RECRUITMENT OF TEACHER I POSITION IN
PUBLIC ELEMENTARY
Who may avail of the Service?
** Applicants who hold a valid certificate
of registration/professional license as a
teacher from the Professional Regulation
Commission (PRC) seeking to be appointed
to a Teacher I position
** Qualified applicant who is with a valid
professional license issued by the Philippine
Regulation Commission and who meets the
requirements of the evaluation selection ap-
plied by the a School Division
** Bonafide resident of San Antonio, Cala-
pan City for at least six months who de-
clared himself/herself as a resident of a the
said barangay in the Personal Data Sheet
Fees: None
Schedule of Availability of Service:
January 15 - Filing of application
Schedule of interview to be announced later following
the DepEd Memo
Requirement for Applicants:
1. Applicant letter
2. CSC Form 212 (revised 2005) in 2 copies with the latest
2x2 ID picture
3. Certified photocopy of the PRC Certificate of Registra-
tion/License
4. Certified photocopy of ratings obtained in the Licensure
Examination for Teachers/Professional Board Examination
for Teacher
5. Certified copies of Transcript of Records for
baccalaureate course
6. Copies of the service record, performance rating and
school clearance for those with teaching experience
7. Certificates of specialized training, if any

Enrolment 2. Application for Certification 3. Recruitment of Teacher 1 position ENROLMENT – Kindergarten and Elementary Who may avail of the Service? 1.Kindergarten age – (5-6 years old) 2. Elementary age – (6-11 years old) Schedule of Availability of Service: 1. Pre-School and Elementary: two (2) weeks before the start of the academic school year Requirements for Enrollee: Kindergarten and Elementary  Report card for Grade II – VI pu- pils/students  Birth Certificate for Kindergarten and Grade I pupils Transferees  Certificate of good moral conduct  Forms 137 and 138  Reading ability assessment / Place- ment test How to avail of the Service (SPED,Kindergarten and ELEMENTARY) Applicant/Client Service Provider Duration of Activity Person In Charge Submit Birth certificate for SPED & Kindergarten pupils and Report card for Grade I to VI pupils. Receive, assess and evaluate Report cards & Birth certificates Within 10 minutes Teacher- in- charge Undergo assessment test/ interview (for Kindergarten and transferee) Conduct assessment test/ reading test or interview Within 30 minutes Teacher -in- charge Enroll and receive books Accept pupils and distribute books Within 30 minutes Teacher -in- charge APPLICATION FOR CERTIFICATION Schedule of Availability of Service: Monday – Friday Who May Avail of the Service? Elementary pupils / Parents/ 4P’s members How to avail of the Service Applicant/ Client Service Provider Duration of Activity Person In Charge Get certification form from the principal Interview and assess. Within 5 minutes Principal Bring the certification to the Teacher-in- charge. Receive the certification. Write the name of the pupil. Affix signature of the Teacher-in- charge. Within 5 minutes Teacher – in- charge Go to the principal’s office. Receive the certification signed by the teacher – in- charge. Affix the signature of the principal. Within 5 minutes Principal RECRUITMENT OF TEACHER I POSITION IN PUBLIC ELEMENTARY Who may avail of the Service? ** Applicants who hold a valid certificate of registration/professional license as a teacher from the Professional Regulation Commission (PRC) seeking to be appointed to a Teacher I position ** Qualified applicant who is with a valid professional license issued by the Philippine Regulation Commission and who meets the requirements of the evaluation selection ap- plied by the a School Division ** Bonafide resident of San Antonio, Cala- pan City for at least six months who de- clared himself/herself as a resident of a the said barangay in the Personal Data Sheet Fees: None Schedule of Availability of Service: January 15 - Filing of application Schedule of interview to be announced later following the DepEd Memo Requirement for Applicants: 1. Applicant letter 2. CSC Form 212 (revised 2005) in 2 copies with the latest 2x2 ID picture 3. Certified photocopy of the PRC Certificate of Registra- tion/License 4. Certified photocopy of ratings obtained in the Licensure Examination for Teachers/Professional Board Examination for Teacher 5. Certified copies of Transcript of Records for baccalaureate course 6. Copies of the service record, performance rating and school clearance for those with teaching experience 7. Certificates of specialized training, if any How to avail of the Service Applicant/Client Service Provider Duration of Activity Person In Charge Submit a written application with the required documents to the school Principal. 1. Receive application and verify documents submitted as to the completeness and authenticity. 2. Submit the list of applicants together with their pertinent documents to the District Sub-Committee.
