This document proposes developing an "ACT Learning Network" to leverage knowledge among primary care leadership in a health system and create a culture of change. There are a lot of emergent knowledge and skills
developed in response to dyad leaders’ daily
challenges. These skills can be properly captured
and adapted for scale within the health system.
