ (Department of Health
2010). Effective discharge must also be
a local priority not only to ensure the
efficient use of care provider resources,
but also to ensure the best possible care
for patients.
Meeting these demands is made more
difficult where care providers are limited
by manual, paper based processes,
incompatible IT systems and other
challenges that make it difficult to share
up-to-date patient data.
As a result of having disparate systems
and business processes, they may:
• Not be using care provider resources
in the most effective manner, leading
to increased costs and patients waiting
unnecessarily to be discharged
• Lack a clear view of where the patient is in
the discharge planning process and have
difficulty managing multiple activities that
may ‘fall between the cracks’ when they
are not picked up in time
• Be unable to access the paper based
patient records kept by other providers
• Have to continually answer questions
about a patient’s status in the discharge
process
• Delay a patient’s discharge resulting
in an extended hospital stay which will
increase costs to the trust and may
increase the risk of depression or loss of
confidence and independence
• Spend more time phoning, faxing and
answering patient queries
The primary challenge care providers
face is planning post release activities in
the most proactive, efficient manner to
reduce unnecessary costs and improve
the quality of care.
BENEFITS
Our CCD Effective Discharge Planning
solution is both cost effective and easy
to implement, which means that care
providers can experience immediate
benefits.
Using a fast track approach, we will
collaborate closely with your business
management and care provider teams
to implement our solution with no major
changes to your underlying systems or
their use.
Care providers will be able to:
• Support the implementation of the ‘10
steps to effective discharge’ as identified
by the Department of Health in 2010
• Manage the Caldicott-approved
‘Discharge Planning Checklist’ and
conduct electronic audits
• Facilitate the ‘Ready to Go – No Delays’
discharge process
• Increase the amount of time nurses
spend on direct patient care by at
least 10%
• Increase cost savings by up to 10%, due
to more effective processes and more
efficient use of resources
• Improve staff morale by enabling them
to spend more time caring for patients
• Monitor the status of services required
by patients and their care providers to
enable them to return home or transfer
to another care setting
Our CCD Effective
Discharge Planning
solution provides
the care teams
involved in discharge
planning with a
single view of all
required activities
and their status
