We identified 934 patients and used clustering analysis to group patients based on their medical data such as diagnoses, treatments, and symptoms. This allowed us to identify subgroups of patients who were likely to develop specific diseases. For example, clustering analysis may identify that patients being treated for fatigue and blurred vision have a strong likelihood of being diagnosed with diabetes. Clustering the patients in this way could help predict disease development and determine effective prevention and treatment strategies. Overall, clustering analysis can help optimize healthcare for both individuals and populations.

Supporting Evidence-based Medicine Training and Implementation in Haiti
Due to both a long-standing presence and geographic proximity, the University of Miami clinicians were some of the first responders in Haiti following the January 2010 earthquake. To support the immediate and varied needs of these responders, the Calder librarians created a website gathering both freely available and commercially licensed material most relevant to the first responders and donated two boxes of core medical textbooks. The Calder Library then received an Express Disaster Recovery Award from the National Network of Libraries of Medicine (NN/LM), Southeastern/Atlantic Regional Library, to bring laptops and other technical support to the University of Miami medical tent and the long-standing training of Haitian physicians and nurses. This award led to an invitation to join a University of Miami field visit to Haiti to observe firsthand the work there and implement a needs assessment of the University of Miami affiliates onsite as well as the Haitian medical students and nurses in training. This presentation will also discuss the Emergency Access Initiative and its first implementation in Haiti, in addition to the planning process, necessary troubleshooting, and the challenges of working in both a disaster setting and a developing country.

To support the immediate and varied needs of these responders, the Calder librarians created a website gathering both freely available and commercially licensed material most relevant to the first responders and donated two boxes of core medical textbooks. The Calder Library then received an Express Disaster Recovery Award from the National Network of Libraries of Medicine (NN/LM), Southeastern/Atlantic Regional Library, to bring laptops and other technical support to the University of Miami medical tent and the long-standing training of Haitian physicians and nurses. This award led to an invitation to join a University of Miami field visit to Haiti to observe firsthand the work there and implement a needs assessment of the University of Miami affiliates onsite as well as the Haitian medical students and nurses in training. This presentation will also discuss the Emergency Access Initiative and its first implementation in Haiti, in addition to the planning process, necessary troubleshooting, and the challenges of working in both a disaster setting and a developing country.

or decades, researchers have relied on biospeci-
mens and data leftover after collection for other
purposes to make important scientific and med-
ical advances1 — and the law has afforded a number of
mechanisms to facilitate such “secondary” research,
often without consent from the human sources from
whom the specimens or data were derived. At the start
of the twenty-first century, however, the previously
arcane issue of aconsensual secondary research began
to gain public prominence. For example, in 2004,
members of the Havasupai Tribe brought a lawsuit
against Arizona State University alleging that blood
samples originally provided for research on diabetes
had been used without their knowledge or consent for
other types of genetic studies, including those expos-
ing tribe members to stigma regarding mental illness
and challenging their deeply held beliefs regarding
tribal ancestry.2 In 2009, parents in Texas filed suit
claiming research use of leftover blood spots from
their newborns’ mandatory public health screenings
was a violation of liberty and privacy rights, as well
as the right against unreasonable search and seizure;3
parents in other states have done the same.4 In both
the Havsupai and blood spot litigation, resulting
settlements entailed the removal of remaining speci-
mens from research use, either through destruction5
or return.6 The debate about newborn blood spots also
resulted in responsive legislation at both state and fed-
eral levels.7
These examples received national news coverage,
but the greatest public awakening regarding biospeci-
men research is probably traceable to the bestselling
book, The Immortal Life of Henrietta Lacks, published
in 2010.8 The book tells the story of a young African
American woman, Henrietta Lacks, who received
treatment for cervical cancer at Johns Hopkins in the
1950s, and died shortly thereafter. Cells collected from
a biopsy of Lacks’s tumor were cultured without her
knowledge or permission, and additional cells may
have been collected specifically for research purposes.
The cells were cultivated into a self-perpetuating cell
line, dubbed “HeLa,” which became the most valuable

Case reports and studies may be defined as the non-experimental description of an individual or a few of cases in terms of new or unusual presentation of the diseases, an unexpected disease course or pathophysiology, and new effects (either beneficial or detrimental) of existing medications or procedures. Although they suffer from the non-experimental nature and other potential bias and errors, case reports and studies have played and will continue to play an important part in the advancement of medicine. They often serve as "primers" leading to discoveries of new diseases/disease pathophysiology as well as development of new preventive and therapeutic measures. Case reports and case studies are also employed as a platform for the training of medical students and/or resident doctors in scientific writing and critical thinking. Although the significance of case reports and studies in medicine has being recognized since the early stage of development of clinical medicine, their value needs to be appreciated in the context of modern clinical research design and the hierarchy of strength of evidence for guiding patient care. This paper discusses case reports and studies within the big picture of clinical research, research design, and evidence-based practice.

Abstract
Among the challenges facing research translation—the effort to move evi-
dence into policy and practice—is that key questions chosen by investigators
and funders may not always align with the information priorities of deci-
sion makers, nor are the findings always presented in a form that is useful
for or relevant to the decisions at hand. This disconnect is a problem par-
ticularly for population health, where the change agents who can make the
biggest difference in improving health behaviors and social and environmen-
tal conditions are generally nonscientists outside of the health professions.
To persuade an audience that does not read scientific journals, strong sci-
ence may not be enough to elicit change. Achieving influence in population
health often requires four ingredients for success: research that is respon-
sive to user needs, an understanding of the decision-making environment,
effective stakeholder engagement, and strategic communication. This arti-
cle reviews the principles and provides examples from a national and local
initiative.
