HealthCare Benefits: Courtesy Care <ul><li>Dental Benefits </li></ul><ul><li>One annual dental visit for examinations, x-rays and teeth cleaning is provided at no cost to the house staff member and his/her dependents. </li></ul><ul><li>Other general dentistry services (i.e. 2 nd examination, cavities, etc.) are covered at 50% of the customary fee to cover lab and material costs. </li></ul><ul><li>Oral Surgery procedures are 100% covered . </li></ul><ul><li>Costs of the fixture and materials related to dental implants are not covered . </li></ul><ul><li>Orthodontics are not covered . </li></ul><ul><li>Co-pays are requested at the time of service . </li></ul><ul><li>Please call to cancel or reschedule. </li></ul>
  9. 9. <ul><li>Dental Benefits </li></ul><ul><li>Services outside the Department of Hospital Dentistry are not covered . This includes the College of Dentistry, the Center for Disabilities and Development and outside non-UIHC practitioners. </li></ul><ul><li>If a referral is made for services outside the Department of Hospital Dentistry, the house staff member is responsible to get prior approval from the Administrative Director of GME at 356-2256 prior to receiving services. </li></ul><ul><li>These services may be covered at 50%. </li></ul><ul><li>General Dentistry: 356-2743 </li></ul>HealthCare Benefits: Courtesy Care
  10. 10. <ul><li>Ob-Gyn </li></ul><ul><li>Each House Staff couple is limited to a maximum of $25,000 in courtesy care benefits for the diagnosis and treatment of infertility. </li></ul><ul><li>Ophthalmology </li></ul><ul><li>Eye exams are provided at no cost to house staff members and their dependents in the Department of Ophthalmology and Visual Sciences. </li></ul><ul><li>Eyeglasses, contact lenses, contact lens fittings & refraction only exams are not covered under courtesy care benefits. </li></ul><ul><li>The Department of Ophthalmology provides LASIK Surgery to house staff at a cost of $250 per eye . </li></ul><ul><li>21 cases per year starting July 1. </li></ul><ul><li>Benefit provided by Ophthalmology Department. </li></ul>HealthCare Benefits: Courtesy Care

Advised to inquire with GME Office prior to purchasing non-UIHC medical equipment and/or services. </li></ul><ul><li>This plan does not apply to house staff members and dependents who elect to seek care outside of UIHC where the same care can be provided at UIHC. </li></ul><ul><li>Medical equipment and/or services must be prescribed by a UIHC physician. </li></ul>

HealthCare Benefits <ul><li>Pharmacy/Prescription Benefits </li></ul><ul><li>Prescription meds, if medically necessary, are available to house staff members and their dependents at no cost per the house staff prescription benefit plan on the UIHC Formulary. </li></ul><ul><li>Medication must be listed on the house staff prescription benefit plan to be covered at no cost, unless otherwise noted. </li></ul><ul><li>Prescription quantities are limited to a one month supply (at a time). </li></ul><ul><li>House Staff member may not prescribe medications for himself or herself. </li></ul><ul><li>Over the counter prescriptions ( OTC ) can be self-prescribed . </li></ul><ul><li>Prescriptions for all controlled substances written for immediate family or personal use must be written by a UIHC staff physician . </li></ul><ul><li>Existing prescriptions must be prescribed by UIHC staff physician to be covered. </li></ul><ul><li>Formulary: www.formularyproductions.com/uihc </li></ul>

Other Insurance Benefits <ul><li>Life Insurance </li></ul><ul><li>House Staff are automatically enrolled in a $40,000 term life policy at no cost . </li></ul><ul><li>House Staff can elect to purchase dependent life insurance at $2 per month – automatically comes out of paycheck. </li></ul><ul><li>If House Staff elect to increase the value of their life insurance policy, contact insurance broker, Bernie Lowe & Associates. Cost borne by the house staff member. </li></ul>

Other Insurance Benefits <ul><li>Disability Insurance </li></ul><ul><li>House Staff are automatically enrolled in the disability plan at no cost . </li></ul><ul><li>If a House Staff member becomes fully disabled , he/she will receive his/her current stipend for first 180 days . </li></ul><ul><li>After the first 180 days, the disability plan pays the house staff member a $2500 monthly disability benefit to age 65 . </li></ul>

Paid Time Off (PTO) <ul><li>Includes both vacation and sick time </li></ul><ul><li>House Staff are eligible for 3 weeks of PTO (15 week days and 6 weekend days) </li></ul><ul><li>PTO must be used within the year (use it or lose it) </li></ul><ul><li>Arrange time off with Program Director </li></ul><ul><li>Absences outside of normal PTO may need to be made up per the discretion of the Program Director. </li></ul><ul><li>House Staff must meet accreditation and Board requirements regarding absences in order to sit for the Boards. </li></ul>

Other Paid Time Off (PTO) <ul><li>Maternity Leave </li></ul><ul><li>Female House Staff are eligible for 6 weeks of paid time off for maternity leave. </li></ul><ul><li>An additional 4 weeks of extra paid leave may be allowed if there are medically necessary reasons that extends the normal maternity leave. GME Office requires documentation from the prescribing physician to extend this paid leave. </li></ul><ul><li>House staff requesting extra non-medically necessary maternity leave must seek approval from Program Director. GME Office needs written documentation of this extension. This type of leave is UNPAID and benefits are suspended temporarily. </li></ul><ul><li>During this unpaid status, house staff can elect to pay the fringe benefit rate for the amount of time they are off (i.e. $34/month ). </li></ul>

Other Paid Time Off (PTO) <ul><li>Parental Leave </li></ul><ul><li>Non-birth parent house staff members have available 5 working days , per event, of paid time off related to the birth or adoption of a child. </li></ul><ul><li>Foster care and guardianship are not included in this policy. </li></ul><ul><li>Exclusive of allotted paid time off (i.e. extra 5 days) </li></ul><ul><li>Not required to use – may use vacation so Board eligibility remains in tact. </li></ul><ul><li>Must be used within the first 3 months of the event. </li></ul><ul><li>Approval of the leave should be documented and a copy sent to the GME Office for filing. </li></ul>

Leaves of Absence <ul><li>Requests must be discussed with Program Director for approval </li></ul><ul><li>These types of leaves are UNPAID and benefits are suspended temporarily </li></ul><ul><li>House staff have the option of paying the fringe benefit rate of $34/month so benefits remain in tact </li></ul><ul><li>Time missed (outside of normal PTO) may result in an extension of GME contract </li></ul><ul><li>Approved requests must be submitted to GME Office for house staff members’ files </li></ul>

To Receive a Paycheck… <ul><li>Must complete an I-9 form within the first 3 days of appointment per mandate by federal regulations. </li></ul><ul><li>First paycheck will come the first of the month after the start of employment (i.e. August 1 st ). </li></ul><ul><li>Residents must complete their I-9 forms at their respective GME orientation dates (6/23, 6/26, 7/1). </li></ul><ul><li>Fellows must complete their I-9 forms with their respective departments . </li></ul><ul><li>Original documentation is needed to verify the house staff member’s identity. </li></ul>

Parking at UIHC <ul><li>On-Call Parking </li></ul><ul><li>Moving a vehicle prior to 8:30 a.m. in Ramp I may not be possible due to patient care responsibilities. </li></ul><ul><li>University Parking Department offers house staff 16 free flex hours from 8:30 a.m. to 4:30 p.m. per month . </li></ul><ul><li>If house staff go over 16 hours in the month, they will be charged for the hours over the free flex hours. </li></ul><ul><li>House staff will receive a University bill directly for the overages. </li></ul>
