1) Community viral load measurement is a flawed metric for tracking HIV incidence and the impact of antiretroviral treatment (ART) programs. 2) It lacks accurate denominators of total HIV infections and numerators of longitudinal viral load data. 3) It obscures critical information like rates of undiagnosed acute HIV infection which is responsible for a large proportion of transmissions. 4) Reliable outcome measurements that account for undiagnosed infections, lost to follow up patients, geography and treatment failure/resistance are needed to properly measure HIV prevention efforts and effects of ART.
