I, as Policy Applicant and payment account holder, confirm I am not acting for any other person, and I authorize The Hongkong
and Shanghai Banking Corporation Limited ("HSBC") to debit from my specified account from time to time for (i) the initial and
subsequent premiums due as instructed by HSBC Life (International) Limited (“HSBC Life” or "you") and/or (ii) any applicable
levy on behalf of the Insurance Authority.
I am in Hong Kong now.
All statements and information I have provided in this application are complete and true. I will immediately tell you to
correct them if the statements or information later become incomplete, untrue or inaccurate before you issue any policy.
You will approve or decline any future claims based on these statements and information.
I understand you will use the personal information I gave you in this application according to the Personal Information Collection
Statement and I must make any request to change my personal information in HSBC’s records through branches or internet
banking.
All personal information I have most recently provided to HSBC (such as ID documents, nationality, mobile number and
payment account correspondence address) is accurate and up to date. I consent to HSBC providing my personal information
to HSBC Life for evaluating and processing this application. I understand HSBC may request updated personal information
(such as ID documentation), and HSBC Life may not issue the policy if I do not give HSBC Life any such requested information
by a specified time.
I understand submitting this application does not guarantee its approval and you will only approve a policy after (i) you are
satisfied I meet your prevailing eligibility and underwriting requirements, and (ii) you have processed my first premium
payment and levy. If you do approve my application, my policy will be effective from the date I submitted a valid application.
I understand I have selected an insurance policy that will pay some or all of the Sum Insured in advance upon diagnosis of
Cancer or Early Stage Cancer / Carcinoma-in-situ, according to Policy Provisions. You will reduce the Death Benefit amount
payable by any such advance payments (and by any unpaid premiums).

I confirmI did not replacein the last 12 months and do not intend to replacein the next 12 months an existing lifeinsurancepolicy(ies).I realizeif I confirmit is not a policy
replacementbut indeed it is a policy replacement,I may jeopardizemy future right of redress if I find later I have been disadvantageddue to such replacement.
