บทนา
VTE เป็นโรคที่มีความผิดปกติเกี่ยวกับระบบการแข็งตัวของเลือด ซึ่งมีอันตรายจนถึงแก่ชีวิตได้ และพบได้
บ่อยในปัจจุบัน โดย VTE สามารถพบได้ในทุกช่วงอายุ ทุกเชื้อชาติ ซึ่งมักจะเกิดตามหลังการได้รับอุบัติเหตุ การ
ผ่าตัดหรือการที่ร่างกายไม่ได้เคลื่อนไหวติดต่อกันเป็นระยะเวลานานหลายชั่วโมง เช่นการนั่งเครื่องบินระหว่าง
ประเทศ หรือผู้ที่มีความผิดปกติเกี่ยวกับระบบการแข็งตัวของเลือด (hypercoagulable disorder) ส่งผลให้เกิดการ
clot ของก้อนเลือดในหลอดเลือด VTE สามารถแบ่งออกเป็น 2 ประเภทย่อยคือ deep vein thrombosis (DVT :
คือก้อนเลือดอุดตันในหลอดเลือดดา) และ pulmonary embolism (PE : ก้อนเลือดอุดตันที่บริเวณปอด) โดย PE
จะมีการดาเนินโรคที่ค่อนข้างรวดเร็วและเกิดอัตราการตายมากกว่า DVT ประมาณร้อยละ 80 ของผู้ป่วย VTE จะ
เสียชวิตด้วย PE
Thrombosis หมายถึงการเกิด ลิ่มเลือด (thrombus) ขึ้นในหลอดเลือด หรือในหัวใจ ถ้าเกิดขึ้นใน
หลอดเลือดแดง เรียกว่า arterial thrombosis ถ้าเกิดในหลอดเลือดดา เรียกว่า venous
thrombosis ก้อนลิ่มเลือดอาจหลุดออกจาก embolus ไปอุดหลอดเลือดที่อวัยวะอื่น เช่น ปอด หรือสมอง
เรียก ปรากฏการณ์นี้ว่า thromboembolism
การรักษา VTE ในปัจจุบันจะทาการรักษาด้วย antithrombotic drug ซึ่งยาในกลุ่มนี้เป็นยาที่ต้องอาศัย
การติดตามการใช้อย่างใกล้ชิดเนื่องจากกลไกการออกฤทธิ์หลักคือ ต้านการแข็งตัวของเลือด หรือการเกิด clot ซึ่งมี
อาการข้างเคียงที่สาคัญคือ การมีเลือดออก (bleeding)
ระบาดวิทยา (Epidemiology)
อุบัติการณ์การเกิด VTE ปัจจุบันยังไม่เป็นที่ทราบแน่ชัด จากการสารวจในประเทศสหรัฐอเมริกาพบว่ามี
ผู้ป่วยประมาณ 2 ล้านคนเป็น VTE ทุกๆ ปีจะมีผู้ที่ต้องเข้ารับการรักษาตัวในโรงพยาบาลด้วย VTE และเสียชีวิตปี
ละ 60,000 คน อุบัติการณ์ของ VTE จะเพิ่มขึ้นตามอายุที่เพิ่มขึ้น โดยอุบัติการณ์จะเพิ่มเป็น 2 เท่าในผู้ป่วยที่มีอายุ
มากกว่า 50 ปี นอกจากนี้ VTE ยังสัมพันธ์กับเชื้อชาติด้วย ชาว African-American จะเสี่ยงมากกว่าชาว
American หรือยุโรป หรือ Hispanic ส่วนชาว Pacific Islanders จะพบ VTE น้อยมาก

กายวิภาคศาสตร์ของหลอดเลือดดาที่ขา (Anatomy of leg vein)
หลอดเลือดดาที่ขาของมนุษย์สามารถแบ่งออกเป็น 2 ระบบตามกายวิภาคศาสตร์คือ
1. หลอดเลือดดาที่บริเวณผิวของขา (superficial system) สามารถแบ่งออกได้เป็น 3 ส่วนคือ
Long saphenous vein เป็นหลอดเลือดดาที่ยาว เริ่มจากบริเวณด้านในของเท้า ผ่านทางด้านหน้า
ของ medial malleolus แล้วขึ้นไปสู่ขาและสิ้นสุดบริเวณด้าน medial ของขาและต่อรวมเข้าสู่ femoral veins
Short saphenous vein จะขึ้นมาจากด้านนอกของเท้า ผ่านด้านหลังของ lateral malleolus และ
ขึ้นมาบริเวณกลางน่องและต่อเข้าสู่หลอดเลือดดาใหญ่
Perforator veins เป็นหลอดเลือดดาที่เชื่อมระหว่าง deep และ superficial systems คือทาหน้าที่
นาเลือดจาก superficial เข้าสู่ deep system
ซึ่งการเกิดการอุดตันที่หลอดเลือดดาที่ผิว (superficial vein thrombosis) มักเกิดในรายที่ให้ยา
หรือน้าเกลือ แล้วมีการอักเสบของหลอดเลือดดา จะเห็นรอยแดงไปตามหลอดเลือดดา และถ้ากดจะเจ็บ
2. หลอดเลือดดาที่ลึก (deep system)
หมายถึงหลอดเลือดดาที่อยู่ชิดกับหลอดเลือดแดงใต้เข่า 3 เส้น ในแต่ละเส้นของหลอดเลือดแดงจะมี
หลอดเลือดดาประกบ 2 คู่ (มีหลอดเลือดทั้งหมด 6 เส้น) ซึ่งหลอดเลือดดาเหล่านี้จะวิ่งขึ้นมารวมกันกลายเป็น
popliteal vein ซึ่งจะอยู่ด้านหลัง popliteal artery ด้านหลังเข่า และในบริเวณต้นขาหลอดเลือดดาจะมาต่อกับ
หลอดเลือดแดง รวมกับ profunda vein และ common femoral vein ดังแสดงในรูปที่ 2 ในหลอดเลือดดาแต่ละ
เส้นเลือดจะสามารถไหลย้อนกลับจากบริเวณข้อหรือเท้า เข้าสู่หัวใจได้ (ในทิศทางที่ต้านกับแรงโน้มถ่วงของโลก)
เนื่องจากหลอดเลือดดาเหล่านี้จะมีลิ้นที่หลอดเลือดดาคอยเปิด-ปิดอยู่ตลอดเวลา นอกจากนี้ยังมีปัจจัยเสริมคือการ
บีบตัวของหลอดเลือดแดง และการบีบตัวของกล้ามเนื้อที่เท้าและน่อง ถ้าร่างกายมีความผิดปกติทาให้ลิ้นไม่สามารถ
ทางานได้ตามปกติก็จะทาให้มีเลือดไหลย้อนกลับลงมาที่ขาได้ โดยลิ้นของหลอดเลือดดามีความสาคัญมากในการ
ป้องกันไม่ให้เลือดไหลผิดปกติ ลิ้นของหลอดเลือดดามีลักษณะเป็น semilunar และเป็น bicuspid ปกติลิ้นเหล่านี้
จะทนต่อความดันเลือดไหลย้อนกลับได้ ลิ้นเหล่านี้จะทาหน้าที่บังคับทิศทางให้เลือดดาไหลจาก superficial
system ไหลไปทาง deep system (ไหลจากเท้าขึ้นมาที่ต้นขาและหัวใจตามลาดับ)

ปัจจัยเสี่ยงและพยาธิกาเนิด (Risk factor and etiology)
นอกจากปัจจัยเสี่ยงที่ได้กล่าวมาข้างต้นแล้ว ยังมีปัจจัยเสี่ยงอื่นๆ อีกดังแสดงในตารางที่ 2 โดยปัจจัยเสี่ยง
ที่มีนัยสาคัญอย่างยิ่งยวด (Strong risk factors) เช่น การได้รับอุบัติเหตุ malignancy จะมีผลทาให้การแข็งตัวของ
เลือดผิดปกติไป ปัจจุบันเราพบว่าพยาธิกาเนิดของ VTE เป็นผลสืบเนื่องมาจากการยับยั้งการไหลเวียนของเลือด เมื่อ
ร่างกายมีการไหลเวียนเลือดลดลงทาให้กระบวนการขจัด และเจือจาง clotting factor ลดลงทาให้เกิดก้อนเลือด
(thrombus) ขึ้นในหลอดเลือด โดยหลอดเลือดดาบริเวณส่วนล่างของร่างกายจะเกิดการสร้างก้อนเลือด
(thrombogenesis) ได้บ่อยกว่าส่วนบนเนื่องจากมีการไหลเวียนเลือดกลับสู่หัวใจช้ากว่าและลิ้นที่กั้นภายในหลอด
เลือดดามีการเปิด-ปิดที่ช้ากว่าส่วนบน

ปัจจุบันยังค้นพบอีกว่าความผิดปกติในระดับ gene จะมีผลทาให้เกิด hypercoagulation ซึ่งสามารถ
ถ่ายทอดทางพันธุกรรมได้ ความผิดปกติของ protein C โดย Protein C เป็นพลาสมาโปรตีนโดยเป็น vitamin K
dependent factor ที่ทาหน้าที่ยับยั้งปัจจัยการแข็งตัวของเลือด Va และ VIIIa (coagulation factor Va และ
VIIIa) ในร่างกายไม่ให้เกิดการแข็งตัวของเลือดที่มากเกินไปจนทาให้การแข็งตัวของเลือดผิดปกติไปนั้นพบได้
ประมาณร้อยละ 5 ในกลุ่มประชากรผิวขาว ซึ่งเป็นสาเหตุสาคัญของการเกิด DVT นอกจากนี้ยังมีการเกิด factor V
Leiden mutation เป็นการกลายพันธุ์ที่พบมากที่สุดในผู้ป่วย venous thrombosis (ประมาณร้อยละ 20 – 40
ของผู้ป่วย VTE) และพบได้ประมาณร้อยละ 5 ในประชากรทั่วไป การกลายพันธุ์นี้ทาให้โปรตีน factor V ทางาน
ผิดปกติ (ในคนปกติ factor V ทาหน้าที่เป็นปัจจัยร่วม (cofactor) เพื่อช่วยให้ factor X ทางานได้ นอกจากนี้ยังทา
ให้เกิดการกระตุ้นการทางานของ protein C ทาให้เกิด Activated protein C (APC) โดย APC สามารถพบได้ใน
ร่างกายของเราซึ่งจะทาหน้าที่เป็นสารป้องกันการแข็งตัวของเลือด) เมื่อร่างกายมี factor V Leiden mutation เกิด
การยับยั้งที่การทางาน protein C จะทาให้เกิดการแข็งตัวเป็นก้อนเลือด การเกิด factor V Leiden mutation
mutation เป็นการกลายพันธุ์ที่เป็นปัจจัยเสี่ยงที่สาคัญของการเกิดก้อนเลือด (thrombotic risk) คนที่เป็น
heterozygote (-/+) ของการกลายพันธุ์นี้ (คือมี gene factor V ผิดปกติ 1 ข้าง) จะพบได้ประมาณร้อยละ 6 คน
กลุ่มนี้จะมีโอกาสเสี่ยงต่อการเกิด VTE มากกว่าคนอื่นๆ ถึง 8 เท่า
การใช้ฮอร์โมน estrogen เป็นยาคุมกาเนิดหรือฮอร์โมนทดแทนหรือยาในกลุ่ม selective estrogen-
receptor modulators (SERMs) จะเสี่ยงต่อการเกิด venous thrombosis ซึ่งกลไกยังไม่ทราบแน่ชัด แต่คาดว่า
ฮอร์โมน estrogen จะมีผลเพิ่มปริมาณของ clotting factor และทาให้เกิดการขาด antithrombin III, protein C
และ S นอกจากนี้ยังมี factor ชนิดอื่นๆ ที่มีผลต่อระบบการแข็งตัวเลือดในร่างกาย

suddenly of pulmonary embolism.

during or following a hospitalization. While
many patients develop a symptomatic DVT
prior to developing a PE, many do not.
Patients may die suddenly before effective
treatment can be initiated.
อาการ The patient may complain of leg swelling, pain,
or warmth.
Symptoms are nonspecific, and objective
testing must be performed to establish the
diagnosis.
The patient may complain of cough, chest
pain, chest tightness,
shortness of breath, or palpitation. The patient
may spit or cough up blood (hemoptysis).
When PE is massive, the patient may complain
of dizziness or light-headedness. Symptoms
may be confused with a myocardial infarction
or pneumonia, and objective testing must be
performed to establish the diagnosis.
อาการแสดง - The patient’s superficial veins may be dilated,
and a “palpable cord” may be felt in the
affected leg.
- The patient may experience pain in back of
the knee when the examiner dorsiflexes the
foot of the affected leg.
The patient may have tachypnea (increased
respiratory rate) and tachycardia (increased
heart rate). The patient may appear
diaphoretic (sweaty). The patient’s neck veins
may be distended. In massive PE, the patient
may appear cyanotic and may become
hypotensive. In such cases, oxygen saturation
by pulse oximetry or arterial blood gas likely
will indicate that the patient is hypoxic. In the
worse case, the patient may go into
circulatory shock and die
within minutes.

เป้าหมายการรักษา
เป้าหมายของการรักษา VTE ประกอบด้วย
1. ป้องกันการเกิด PE และป้องกันการเสียชีวิตจาก PE
2. ป้องกันการเกิด postphlebitic syndrome
3. บรรเทาและรักษาผู้ป่วยโดยทาให้เกิดอาการไม่พึงประสงค์จากการรักษาน้อยที่สุด
4. ป้องกันการกลับเป็นซ้า
ยาที่ใช้ในการรักษา VTE
1. Unfractionated heparin (UFH)
การใช้ UFH การป้องกันและรักษา thrombosis นั้นมีมาตั้งแต่ปี ค.ศ. 1930 โดย UFH ถูกค้นพบ
โดย McLean ในปี ค.ศ. 1916 ซึ่งค้นพบว่าสารสกัดจากตับสุนัขสามารถยับยั้งกระบวนการแข็งตัวของเลือดได้ และ
ในปี ค.ศ.1939 ก็ได้เรียกสารชนิดนี้ว่า heparin cofactor ซึ่งมีฤทธิ์ในการยับยั้งการแข็งตัวของเลือด heparin เป็น
สารจาพวก D-glycosaminoglycan ออกฤทธิ์โดยการจับกับ antithrombin III เนื่องจาก heparin เป็นสารที่มีมวล
โมเลกุลขนาดใหญ่ (5,000-30,000 ดาลตัน เฉลี่ย 15,000 ดาลตัน) ทาให้ไม่ถูกดูดซึมและถูกทาลายด้วยกรดจาก
ทางเดินอาหาร จึงต้องบริหารยาโดยการฉีด IV และ SC เท่านั้น และการฉีด heparin มีอาการไม่พึงประสงค์คือ
hematoma ทาให้ปัจจุบันนิยมใช้น้อยลง
กลไกการออกฤทธิ์
Heparin ออกฤทธิ์โดยใช้ส่วนที่เรียกว่า pentasaccharide บนโมเลกุล (พบส่วน pentasaccharide
เพียง 1/3 ของโมเลกุล heparin) จับกับ antithrombin III ซึ่งเป็น alpha-globulin ที่ทาหน้าที่ยับยั้ง active
clotting factor เช่น IIa, IXa, Xa, XIIa โดยในภาวะที่ไม่มี heparin การยับยั้ง active clotting factor ของ
antithrombin III จะเกิดขึ้นอย่างช้าๆ แต่ในภาวะที่มี heparin จะเกิดการยับยั้งได้เร็วกว่าถึง 1,000 เท่า ทาให้ยานี้
สามารถออกฤทธิ์ได้อย่างรวดเร็ว (onset สั้น) ดังแสดงในรูปที่ 7

เภสัชจลนศาสตร์ (Pharmacokinetics)
UFH ไม่ถูกดูดซึมจากระบบทางเดินอาหารเพราะมีโมเลกุลที่ใหญ่และมีสูตรโครงสร้างเป็น anionic
นอกจากนี้ค่า bioavailability ยังขึ้นกับ protein หรือ plasma binding, platelet factor-4 (PF-4),
macrophage, fibrinogen, lipoproteins และ endothelial cell การบริหารยาแบบ SC ค่า bioavailability จะ
เป็นแบบ dose dependent หลังจากที่บริหารยาไปแล้วจะมีค่า onset of action อยู่ที่ 1-2 ชั่วโมงเนื่องจากการ
ให้ยาแบบ SC จะไม่สามารถทานายการออกฤทธิ์ได้อย่างแน่นอน ถ้าต้องการฤทธิ์ anticoagulant อย่างรวดเร็วควร
ให้แบบ IV โดยส่วนใหญ่จะให้แบบ IV infusion เพราะการให้แบบ IV bolus จะเสี่ยงต่อการเกิด bleeding
มากกว่าสาหรับการให้แบบ IM จะไม่นิยมให้เพราะจะเกิด hematomas ได้ง่าย
UFH มีค่า volume of distribution = 60 มิลลิลิตรต่อกิโลกรัม ดังนั้นการให้ยาต้องคานวณตาม
น้าหนักตัว (weigh-based dosing) โดยขนาดยาสูงสุดสาหรับการให้แบบ loading dose คือ 10,000 ยูนิตต่อวัน
และ infusion rate คือ 2,300 ยูนิตต่อชั่วโมง แต่อย่างไรก็ตามสาหรับคนที่อ้วนมากๆ ยังไม่มีข้อมูลที่ชัดเจนว่าการ
ให้ในขนาดเกินกว่า maximum dose จะเกิดผลดี ยามีค่าครึ่งชีวิตอยู่ที่ 30-90 นาที แต่อาจมากถึง 150 นาทีใน
ผู้ป่วยบางรายเมื่อให้ยาในขนาดสูง ยาถูกขจัดออกทางไตเป็นหลัก มีส่วนน้อยถูกขจัดออกทางตับ ดังนั้นจึงต้องปรับ
ขนาดยาในผู้ป่วยโรคไตและตับ
ขนาดยาและการบริหาร (Dosing and administration)
ขนาดและวิถีทางการบริหารยา จะขึ้นกับข้อบ่งใช้ และเป้าหมายของการรักษาทั้งนี้เนื่องจากผู้ป่วยแต่
ละรายจะมีการตอบสนองต่อยาที่แตกต่างกัน โดยการใช้สาหรับป้องกัน VTE นั้นจะให้แบบ SC ในขนาด 5,000
unit ทุก 8-12 ชั่วโมงแต่ถ้าต้องการฤทธิ์ anticoagulant อย่างรวดเร็วก็ให้แบบ IV bolus แล้วตามด้วย IV
continuous ดังแสดงในตารางที่ 6 นอกจากนี้ยังมีวิธีการบริหารยาแบบอื่นๆ คือแบบ time-honored dosing
requirement โดยจะเริ่มให้ยาในขนาด 5,000 unit แบบ IV bolus แล้วตามด้วย IV infusion rate 1,000-2,000
unit/h ซึ่งจากการศึกษาที่ผ่านมาพบว่าการให้แบบ weigh-based dosing จะทาให้ผู้ป่วยเข้าสู่ therapeutic
response ได้เร็วกว่าการให้แบบ time-honored dosing requirement และยังลดการกลับเป็นซ้าของ
thrombotic event ได้ดีกว่า

การติดตามผลการรักษา (Therapeutic monitoring)
การติดตามผลการรักษาด้วย UFH จะอาศัยผลทางห้องปฏิบัติการเกี่ยวกับการแข็งตัวของเลือดซึ่ง
ได้แก่ whole blood clotting time, activated partial thromboplastin time (aPTT), activated
clotting time (ACT), anti factor Xa activity และ plasma heparin concentrations ในประเทศไทยจะ
นิยมใช้ค่า aPTT (ค่าปกติ 20-30 นาที) ในการติดตามผลการรักษาด้วย UHF มากที่สุดโดยจะกาหนดค่า aPTT
ให้อยู่ในช่วง 1.5-2.5 เท่าของค่าปกติ
อาการไม่พึงประสงค์ (Adverse drug reaction)
ภาวะเลือดออก (bleeding) เป็นอาการไม่พึงประสงค์ที่สาคัญ โดยเฉพาะการให้ยาแบบ IV bolus
ซึ่งสามารถป้องกันได้โดยการปรับขนาดยาให้เหมาะสมโดยใช้ค่า aPTT และประเมินปัจจัยเสี่ยงต่อการเกิด
bleeding ของผู้ป่วยร่วมด้วย ดังแสดงในตารางที่ 8
ตารางที่ 8 ปัจจัยเสี่ยงต่อการเกิด bleeding เมื่อได้รับ UFH
Hypersensitivity reaction อาการที่พบได้แก่ ไข้ หนาวสั่น ลมพิษ และ anaphylactic shock ซึ่ง
เป็นอาการที่พบได้บ่อยจากการใช้ heparin ที่ไม่บริสุทธิ์หรือชนิดที่เตรียมจากสัตว์
Thrombocytopenia (ภาวะที่มีเกล็ดเลือดต่ากว่า 150,000/mm3
) ซึ่งภาวะนี้อาจเกิดได้ภายใน 5
วันแรกของการใช้ยา ซึ่งเป็นผลจาก heparin-induced platelet aggregation ซึ่งภาวะนี้มักจะหายได้เองและไม่มี
อันตรายมีเพียงส่วนน้อยที่เกิดจนเป็นอันตราย นอกจากนี้อาจเกิดการสร้าง antibody ต่อต้าน heparin-platelet

ข้อห้ามใช้ (contraindication)
ห้ามใช้ในผู้ป่วยที่แพ้ยา และผู้ป่วยที่มีภาวะเลือดออก หรือมีโรค/ภาวะต่างๆ ดังแสดงในตารางที่ 8

การรักษาภาวะ Bleeding อันเนื่องมาจากการได้รับ UFH เกินขนาด
การได้รับ UFH เกินขนาดจะเป็นสาเหตุที่สาคัญของการเกิด bleeding ในผู้ที่เกิด bleeding อย่าง
รุนแรงหรือเกิด bleeding ในอวัยวะสาคัญเช่น intracranial, pericanial, intraocular และต่อมหมวกไต ซึ่งอาจ
เป็นอันตรายจนถึงแก่ชีวิตได้ ส่วนอาการทั่วไปได้แก่ ปวดศีรษะ ปวดข้อ เจ็บแน่นหน้าอก ปวดท้อง บวมตามแขนขา
ปัสสาวะเป็นเลือด หรือถ่ายเป็นเลือดสด ในผู้ที่เกิด bleeding อย่างรุนแรง จาเป็นที่จะต้องได้รับยารักษาที่เฉพาะคือ
protamine sulfate บริหารแบบ IV ในขนาด 1 มิลลิกรัม ของ protamine ต่อ 100 ยูนิต ของ UFH โดยสามารถ
เพิ่มปริมาณของ protamine ได้จนถึง maximum dose คือ 50 มิลลิกรัม หลังจากที่ได้รับยาไปแล้ว protamine
ซึ่งเป็น basic peptide ที่มีประจุตรงข้ามกับ UFH จะมารวมตัวกันและทาให้ UFH หมดฤทธิ์ภายใน 5 นาที ใน
ผู้ป่วยที่มีโรคไตร่วมด้วย อาจเกิด rebound bleeding ได้เนื่องจากยาถูกขับออกทางไตเป็นหลัก ดังนั้นในผู้ป่วยกลุ่ม
นี้อาจให้ protamine ซ้าร่วมกับการติดตามค่า Hct, Hgb และอาการอย่างใกล้ชิด
การใช้ UFH ในผู้ป่วยกลุ่มพิเศษ
UFH จัดเป็น anticoagulant of choice ในระหว่างการตั้งครรภ์ซึ่งยานี้จะมีความปลอดภัยมากกว่า
anticoagulant ตัวอื่นๆ โดยจะให้ UFH ไปเรื่อยๆ จนถึงไตรมาสสุดท้ายของการตั้งครรภ์และ UFH ไม่ถูกขับออก
ทางน้านมจึงสามารถใช้ในหญิงให้นมบุตรได้
การรักษา acute thrombosis ในเด็กขนาดที่ใช้คือ 50 units/kg IV bolus แล้วตามด้วย IV
infusion 20,000 units/m2
/day หรือให้ในขนาด loading dose 75 units/kg นานมากกว่า 10 นาทีแล้วตามด้วย
maintenance dose 28 units/kg สาหรับเด็กอายุ 1 ปี ข้อห้ามใช้ดังแสดงในตารางที่ 9

6. Warfarin
