A 41-year-old woman with a history of tobacco use presented with cough and dyspnea, and was diagnosed with α1-antitrypsin deficiency (AATD) after severe obstructive pattern noted in pulmonary function tests and CT imaging revealing emphysema and bronchiectasis. AATD, underdiagnosed genetically, typically presents in the fourth or fifth decade of life with respiratory symptoms, and is associated with a significant risk of lung and hepatic disease. Treatment includes conventional COPD therapy and possibly augmentation therapy, with the patient's symptoms improving after initiating treatment.
