This document discusses the process of provisionally arranging and trying in the upper six anterior teeth during complete denture fabrication. Key steps include: 1. Arranging the six anterior teeth on the cast according to anatomical landmarks and ensuring proper width, height, and spacing. 2. Constructing a wax base plate and reinforcing it with wire to hold the teeth in place. 3. Placing the central incisors in the center and canines where the alar nasi line crosses the canine papilla line. 4. Trying in the teeth assembly and checking for proper lip support, symmetry, occlusion, and patient approval before proceeding.

1. Six anterior teeth registration and try in
During the 3rd visit, for recording the jaw relation, the upper six anterior teeth should be ready
for try in. These are arranged according to the indications on the cast.
The final adjustment for their position, tilt, vertical length and labial support should be made at
the chair side during the try in.
After taking the final impression, select the size, mould and shade of upper six teeth with
patient’s agreement.
Usually tip to tip inter canine width is similar to the nose width between the alar nasi. Here, teeth
are arranged in the curvature of the anterior arch, their intercanine width should be chosen a little
greater about 3-5mm than the nose width.
Apart from this, there should be no rigid rule in the selection for natural appearance. Selection of
teeth should not be regarded as a small matter because many patients judge the value of denture
by the appearance of the visible position of the anterior teeth alone.
Provisional arrangement of upper six anterior teeth
1. On the upper cast, reproduce vertical indication for canine line and alar nasi line. On the land
opposite the canine region on either side, canine papilla canine line (CPC line) passing through
the center of the incisive papilla is noted.
2. On the upper cast, construct a wax base plate, with its periphery fully adapted into the sulci.
Reinforce by embedded stiff wire along the ridge and along the posterior edge.
3. Finally place the central incisors adjacent to the centre line. Their edges should be about 8-
10mm from the centre of incisive papilla.
4. Ensure to have distance from the incisal edge to the vestibule or periphery of labial flange is
about 22 mm.
5. Next place the canines where alar nasi line passing the CPC line. From the front, only the
mesial half should be visible.
6. Place lateral incisors between them.
In their natural position, the upper anterior six teeth generally full labial to the alveolar.
After extraction, their anterior wall of socket is resorbed and residual ridge represents only the
palatal wall of the alveolus.---------------
After the final adjustment at the chair side, these six teeth give the most reliable key point
for establishing natural position of all the remaining teeth.

2. Try in of upper six anterior teeth
1. Insert the six teeth assembly and allow sometime for the lips and cheeks to settle.
2. Examine the appearance in full face (with the lip seal)
Full face front view-
Labial support (under support, over support, or normal)
Under support – upper lip will be fallen, nasolabial groove deeper, Cupid’s bow less
prominent (should be deeper for the age), vermilion border less visible
Over support – due to teeth forward, thick labial flange and philtrum may appear flatten,
over prominent of canine eminence
Left right symmetrical or not.
In profile view
Labial contour should be in harmony with general profile of the face
Nose-philtrum angle should be about 90 degree (90 - Indian, < 90- European, >90 - Tibetan-Burma, Asian)
Check intraorally,
1. With the lip open, mid line of central incisors should coincide with the center of the face.
2. Swaying is present or not in the vertical axis.
3. Canines being at the corner of the mouth bring up the character of person of the mouth.
Facial expression at the corner of the mouth on the modiolus, support by labial fullness.
Canine prominent in the man with prominent of neck.
In the normal, only mesial half should be visible, if more mesial, more muscular.
Canine either suit the sex or not.
4. Firstly setting central incisors, canine and then lateral incisor.
Vertical height standard 2mm may not visible
Teeth anteroposterior and labial flange (thin or thick)
Orientation of anterior occlusal plane should be parallel with interpupillary line.
5. Phonetic test (F,V sound)
Vermilion border of lower lip touch the upper anterior teeth tips

3. 6. Give mirror to the patient and take the approvals of size, shade, shape (mould), length,
position and irregularity of the teeth.
In old denture patient, simulate old denture and should not change suddenly.
Registration using anterior six teeth assembly block
After checking the requirements of anterior six teeth assembly block, first registration
procedure is performed using routine upper record block together with lower block by applying
routine registration principles. And then using anterior six teeth assembly block, second
registration with same lower record block is done. As an advantage, one step of wax denture
try in procedure can be skipped in complete denture procedure. Other objectives of anterior six
teeth assembly are to get the natural position and arrangement of teeth as closely as possible and
to facilitate desired aesthetics.
