Ward Handover enables a more efficient handover of patients between shifts enabling a more effective patient discharge process. The solution is split into two parts, based around 6PM's CareSolutions database:
Ward Handover System:
A proven ward handover application which enables clinical staff to maximize their care time whilst delivering a single view of the patient by allowing them to enter the discharge notes into a single location that may be accessed by all clinical teams involved in the patient's care – including doctors, ward nurses, specialist nurses and Allied health Professionals. The system therefore allows predictability of bed utilization to maximize their occupancy and assists the bed management team in proactive assessment whilst reducing overall costs of related activities.
Patient Discharge Reporting:
The second part of the solution is real time reporting. The solution takes feeds from the Trust's CRS application every 15 minutes regarding patient status and predicted discharge dates which is then used to create a number of reports for ward staff regarding the workload and patient status. These reports can then be viewed either by ward staff or by consultants.

How is Digitalization Helping in Healthcare Management.pdfbasilmph
Healthcare management may interest someone who wants to contribute significantly to
healthcare without having direct patient contact. A person can play a significant role in the medical field without working in an operating room, delivering medication, or directly caring for patients.
When the pandemic happened, this prominent healthcare provider from the Tri-State area couldn’t rely on their painfully slow and inflexible systems to create meaningful customer experiences. They partnered with TVS Next to rapidly modernize their systems and increase their appointment booking capability by 2000%, winning the hearts of the community they served.

SmartStream, as an EMR extender, searches for whether the patient has completed a behavioral health assessment
whenever he schedules an appointment or his insurance is verified. If no current assessment exists, SmartStream
will send a notification to the patient to complete a behavioral health assessment, M3, or ‘Monitor My Mood’. The notification will include an automated hyper-link to M3 and is sent using secure email or secure mobile messaging. The objective is for the patient to complete the behavioral health assessment BEFORE their scheduled office visit.
National statistics show that as of 2010, “85% adults and 75% teens own cell phones”. Plus “Ethnic minority groups

Successfully Implementing a Federally-Facilitated ExchangeCognosante
The Affordable Care Act directs the Secretary of Health and Human Services to establish and operate a Federally-Facilitated Exchange in any state that does not elect to establish a state-based exchange. As the deadline for state submission of a state blueprint and declaration letter approaches, many states are faced with implementing a FFE by the statutorily required enrollment date of October 1, 2013. While a FFE allows states to meet federal timeline mandates, its implementation provides states with limited options and flexibility. Cognosante's latest white paper helps states understand how to successfully implement a Federally-Facilitated Exchange.

Improve EPSDT Service Quality Through Meaningful Use of ITCognosante
The Early and Periodic Screening, Diagnostic and Treatment (EPSDT) benefit provides comprehensive and preventive health care services for children under age 21 who are enrolled in Medicaid. A legal standard of coverage and access for low-income children. EPSDT has remained a central component of Medicaid, because of the operational and financial capacity it gives states to create appropriate pediatric health care systems (its ability to finance early, preventive, and on going healthcare for children at medical and social risk remains unparalleled in public or private health insurance). This paper provides an ntroduction to EPSDT, an overview of current EPSDT service and quality monitoring, and how EPSDT can be better supported through Meaningful Use of health IT.
MMIS Solutions: A New Approach to Forming MMIS Project TeamsCognosante
It’s not just about filling the boxes on your organization chart – it’s about forming a project team that ensures the success of your MMIS project. Among the most important phases of the MMIS Design, Development and Implementation (DDI) project is the planning phase, and within the planning phase, there is no more important step than establishing a strong MMIS DDI project team. The project team represents the most important contributing factor to the success or failure of the MMIS DDI project. While the approach to forming an MMIS DDI project team may seem similar to other state information technology (IT) projects, it is far from the norm. The size, complexity and risk associated with MMIS DDI projects require a greater level of detailed planning and creativity when forming the project team .
How Should Medicaids Respond to the ICD-10 Postponement?Cognosante

The news came in response to political pressures to postpone the original October 2013 deadline and ongoing industry concerns related to the bumpy transition to the new 5010 versions of electronic transactions. This paper outlines three key actions states can take to ensure the continuity of their ICD-10 transition efforts in the 'extra year.'
Consumer Assistance and Health Insurance Exchanges: Analysis of Options Avail...Cognosante
The enactment of the Patient Protection and Affordable Care Act (ACA) and the subsequent upholding of the majority of the law by the United States Supreme Court will continue to result in significant changes to the way Americans access health insurance. A key feature of the ACA, and one that may have the biggest impact on state budgets and personnel, is the requirement to have a Health Insurance Exchange (HIX) available and functioning by government-defined deadlines. The Consumer Assistance function of the exchange provides the primary means by which applicants and consumers receive information about the HIX and their options, as well as make health plan enrollment choices.
A Roadmap for States Using the Federally Facilitated Exchange Cognosante
The passage of the Patient Protection and Affordable Care Act (ACA) resulted in significant changes to the ways Americans will access and purchase health insurance. A key feature of the ACA, and one that may have the biggest impact on state budgets and personnel, is the requirement for states to have a health insurance exchange (HIX) up and functioning by January 1, 2014.
This paper outlines the steps which states must take should they elect the Federally Facilitated Exchange (FFE) model and includes a high-level roadmap for implementing the FFE at the state level. Cognosante summarizes the recommended actions for states to adopt the FFE model and highlights key areas and actions required to achieve successful implementation.

Antibiotic Stewardship by Anushri Srivastava.pptxAnushriSrivastav
Stewardship is the act of taking good care of something.
Antimicrobial stewardship is a coordinated program that promotes the appropriate use of antimicrobials (including antibiotics), improves patient outcomes, reduces microbial resistance, and decreases the spread of infections caused by multidrug-resistant organisms.
WHO launched the Global Antimicrobial Resistance and Use Surveillance System (GLASS) in 2015 to fill knowledge gaps and inform strategies at all levels.
ACCORDING TO apic.org,
Antimicrobial stewardship is a coordinated program that promotes the appropriate use of antimicrobials (including antibiotics), improves patient outcomes, reduces microbial resistance, and decreases the spread of infections caused by multidrug-resistant organisms.
ACCORDING TO pewtrusts.org,
Antibiotic stewardship refers to efforts in doctors’ offices, hospitals, long term care facilities, and other health care settings to ensure that antibiotics are used only when necessary and appropriate
According to WHO,
Antimicrobial stewardship is a systematic approach to educate and support health care professionals to follow evidence-based guidelines for prescribing and administering antimicrobials
In 1996, John McGowan and Dale Gerding first applied the term antimicrobial stewardship, where they suggested a causal association between antimicrobial agent use and resistance. They also focused on the urgency of large-scale controlled trials of antimicrobial-use regulation employing sophisticated epidemiologic methods, molecular typing, and precise resistance mechanism analysis.
Antimicrobial Stewardship(AMS) refers to the optimal selection, dosing, and duration of antimicrobial treatment resulting in the best clinical outcome with minimal side effects to the patients and minimal impact on subsequent resistance.
According to the 2019 report, in the US, more than 2.8 million antibiotic-resistant infections occur each year, and more than 35000 people die. In addition to this, it also mentioned that 223,900 cases of Clostridoides difficile occurred in 2017, of which 12800 people died. The report did not include viruses or parasites

1. How HealthInsight Nevada and HealtHIE Nevada are
Benefitting from Using SmartStream™ for Transitions of Care
This confirmed to Erick Maddox, HIT Manager for HealthInsight Nevada, that his
organization was on the right track. HealthInsight Nevada, manages the HealtHIE Nevada
HIE, the first private, non-profit, community-based HIE in Nevada, who just went live with
a project to study the effectiveness of using alerts to improve transitions of care.
48 States. 25+ Years. Unsurpassed Experience.
®
At an ONC meeting in January 2014, a question arose, “How do we
make alerting from a Health Information Exchange (HIE) a reality
within our community?”
Customer Story
Current Challenges
In January 2013, CMS issued guidelines to
improve Transitions of Care and to reduce 30-
day hospital readmissions. These guidelines
render eligible physicians to receive incentive
payments through timely follow-up with patients
after discharge from the hospital or emergency
room. This ensures a seamless transition of
care. The barrier to physicians receiving the
incentives is that they are often not aware their
patients were discharged from the hospital which
makes it challenging for follow-up. Additionally,
current alert capabilities stop short of workflow
integration thus making them less usable for
physicians.
SmartStream
for Transitions of Care
HealthInsight Nevada and the HealtHIE Nevada
HIE are using Cognosante’s SmartStream, an
Intelligent Health Information Delivery system.
This allows physicians to move beyond simple
notifications and instead receive actionable
information in real time. Unlike other solutions
available on the market, SmartStream includes
workflow enablement features allowing
HealthInsight Nevada to focus on physician
efficiencies throughout the transition of care
process for a patient.

Customer Story
HealthInsight Nevada also serves as the Regional Extension Center and the
Quality Improvement Organization in the state. With their vast knowledge of
clinical and administrative processes, and experience in deploying quality
initiatives, they understood that providing notifications of significant events was
simply not enough. HealthInsight Nevada began working with Cognosante to
design a system to bring significant value to the physicians they serve. Armed
with input from their clinical team, the project team designed a solution with
the physician’s workflow in mind.
How it Works
When a physician’s patient is discharged from the hospital, SmartStream™
extracts data from a real-time feed provided by the HIE. Using this data,
SmartStream then generates and sends notifications to physicians by
Direct secure message. Simultaneously, the physician is notified by text or
by unsecured email with a link to the physician’s Direct secure account. A
key advantage is the physician will immediately know if there is important
information in his/her Direct secure account.
After receiving an alert, physicians are asked to acknowledge what action
was taken. Using this control feedback mechanism, SmartStream tracks
action taken and sends reminders, as needed, from the time of discharge to
completion of the follow-up appointment.
Dashboards are available at any time to view the volume of discharges
versus alerts sent out, how physicians responded, and within what time
period. Reports providing historical information are available, and are critical
in evaluating the effectiveness of the system and how it can be optimized.
Implementation Success
The pilot project went live in early January 2014 at five hospitals in the Las
Vegas area. The expected benefits of deploying the SmartStream Transition
of Care solution are:
• Increased Compliance meeting CMS guidelines regarding patient follow-up
after discharge
• Increased Revenue Generation helping physicians qualify for CMS
reimbursement. Additionally, the Transition of Care solution helps hospitals
avoid costly CMS penalties
• Increased Quality of Care ensuring proper follow-up upon patient discharge
• Reduced Costs automating and streamlining manual processes currently
employed by hospitals and physicians to coordinate care and prevent 30-day
hospital readmissions
• Increased Operational Efficiency pushing critical patient information
to physicians in real-time using a closed loop system to manage tasks
throughout the Transition of Care process
• Increased HIE Usage bringing the data in the HIE alive at the point of care
“The physicians participating
in the study are excited. This
excitement has sparked interest
from other physicians who want
to be included”

HIT Project Coordinator -
Physician Liaison.
