Telepractice is routinely used for provision of diagnostic and rehabilitative hearing services. Validation of procedures including audiometry, video-otoscopy, cochlear implant and hearing aid programming and rehabilitation is reported in the literature. Speech perception testing using telepractice is an audiological procedure that is yet to be validated. The current study has developed an APP that can be used to provide on line assessment of speech perception to evaluate device suitability (hearing aids or cochlear implants) and to monitor progress with devices over time.

Abstract
Telepractice is routinely used for provision of diagnostic and
rehabilitative hearing services. Validation of procedures
including audiometry, video-otoscopy, cochlear implant and
hearing aid programming and rehabilitation is reported in
the literature. Speech perception testing using telepractice

Clinical relevance
How can we ensure it is effective and
efficient?
Effectiveness and efficiencies in telepractice must, at least,
emulate those found in traditional models of service
delivery (MOSD) involving key stakeholders on site within
the clinic. The overarching assumption when using
telepractice is that the service provided will be as efficient
and as effective as a face-to-face service and will adhere to
a code of ethics and scope of practice

Speech discrimination testing at word and sentence
level in quiet and noisy listening conditions is
performed regularly throughout the journey with
hearing devices. The APP will measure a clients
speech perception in real time over telepractice
(synchronously) or asynchronously whereby the
client can upload their responses to be scored by
their clinician at a later date or time.
The clients scores will be used to determine action
that may or may not be required in their device
management and will preclude the need to travel to
a clinic.

Evaluation of the APP requires a battery of
procedures considering stakeholder characteristics
(eg age, motivation, cognition) efficiency (eg cost
effectiveness, and effectiveness (clinical and
qualitative outcomes).
This study will involve 40 subjects varying in age and
hearing levels (including device use).

The Apple iPad platform has been selected for use
because of its stability and consistency of its microphone
specifications. Materials are CNC words and BKB like
sentences from the AusTIN test. The APP design
enables use of other materials as required*. Direct input
is not used to enable a range of devices to be tested.
The APP has been piloted for ease of use and
instructions. Once the above considerations have been
solved and embedded in the APP, the study will be
undertaken to validate its use.

A study collected data before and after decentralizing three nursing units to evaluate the impact. Walking distance for nurses also increased with the decentralized design. The consistency of these changes across the three units provides evidence that decentralization can impact how nurses spend their time and perform tasks.

The VA case demonstrates that health IT, performance measurement, quality improvement efforts, and integrated service delivery and financing can drive transformational change in a large health system.

UCLA CTSI and University of Minnesota Cross-Institutional Award Projects Principal Investigators: Hector Rodriguez (UCLA) and William Riley (University of Minnesota) Large-scale federal investments in health information technology (HIT) are intended to spur health care providers and organizations to share patient information to better coordinate and improve quality of care. However, the uptake of HIT has lagged in ambulatory care settings that care for high proportions of low-income patients. Our pilot study seeks to generate knowledge about facilitators and barriers to the spread of electronic health information exchange (HIE) for improving quality of care among underserved populations. We have established partnerships with two health care organizations: Citrus Valley Health Partners (CVHP), a provider network serving many underserved patients in the East San Gabriel Valley, Calf., and the Federally Qualified Health Center Urban Health Network (FUHN), a group of ten clinic organizations serving the Minneapolis-St. Paul area. We are conducting key informant interviews of physicians, front office staff, IT personnel, and executives. Through these interviews, we are learning about the barriers to electronic exchange of health information within the clinics and practices, between these sites, and at city-wide or regional levels. This multi-level framework elucidates the opportunities and challenges for ambulatory care practices serving underserved populations in adopting and sustaining HIT.

This study assessed the communication skills of 127 family medicine residents across 6 residency programs using the Communication Assessment Tool (CAT), a 14-item patient satisfaction survey. Overall residents received a mean rating of 69.7% excellent from 1880 surveys. Residents scored highest on paying attention, respect, and care/concern. Scores differed by year of training but not gender or native language. The CAT provides an empirical measure of communication competency and can differentiate resident skills and track changes over time. Larger samples and continued benchmarking are needed.

This dissertation investigated the effects of amplitude compression on speech perception using measures of attention and intelligibility. Two experiments were conducted using dual-task paradigms to measure listening effort associated with compressed speech. Experiment 1 found that compressed speech decreased performance on a secondary visual motor tracking task compared to uncompressed speech, indicating increased listening effort. However, compressed speech did not decrease primary word recognition performance. Experiment 2 found that a secondary visual lexical decision task did not interfere with the primary word recognition task and did not reduce dual-task performance for compressed or uncompressed speech. The dissertation explored alternative measures to traditional intelligibility to provide a more comprehensive understanding of how compression affects speech perception.

This study aimed to develop a wireless digital broadcasting with voice alarm and text-to-speech (TTS) feature that can wirelessly transmit audio signals from the main campus to its satellite campus over long distances. Also, the prototype has a TTS feature that can offer high-quality and stable speech. The main campus is the Colegio de San Antonio de Padua (CSAP) located at Guinsay, Danao City Cebu, Philippines with a satellite campus at Barangay Suba, Danao City, with a distance of not less than four kilometers and not greater than five kilometers from the main campus. The researcher used the descriptive developmental method of research as the systematic study of designing, developing, and evaluating programs that must meet the criteria of internal consistency and effectiveness. The rapid prototyping model was used during the system development while the criteria in McCall’s Factor Model were used to test the system according to its usability, applicability, and efficiency.

Speech recognition techniques are one of the most important modern technologies. Many different systems have been developed in terms of methods used in the extraction of features and methods of classification. Voice recognition includes two areas: speech recognition and speaker recognition, where the research is confined to the field of speech recognition. The research presents a proposal to improve the performance of single word recognition systems by an algorithm that combines more than one of the techniques used in character extraction and modulation of the neural network to study the effects of recognition science and study the effect of noise on the proposed system. In this research four systems of speech recognition were studied, the first system adopted the MFCC algorithm to extract the features. The second system adopted the PLP algorithm, while the third system was based on combining the two previous algorithms in addition to the zero-passing rate. In the fourth system, the neural network used in the differentiation process was modified and the error ratio was determined. The impact of noise on these previous systems.
