'Use of linked health care data for research: experiences with the Hampshire ...Health Innovation Wessex
The document discusses the Hampshire Health Record (HHR), a database containing pseudonymized linked extracts of primary and secondary care records for about 1.3 million people in Hampshire, UK. It describes several uses of the HHR for research, including evaluating interventions, understanding variations in outcomes for conditions like COPD, and studying antibiotic prescribing in nursing homes and the incidence of chronic kidney disease and acute kidney injury. The HHR is seen as a valuable resource but also has some limitations like missing or incomplete data. Ongoing research uses the HHR to study topics like dementia pathways, cancer survivorship, and multimorbidity. Healthcare professionals report that the HHR enables informed decision making and provides a more comprehensive view of patients.
This document summarizes research on the association between smoking and two infectious diseases: tuberculosis and Legionnaires' disease. It reviews multiple studies that find smoking increases the risks of active tuberculosis, relapse after treatment for tuberculosis, and mortality from tuberculosis. The document also analyzes data from Barcelona on relationships between smoking status and tuberculosis outcomes. Additionally, it presents information on Legionnaires' disease and reviews a French case-control study that identified older age and smoking as risk factors.
BioSHaRE: Risk stratification using genomic and lifestyle information - Samul...Lisette Giepmans
This document discusses using genomic and lifestyle data for cardiovascular disease risk stratification. It summarizes that genome-wide studies have provided unprecedented information about genetic backgrounds of complex diseases. Large-scale prospective data combining genomic screening, lifestyle factors, and long-term health outcomes could be used to develop personalized risk algorithms and apps to communicate risk. Pilot studies in Finland have shown genetic risk scores can reclassify risk and identify additional individuals who could benefit from prevention.

Smoking is linked to worse outcomes in men undergoing radical prostatectomy for prostate cancer. The study found that smokers had higher cancer volumes and greater amounts of high-grade cancer compared to non-smokers. Heavy smokers, defined as those with a 20 pack-year or more history of smoking, faced a higher risk of biochemical recurrence following surgery. Smoking independently predicted larger cancer volumes, more high-grade cancer, and increased risk of PSA levels rising after surgery. The researchers conclude that smoking history should be considered in risk assessment models, as quitting smoking may help prostate cancer survival.
Type 1 diabetes and alopecia areata are organ-specific autoimmune diseases sharing a number of striking
similarities. Careful consideration of these may forward the clinical and research goals of both fields.
This document discusses lung cancer biomarkers and new methods for their rapid detection. It notes that lung cancer has poor patient outcomes and biomarkers could help improve patient assessment. The document presents two potential cellular biomarkers - Lamin-A/C and EGFR - and describes developing a new tissue microarray method that reduces biomarker processing time from 3 hours to 12 minutes per sample. The goal is to implement improved biomarker detection methods.
This report examined the association between renal failure admissions (RFA) to hospitals in Victoria, Australia and socioeconomic disadvantage across 79 local government areas from 2011-2014. The results showed that RFA rates varied inversely with socioeconomic disadvantage and the most disadvantaged areas had RFA rates 2.4 times higher than the least disadvantaged areas. Areas with high RFA rates also had high rates of other conditions like heart failure and COPD. The variations in RFA rates likely reflect factors like access to adequate care, delayed referral to nephrology services, and lack of understanding about the disease among patients. Addressing chronic kidney disease and renal failure remains a public health challenge.

The AVERT and CASSINI trials evaluated the efficacy and safety of apixaban and rivaroxaban for preventing venous thromboembolism in ambulatory cancer patients at high risk. The trials found that both apixaban and rivaroxaban significantly reduced the risk of VTE compared to placebo, with acceptable rates of major bleeding. However, a large percentage of patients discontinued the anticoagulant therapy. While the results support the use of DOACs for VTE prevention in selected high-risk ambulatory cancer patients, more research is needed focusing on specific cancer types and chemotherapy regimens. Currently, low molecular weight heparin remains the standard for acute medically ill or surgical cancer patients, while

Gallstones are crystal like collections that formed by merging of normal and abnormal gallbladder content. Usually there are two types of gallstones exist i.e. cholesterol stones & pigment stones. The current paper focuses on symptoms of the disease, major cause for the disease and on the treatments that majority of patients preferred. For this purpose, sample of size 170 data from different hospitals in Multan is collected by using convenience sampling. Main demographic factors involved in this study are Gender, Age group, marital status for patients of GSD. Frequency distribution has been formed for these different demographic and social factors and a bar chart is constructed for differentiating between gender as gender is also an important factor in GSD. For weight factor, paired t test is applied to see the difference between before and after weight after having treatment. Findings show that 67 percent people prefer govt. hospitals because of the people suffering from this disease were from backward areas or villages & their income not meet to pay the private hospitals expense.

This study aimed to determine the prevalence of cardiovascular diseases among diabetes patients attending Kampala International University Teaching Hospital. The study found a cardiovascular disease prevalence of 54.1% among the 98 diabetes patients. Hypertension was the most common complication at 53%, followed by diabetic retinopathy at 14%. Poor blood sugar monitoring and control was also observed. The study concludes that the cardiovascular disease prevalence among diabetes patients is high, calling for improved diabetes management and care.
Chronic Kidney Disease Challenges and New SolutionsViewics
A critical population management challenge concerns chronic kidney disease (CKD), which impacts about half of the Medicare population and of diabetics. More than 50% of adults over 30 years of age are likely to develop CKD during their lifetime, and the prevalence of CKD is expected to climb over the next 15 years. Current CKD management is variable and suboptimal, as categorizing the very heterogeneous CKD patient population into risk cohorts for purposes of appropriate treatment is inaccurate. Without accurate risk classification, many patients are over-treated, leading to wasted expenses and adverse events, while others are not identified in time to receive interventions that change the course of the disease.
A new algorithm has been created that predicts patients’ risk of renal failure based on a specific set of laboratory tests combined with patient age and gender. Validated by more than 720,000 patients spanning 30 countries, it can reliably predict a patient’s risk of experiencing renal failure requiring dialysis or transplant. Studies show that a lab-based analytics program that incorporates this algorithm with care protocols, dashboards, and educational patient reports can generate substantial savings and improved outcomes for ACOs and health systems.

The Natural History of Liver Fibrosis Progression Rate in Hepatitis C Infecti...aranush
The study examined factors associated with the rate of liver fibrosis progression in 72 patients with hepatitis C virus (HCV) infection who had more than one liver biopsy. Approximately one-third of patients showed fibrosis progression over a mean of 4 years between biopsies. Demographic factors did not predict progression rate. A history of failed interferon therapy was associated with a trend towards a lower progression rate, although this was not statistically significant. Co-morbid illnesses, social factors, and lab values did not correlate with progression rate. The overall mean fibrosis progression rate was low.

Watching Too Much TV Raises the Risk of Blood Clots, Study SaysMichaelSmithh
A recent study featured in the American Heart Association’s Scientific Sessions 2017 found that prolonged exposure to television raises the chances of developing venous thromboembolism (VTE). Experts say that results might help researchers find a way to curb cases of VTE, which is a growing health concern that affects about 600,000 people in the country.
HCV Treatment Access across Europe - Raquel PeckReShape
The document discusses hepatitis C virus (HCV) treatment access across Europe. It notes that the World Health Organization and Global Hepatitis Summit have set goals to eliminate viral hepatitis by 2030. The document then provides statistics on HCV prevalence and treatment rates across the European Union, with over 3.6 million estimated viremic infections but only 133,000 patients treated in 2015. Case studies are presented on HCV elimination efforts in countries like France, Germany, Portugal, and Australia. The document argues for more active screening, prevention efforts, expanding treatment eligibility criteria, and ensuring strong patient advocacy to work towards HCV elimination in the EU.
1. Cardiogenic shock is defined as a state of acute cardiac hemodynamic instability resulting from disorders impairing the myocardium, valves, conduction system, or pericardium.
2. An analysis of hospital data showed an increase in cardiogenic shock from 6% to 10% of overall patients presenting with STEMI, and from 7% to 12% of patients over age 75. In-hospital mortality decreased slightly but remained high at 34% overall and 55% for patients over age 75.
3. The most common underlying causes of cardiogenic shock are acute coronary syndrome (81% of cases) and acute decompensation of chronic heart failure (30% of cases).
This study analyzed data from the 2004 National Long Term Care Survey to compare cognitive and functional impairment between enrolled and non-enrolled veterans aged 65 and older. It found that enrolled veterans had higher rates of cognitive and physical disability than non-enrolled veterans. Specifically, new enrollees between 1999-2004 had greater disability levels than previously enrolled veterans. The findings suggest the VA enrolls a disproportionate share of cognitively and functionally impaired veterans, with implications for understanding future demands on long-term care benefits.
Hashimoto's thyroiditis is an autoimmune condition where the immune system attacks the thyroid gland, reducing hormone production and commonly causing hypothyroidism. This prospective study investigated whether the known link between Hashimoto's and thyroid cancer is specific to certain thyroid antibodies. The study found that elevated thyroglobulin antibodies (TgAb) were a risk factor for thyroid cancer in patients with thyroid nodules, but thyroid peroxidase antibodies (TPOAb) showed little variation between cancerous and benign nodules. The results suggest the association between Hashimoto's and thyroid cancer is specific to TgAb levels.

An assistant helps the photographer during a photo shoot. They
help with things like:
- Setting up lighting and equipment.
- Helping the actors/models get into position.
- Handing props and costumes to the photographer.
- Taking notes and keeping track of what photos have been taken.
- Helping to move lighting or backgrounds between shots.
Having an assistant is important as it allows the photographer to focus solely
on taking the best photos possible. It speeds up the process.

The document provides an overview of the key players in Canada's capital markets and the investment process. It identifies institutional investors like pension funds and mutual funds as the largest pool of capital, which gets invested through asset managers. The main barriers to integrating sustainability into capital markets are: 1) environmental and social impacts are often not reflected in prices due to market failures; 2) the short-term focus of most investors; 3) lack of consistent and comparable sustainability data; and 4) limited understanding of sustainability risks. The document also discusses some opportunities to help mobilize more capital towards sustainable economic activities.

This presentation was delivered at Wessex AHSN's 2016 conference - Predict, Prevent, Adapt.
This document provides information on alcoholic liver disease, including its incidence, progression, risk factors, pathogenesis, clinical findings, diagnostic testing, differential diagnosis, prognosis, and screening tools. Some key points:
- Heavy alcohol use can lead to fatty liver in 90-100% of people within 10 years, while only 10-35% will develop alcoholic steatohepatitis and 8-20% will progress to cirrhosis.
- Risk is increased by factors like younger age of onset, female sex, certain ethnicities, coinfection with hepatitis B or C, iron overload, and obesity.
- Alcohol causes liver injury through mechanisms like centrilobular hypoxia, neutrophil infiltration, antigen formation,
The document discusses renal failure and its relationship to cardiovascular disease. It provides statistics on the prevalence, incidence, and mortality rates of renal failure in the US and Australia. It then covers topics such as the structure and function of the kidneys, classification of renal failure, its effects on the cardiovascular system, and approaches to treatment including dialysis, transplantation, and their risks.

This document discusses the screening, management, and treatment of chronic kidney disease. It notes that chronic kidney disease affects over 10% of US adults and risk increases with age. Diabetes and hypertension are the most common causes of end-stage renal disease. It recommends referring patients with decreased eGFR and increased albuminuria to a nephrologist. The management of complications such as hypertension, dyslipidemia, anemia, acidosis, and bone mineral disorders is also covered.
This document discusses alcoholic liver disease (ALD). It notes that ALD ranges in severity from fatty liver to alcoholic hepatitis to cirrhosis. Risk factors include the amount of alcohol consumed daily and genetically. Diagnosis involves blood tests like GGT and liver biopsy. Severe alcoholic hepatitis has high short-term mortality and is treated with corticosteroids or pentoxifylline to reduce inflammation. Prognosis can be predicted using scores like Maddrey DF and management involves lifestyle changes like abstaining from alcohol and adequate nutrition.
The document discusses hepatic encephalopathy, a neurological syndrome caused by liver dysfunction. It covers the pathogenesis, which involves neurotoxins like ammonia crossing the blood brain barrier and disrupting neurotransmitter levels. Symptoms range from mild confusion to coma and can be precipitated by factors that increase ammonia production or permeability of the blood brain barrier. Treatment focuses on managing precipitating factors and restricting protein intake to control ammonia levels.
Cirrhosis results from various mechanisms of liver injury and inflammation that lead to fibrosis and scarring of the liver. This scarring distorts the liver's vascular architecture and increases resistance to blood flow, resulting in portal hypertension. The three main causes of cirrhosis in developed countries are hepatitis C infection, alcohol misuse, and non-alcoholic fatty liver disease. Cirrhosis progresses from compensated to decompensated stages and can lead to complications like ascites, bleeding varices, hepatic encephalopathy, and ultimately liver failure without treatment.
1) Alcohol-related liver disease (ARLD) places a significant burden on healthcare systems. Patients with ARLD often have multiple hospital admissions before death from liver failure or complications.
2) Early identification of alcohol misuse and intervention can help reduce further liver damage if patients change their drinking behavior. However, screening and treatment of ARLD remains suboptimal in many hospitals.
3) Prognosis in ARLD depends on severity of liver disease and presence of complications. Scores like MELD and CLIF-SOFA can help predict outcomes but assessment of social factors is also important.
Celiac disease. One in 132 Americans has it. We know about the malabsorption, the anemia, the osteoporosis associated with celiac disease. But what of associations with neurological disease, reproductive health, and other organ systems? What DON'T you know about this common condition?
Abnormal LFTs rate of deco and NAFLD.pptxzeus70441
1) The document discusses abnormal liver function tests (LFTs) and non-alcoholic fatty liver disease (NAFLD) in a population study conducted in East London. The study found that 31.6% of adults had LFTs tested, and of those 14.5% had at least one abnormal result.
2) NAFLD is discussed as a common cause of abnormal LFTs and liver disease. Risk factors for NAFLD include certain ethnicities, diabetes, hypertension, and increased BMI. NAFLD can progress to non-alcoholic steatohepatitis (NASH), which in some cases can lead to cirrhosis or liver cancer.
3)
The document discusses the pathophysiology of hepatic encephalopathy secondary to liver cirrhosis. It describes how ammonia produced in the gastrointestinal tract is normally converted to urea by the liver, but when liver function is impaired, ammonia levels rise and can lead to hepatic encephalopathy. Precipitating factors include constipation, increased protein intake, and other sources of increased ammonia. An ecologic model is presented showing the interaction between host factors like the patient's age, sex, behaviors, and environment in predisposing them to developing liver cirrhosis and subsequent hepatic encephalopathy.

Alcoholic hepatitis is a common condition caused by heavy alcohol consumption that carries a high mortality risk. Key aspects include:
- Presentation includes jaundice, fever, tender hepatomegaly and abnormal liver function tests.
- Severity is assessed using Maddrey's discriminant function, with scores over 32 indicating poor prognosis.
- Treatment of severe cases involves corticosteroids to reduce immune-mediated injury, pentoxifylline to inhibit tumor necrosis factor production, and nutritional support to address negative nitrogen balance and increased energy needs.
- Corticosteroids and pentoxifylline have been shown to improve short-term survival in randomized controlled trials for patients with severe disease.

This document evaluates laboratory tests for diagnosing cirrhosis and detecting alcohol use in patients with alcoholic cirrhosis. It compares test results between 1578 patients with and without alcoholic cirrhosis, and between cirrhotic patients who were recently drinking or abstinent. International normalized ratio (INR) and bilirubin best distinguished cirrhotic from non-cirrhotic patients, with areas under the ROC curve of 0.91 and 0.88 respectively. Gamma glutamyl transferase (GGT) and aspartate aminotransferase (AST) detected recent drinking in cirrhotic patients using appropriate cut-off values, even in those without ascites. INR and bilirubin are most accurate for diagn
Mr. DB, a 51-year-old man with cirrhosis and ascites, presented to the emergency room with tense ascites. He underwent a paracentesis that removed 7 liters of fluid and was prescribed diuretics and supplements. Cirrhosis is the final stage of chronic liver disease characterized by scarring and loss of liver function. Common complications include ascites, edema, and hepatic encephalopathy. Nutrition therapy aims to meet increased calorie and protein needs while restricting sodium and fluid intake as needed.

Overall life span (LS) was 1671.7±1721.6 days and cumulative 5YS reached 62.4%, 10 years – 50.4%, 20 years – 44.6%. 94 LCP lived more than 5 years without cancer (LS=2958.6±1723.6 days), 22 – more than 10 years (LS=5571±1841.8 days). 67 LCP died because of LC (LS=471.9±344 days). AT significantly improved 5YS (68% vs. 53.7%) (P=0.028 by log-rank test). Cox modeling displayed that 5YS of LCP significantly depended on: N0-N12, T3-4, blood cell circuit, cell ratio factors (ratio between cancer cells-CC and blood cells subpopulations), LC cell dynamics, recalcification time, heparin tolerance, prothrombin index, protein, AT, procedure type (P=0.000-0.031). Neural networks, genetic algorithm selection and bootstrap simulation revealed relationships between 5YS and N0-12 (rank=1), thrombocytes/CC (rank=2), segmented neutrophils/CC (3), eosinophils/CC (4), erythrocytes/CC (5), healthy cells/CC (6), lymphocytes/CC (7), stick neutrophils/CC (8), leucocytes/CC (9), monocytes/CC (10). Correct prediction of 5YS was 100% by neural networks computing (error=0.000; area under ROC curve=1.0).

Cell Therapy Expansion and Challenges in Autoimmune DiseaseHealth Advances
There is increasing confidence that cell therapies will soon play a role in the treatment of autoimmune disorders, but the extent of this impact remains to be seen. Early readouts on autologous CAR-Ts in lupus are encouraging, but manufacturing and cost limitations are likely to restrict access to highly refractory patients. Allogeneic CAR-Ts have the potential to broaden access to earlier lines of treatment due to their inherent cost benefits, however they will need to demonstrate comparable or improved efficacy to established modalities.
In addition to infrastructure and capacity constraints, CAR-Ts face a very different risk-benefit dynamic in autoimmune compared to oncology, highlighting the need for tolerable therapies with low adverse event risk. CAR-NK and Treg-based therapies are also being developed in certain autoimmune disorders and may demonstrate favorable safety profiles. Several novel non-cell therapies such as bispecific antibodies, nanobodies, and RNAi drugs, may also offer future alternative competitive solutions with variable value propositions.
Widespread adoption of cell therapies will not only require strong efficacy and safety data, but also adapted pricing and access strategies. At oncology-based price points, CAR-Ts are unlikely to achieve broad market access in autoimmune disorders, with eligible patient populations that are potentially orders of magnitude greater than the number of currently addressable cancer patients. Developers have made strides towards reducing cell therapy COGS while improving manufacturing efficiency, but payors will inevitably restrict access until more sustainable pricing is achieved.
Despite these headwinds, industry leaders and investors remain confident that cell therapies are poised to address significant unmet need in patients suffering from autoimmune disorders. However, the extent of this impact on the treatment landscape remains to be seen, as the industry rapidly approaches an inflection point.
