This document summarizes several clinical trials conducted by the NCIC CTG on breast cancer treatment:
1. Trial MA.5 from 1989-1993 compared CEF vs CMF chemotherapy and found improved DFS and OS with CEF.
2. Trial MA.12 from 1993-2000 randomized premenopausal women to 5 years of tamoxifen or placebo after chemotherapy. It found improved but not statistically significant DFS and OS with tamoxifen. Compliance was suboptimal.
3. Correlative studies explored prognostic biomarkers like OPN and insulin resistance from several trials. OPN levels correlated with survival and may help determine management.

Up to 20% of all invasive female breast cancer diagnoses are defined by the clinically significant absence of three hormone receptors i.e., ER, PR and HER2. This group of highly heterogeneous tumors exhibit aggressive growth patterns and are known as TNBCs. Although most TNBCs are ductal carcinomas (no special types), the identification of specific histologic/molecular subtypes potentially open up further modes of treatment for a disease that has thus far mainly been treated with cytotoxic chemotherapies. Biologic features in tumor subsets that carry such implications include BRCA pathway inhibition, increased tumor infiltrating lymphocytes (TILs), detection of other biomarkers paving the way for immunotherapies such as elevated PD-L1 expression and AR expression. Here, is some of the relevant information about TNBCs.

The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.

35% by 3.5 yrs)
• in a cohort of 1633 Scottish women, longer duration of adjuvant T
was associated with improved OS

• compliance with T vs P was suboptimal in both arms,
only 52% completing 5 yrs of treatment
• poor compliance is likely to have influenced the efficacy of
treatment in this trial, and other similar studies
• these findings have implications for use of oral anti-cancer
medications in many settings

more from one of the 3 types of chemotherapy

33. Lessons learned
• there are many excellent reasons to participate in
Cooperative Group activities (vs Industry)
• make active contributions – these will be noticed and recognized
• if invited to be a Principal Investigator – make your mark!
• incorporate a tumor specimen bank into the clinical trial protocol
• tumor banks need to be well-organized and adequately funded
• be open/search for opportunities for trial-related
translational research studies
• cultivate collaborations with scientists in your own institution,
but also establish networks across the country
