Service Service Description
Exterior Work
Gutters
Clean out: unclog and clean out all gutters from leaves, vegetation, and debris to allow rain water to properly flow through
the system.
Landscape
Remove all brush, overgrown vegetation, dead shrubs, and debris. Lawn clean-up. Landscape, edging, mulch. Clean
debris in rear of house. The exterior yard areas must be free from accumulations of garbage, debris, metals, yard
clippings, weeds, tires, automotive parts, tools, machinery, etc.
Roof Inspected; no signs of leakage, water damage or entry.
Tuck Point
Tuck point steps & porch, sealant is compromised and susceptible to weather. Use Quikrete cement, apply two coats
and clean grooves. Tuck-point the following areas with cement mortar only in designated area
Storm Door: Remove and Repair jamb.
Ext. Door: Replace front entry & side door with steel. Replace with brushed nickel lockset.
Repair/replace with approved hardware, deteriorated/damaged exterior entrance doors.
# of Damaged: Install window screens or repair/replace all torn screens on dwelling windows/doors.
Specified Room: Replaced broken/missing windows on dwelling/garage or re-glaze and paint exterior side of window as
necessary. All windows must go up and down smoothly - have locks and handles - and no missing sash cords, rods or
ropes. Metal clips may replace sash ropes/rods
Awning Scrape & Paint (3) awnings with Sherwin Williams Black exterior paint.

Vinyl: Remove and replace damaged siding on West side of property with J-Channel from
Roof: The dwelling/garage structure roof is in need of complete replacement. All replacements must match and conform to
the same design and color.
Siding: Replace garage overhead door and garage man door. All must open and close properly.
Floor: Replace existing concrete pad. Rip and replace and create forms for newly poured concrete garage pad. Pass pre-
inspection with city of Maple Heights. Lay stone and then pour new cement and pass final inspection. All work must be
permitted.
Sidewalk: Replace concrete in designated area(s) UNDER PERMIT of the Maple Heights Building Department. call for and
pass inspection with Maple Heights municipality. Pour blocks where marked.

Install and paint new 6-panel door slab throughout. All interior room entrance and closet doors must be properly installed,
open, and close and latch properly and have proper hardware installed. All bedrooms and bathrooms must have a door
installed for privacy. Make fully operable.
Hardware: Install new brushed nickel door hardware on all interior doors throughout.
Gas/Electric: Gas
Inspect or Change-out: Remove out dated furnace and replace with a new Goodman 80k btu Up-flow furnace. Run new
sheet metal heat runs where existing is damaged or compromised. Cap all open gas lines and install lever type ball shut
off valves on the entire gas supply line
Furnace
Check, clean and adjust, for safety, furnace, boiler, A/C and hot water heater. Associated gas lines have approved lever-
type shut-off gas valves. Flue pipes have 26 gal. galvanized sheet metal, sealed at joints with screws, cement sealed at
the chimney. Install new Hot water heaters, must have been approved T & P relief valves, same size as valve within 6
inches of the floor. Work was be performed by a City of Maple Heights HVAC registered contractor.
Hot Water Tank
Install No. 6 copper bonding wire around the water meter using approved U.L. listed ground clamps. Bond hot and cold
gas lines to the hot water tank.
Electrical
Inspect and certify the condition of electrical panel. Terminate outlet located in light fixture with one that does not feature
an outlet. POWER TO PROPERTY WAS SHUT OFF AT TIME OF INSPECTION--- ALL ELECTRICAL DEVICES/ OUTLETS WILL NEED TO BE RE-
INSPECTED. ALL VIOLATIONS FOR ALL ELECTRICAL COMPONENTS AND PARTS WILL BE ADD TO THE VIOLATIONS LIST. NOTE: ELECTRICAL
PERMIT REQUIRED FOR SERVICE TO BE RESTORED. Install grounded Tamper Resistant type GFCI outlet on all exterior outlets, in
all garage outlets, laundry area, bathrooms, and kitchen above all counter tops, in any area within six feet of water

lines throughout house to replace damaged or non-existent copper.

Install or make operable a mechanical testable smoke detector on the ceiling directly outside of sleeping room(s) and on
the utility/basement room ceiling at bottom of stairs. Install new smoke and carbon monoxide detectors in home to meet
