This document provides guidelines for diagnosing and managing blunt cardiac injury (BCI) according to the Eastern American Society of Trauma. It states that BCI is commonly caused by motor vehicle crashes and other deceleration injuries. While an abnormal electrocardiogram (ECG) or elevated troponin I levels indicate possible BCI, a normal ECG alone does not rule it out. The guidelines recommend obtaining an admission ECG on suspected patients and admitting those with new abnormalities for monitoring. A normal ECG and troponin I can rule out BCI. Echocardiography is recommended for instability or arrhythmias. Sternal fractures alone do not predict BCI. Cardiac imaging like CT or MRI can help
