The score effectively rules out appendicitis at a cut-off of 5 (99% sensitivity) but performs poorly for ruling in appendicitis at a cut-off of 7, indicating different accuracies based on gender and age. Overall, while the Alvarado score is well-calibrated in men, it tends to over-predict appendicitis in women and children.
