that we accomplish in the school?
• School Register ( Form 1 )
• Teacher’s Monthly Report on Enrolment
and Attendance ( Form 2 )
• Report on Promotion ( Form 18-E )
• Elementary School Permanent Record
( Form 137-E )
• Pupil’s Report Card ( Form 138-E )
Form 1 - School Register
• should have been accomplished by July
2013 (every school year )
• transfer in/ late enrollees should be
written after the last entry and will not be
included in the alphabetical order.
• Monthly attendance should be
accomplished regularly.

• Total enrolment for month to date is your
latest monthly enrolment including the
transferees in / excluding the transferred
out / dropped out

• Daily attendance should be filled up
regularly
