This workshop will examine strategies that communities are using to promote the effective use of HPRP resources and other interventions to end homelessness. The workshop will examine the use of community learning laboratories that evaluate emerging data and HPRP implementation to refine interventions. Strategies to support the transfer of effective strategies across local communities will also be explored.

Bringing Public Health and Primary Care Together: The Practical Playbook National Meeting was at the Hyatt Regency in Bethesda, MD, May 22 - 24, 2016. The meeting was a milestone event towards advancing robust collaborations that improve population health. Key stakeholders from across sectors – representing professional associations, community organizations, government agencies and academic institutions – and across the country came together at the National Meeting to help catalyze a national movement, accelerate collaborations by fostering skill development, and connect with like-minded individuals and organizations to facilitate the exchange of ideas to drive population health improvement.

Assertive Community Treatment (ACT) is a treatment model developed in the 1970s to help individuals with severe mental illness live in the community and improve their quality of life. It involves a multidisciplinary team providing individualized services wherever the client is, with a focus on outreach and flexibility. Family Assertive Community Treatment (FACT) adapts this model to serve homeless families with complex needs through intensive case management and connecting families to services for at least 18-24 months. The document outlines the key components of traditional ACT and how FACT has adapted these to better serve families experiencing homelessness and related issues.

Yet despite its importance, health governance investments are often overlooked. Health governance is frequently misunderstood by governments and the global health community, because governance in practice (vs. theory) is poorly defined and difficult to operationalize.
In this session, participants will learn how Haiti has defined and is addressing dimensions of governance for health financing and human resource reforms. Participants will apply these dimensions of health governance to work/activities that they are involved in, and consider how addressing these dimensions can strengthen health governance in their countries and enhance the impact of health financing, human resource, and service delivery reforms.

- There is a need for a more collaborative business model between EPE and programs to better integrate epidemiology, evaluation and programmatic expertise from the beginning of planning through implementation and evaluation.
- An ideal model would have epidemiologists, evaluators and program staff working more as horizontal partners at each stage of the public health process.
- This could involve co-locating staff when possible, as well as better aligning goals and communication across divisions.
- Developing joint logic models and sharing data more openly were discussed as ways to break down barriers between groups and improve multi-disciplinary work.

effective ACT services, many systems which interface

Avenues for Homeless Youth
Avenues for Homeless Youth operates four programs in the Twin Cities that provide shelter and transitional housing for over 200 homeless youth per year. The programs include a shelter in North Minneapolis, as well as GLBT, suburban, and Minneapolis host home programs. Host homes provide a safe, stable transitional housing option at 50% lower cost than congregate housing. They aim to build long-term supportive relationships critical for youth success. A wound is a break in the integrity of the skin or tissues, which may be associated with disruption of the structure and function.
Healing is the body’s response to injury in an attempt to restore normal structure and functions.
Healing can occur in two ways: Regeneration and Repair
There are 4 phases of wound healing: hemostasis, inflammation, proliferation, and remodeling. This document also describes the mechanism of wound healing. Factors that affect healing include infection, uncontrolled diabetes, poor nutrition, age, anemia, the presence of foreign bodies, etc.
Complications of wound healing like infection, hyperpigmentation of scar, contractures, and keloid formation.
