Some Common Causes of Ride Related Incidents
Poor design or construction / Installation
Inadequate supervision
Lack of secure safety locks
Size mismatch between guest and ride seats/safety lock
Intensity of motion and emotion
Riding or standing in an unsafe position
Equipment failure due to inadequate maintenance or defective components
Operator issues (inadequate training, distracted operators, etc.)
Rides under the control of patrons (Water slides, Go-Karts, etc.)
Guest Behavior including horseplay, force opening of lock, trying to
disembark before stopping the ride, jump over handrails, etc.

• A work permit system is a
formal system stating exactly what work is
to be done, where, and when.
• The people doing the job sign
the permit to show that they understand
the risks and precautions necessary. PPEs
need to wear while doing any activity
• Lockout - Tagout (LOTO) is a safety
procedure used to prevent the accidental
activation of hazardous energy or
machines

and analysis technique used by industry
to evaluate the properties of a material,
component, structure or system for
characteristic differences or welding
defects and discontinuities without
causing damage to the original part.

controls, Procedure for adverse weather & emergency conditions.
Training program for the Ride assistants includes - Instruction on loading
and unloading of guests, Instruction on passenger restrictions, Controlling
of waiting area, operating passenger restraint

a better guest experience in amusement rides.
Aspects of Guest Management for Ride Safe Usage:
GUEST MANAGEMENT
• Criteria for Ridership- Such as height, weight
or age limit if any or any other restriction
• Queue Management- To direct guests in an
orderly manner to the ride loading area so
that loading can be done safely.
• Forewarnings (Signage)- Such as visual
signage, printed Instructions & verbal
instructions
• Behavior Spotting & Intervention- Such as monitoring of acceptable and
unacceptable behavior of guest throughout the ride

Proper corrective actions to be taken and effectiveness to ensured periodically
INCIDENTS – REPORTING & INVESTIGATION
Incident reporting procedures and Investigation process is applicable also to OHS
incidents reporting, investigation, corrective action and its effectiveness of the
action taken. Incidents are classified into Injury, Ill-Health, Illness & Near Miss.
Injuries to be scaled based on the severity and
necessary actions to be taken to avoid the
same in future.
Investigation process to be carried out based
on the incident report and to be submitted to
the top management.

Aspects of Guest Management for Ride Safe Usage: GUEST MANAGEMENT • Criteria for Ridership- Such as height, weight or age limit if any or any other restriction • Queue Management- To direct guests in an orderly manner to the ride loading area so that loading can be done safely. • Forewarnings (Signage)- Such as visual signage, printed Instructions & verbal instructions • Behavior Spotting & Intervention- Such as monitoring of acceptable and unacceptable behavior of guest throughout the ride
  • 27. 27 Proper corrective actions to be taken and effectiveness to ensured periodically INCIDENTS – REPORTING & INVESTIGATION Incident reporting procedures and Investigation process is applicable also to OHS incidents reporting, investigation, corrective action and its effectiveness of the action taken. Incidents are classified into Injury, Ill-Health, Illness & Near Miss. Injuries to be scaled based on the severity and necessary actions to be taken to avoid the same in future. Investigation process to be carried out based on the incident report and to be submitted to the top management.
