it is condition of eye in which the two eye refractive power is unequal. a difference of 1D in two eye then cause 2%difference in the size of 2 retinal image of 2 eye is well tolerated and 2.5-4D can be tolerated depending upon the individual sensitivity but it is more than 4D is not tolerated.
hypermetropia (hyperopia) is also known as long sightedness. it is condition in which parallel rays of light coming from infinity are focused behind of retina with accommodation being at rest
visual acuity is very important for us . its the spatial resolving capacity of the visual system . visual perception (sensation) from stimulation of the retina by light and its of four type .
1- light sense
2- form sense
3- sense of contrast
4- colour sense
optical aberration is very important for optometrist .
eyeball is not optically perfect it shows some optical flaws which reduce resolution of the focused image they are called aberration.
New Directions in Targeted Therapeutic Approaches for Older Adults With Mantl...i3 Health
i3 Health is pleased to make the speaker slides from this activity available for use as a non-accredited self-study or teaching resource.
This slide deck presented by Dr. Kami Maddocks, Professor-Clinical in the Division of Hematology and
Associate Division Director for Ambulatory Operations
The Ohio State University Comprehensive Cancer Center, will provide insight into new directions in targeted therapeutic approaches for older adults with mantle cell lymphoma.
STATEMENT OF NEED
Mantle cell lymphoma (MCL) is a rare, aggressive B-cell non-Hodgkin lymphoma (NHL) accounting for 5% to 7% of all lymphomas. Its prognosis ranges from indolent disease that does not require treatment for years to very aggressive disease, which is associated with poor survival (Silkenstedt et al, 2021). Typically, MCL is diagnosed at advanced stage and in older patients who cannot tolerate intensive therapy (NCCN, 2022). Although recent advances have slightly increased remission rates, recurrence and relapse remain very common, leading to a median overall survival between 3 and 6 years (LLS, 2021). Though there are several effective options, progress is still needed towards establishing an accepted frontline approach for MCL (Castellino et al, 2022). Treatment selection and management of MCL are complicated by the heterogeneity of prognosis, advanced age and comorbidities of patients, and lack of an established standard approach for treatment, making it vital that clinicians be familiar with the latest research and advances in this area. In this activity chaired by Michael Wang, MD, Professor in the Department of Lymphoma & Myeloma at MD Anderson Cancer Center, expert faculty will discuss prognostic factors informing treatment, the promising results of recent trials in new therapeutic approaches, and the implications of treatment resistance in therapeutic selection for MCL.
Target Audience
Hematology/oncology fellows, attending faculty, and other health care professionals involved in the treatment of patients with mantle cell lymphoma (MCL).
Learning Objectives
1.) Identify clinical and biological prognostic factors that can guide treatment decision making for older adults with MCL
2.) Evaluate emerging data on targeted therapeutic approaches for treatment-naive and relapsed/refractory MCL and their applicability to older adults
3.) Assess mechanisms of resistance to targeted therapies for MCL and their implications for treatment selection

Ethanol (CH3CH2OH), or beverage alcohol, is a two-carbon alcohol
that is rapidly distributed in the body and brain. Ethanol alters many
neurochemical systems and has rewarding and addictive properties. It
is the oldest recreational drug and likely contributes to more morbidity,
mortality, and public health costs than all illicit drugs combined. The
5th edition of the Diagnostic and Statistical Manual of Mental Disorders
(DSM-5) integrates alcohol abuse and alcohol dependence into a single
disorder called alcohol use disorder (AUD), with mild, moderate,
and severe subclassifications (American Psychiatric Association, 2013).
In the DSM-5, all types of substance abuse and dependence have been
combined into a single substance use disorder (SUD) on a continuum
from mild to severe. A diagnosis of AUD requires that at least two of
the 11 DSM-5 behaviors be present within a 12-month period (mild
AUD: 2–3 criteria; moderate AUD: 4–5 criteria; severe AUD: 6–11 criteria).
The four main behavioral effects of AUD are impaired control over
drinking, negative social consequences, risky use, and altered physiological
effects (tolerance, withdrawal). This chapter presents an overview
of the prevalence and harmful consequences of AUD in the U.S.,
the systemic nature of the disease, neurocircuitry and stages of AUD,
comorbidities, fetal alcohol spectrum disorders, genetic risk factors, and
pharmacotherapies for AUD.

2. Asthenopia - it is condition in which the slight
ocular discomfort , headache , general be following
symptom is present as eyestrain, fatigueness after
reading, heaviness of lids and sleepiness after
reading
Cause -
1. Uncorrected refractive error.
2. Defects of ocular motility.
3. Accommodation and convergence insufficiency.
4. Nutritional diet and vitamins.
3. 1-Asthenopia in uncorrected refractive error -
It is small degree of refractive cases.
● Myopia - in myopic patient held the book more near
and normal distance. He is required more
convergence.
● Hypermetropia - if the patient is required more use
of ciliary muscle to focus the near object sharply.
● Astigmatism - these symptoms are more severe
cases of hypermetropia astigmatism in which
accommodation makes effort to overcome
hypermetropia.
4. ● Anisometropia - the patient is unequal blur.
Accommodation is developed these symptoms.
2- asthenopia in defects of ocular motility -
asthenopia is seen in the hypermetropia deviations
of eye and break the fusion of patients is focus the
object to develop the asthenopic symptoms.
3- asthenopic in accommodation and convergence
insufficiency - it is for near object change the power
of eye. The accommodation is insufficiency the eye
power is not fully change ,in convergence
insufficiency the eye is not fully converge the focus
object these are more object of accommodation
5. insufficiency due to illness and toxic condition
disorders.
Clinical profile -
● Nonvisual ocular symptoms - the patient usually
complain of tiredness of the eye after near work.
A temporary relief is obtained by resting or
rubbing. It is unhealthy condition of lid and
conjunctiva watery suffered and blurred.
● Headache - headache is not fully understood but
presumably is rests upon same basis as other
referred pain of visceral origin. So ciliary pain
caused by increased muscular efforts in
6. Asthenopia is referred to the areas associated with
the cervical segment which connect with the
superior cervical ganglion. The ophthalmic devision
of the gift nerve is represented most caudally so
that ciliary pain primarily frontal and occipital in
distribution.
● Visual symptoms - this is especially seen the eye
much for reading or study of small objects over
long periods of time while fine sewing, the cinema
, motor driving, in the distraction of confusing
traffic. The ciliary muscle gives up
7. Attempt to focus and the image becomes indistinct
or the ocular muscle slip back into their condition
of rest and diplopia results.
● Other general symptoms of eyestrain - digestive
upset such as dyspepsia, nausea, vague
nervous disorders such as dizziness, insomnia
and depression and many other symptoms have
all in the past been variously attributed to
eyestrain.
8. Management of asthenopia
● Refraction - it is done under cycloplegic
prescribed actual required to glass.
● Orthoptic treatment - orthoptic exercise to
improve the muscle power and develop the
vision.
● Prismotherapy - base in prism are prescribed in
convergence insufficiency only if orthoptic
exercises fail.
● General management - improve the general
health and advise the multivitamin and
nutritional diet.
