ตารางที่4 ขอบงชี้ในการสงตรวจCT Scan(7)
A cranial CT scan is recommended if 1 or more of the following criteria are present
• Age less than 60 years
• Rapid (e.g. over 1 or 2 months) unexplained decline in cognition or function
• “Short” duration of dementia (less than 2 years with reliable history)
• Recent and significant head trauma
• Unexplained neurologic symptoms (e.g., new onset of severe headache or seizures)
• History of cancer (especially in sites and type that commonly metastasize to brain)
• Use of anticoagulants or history of bleeding disorders
• History of urinary incontinence and gaitapraxia early in the course of dementia (as may be found
in normal pressure hydrocephalus)
• Any new localizing sign (e.g., hemiparesis or Babinski’ssign)
• Unusual or atypical cognitive symptoms or presentation (e.g. progressive aphasia)
• Gait apraxia
ดังนั้นการตรวจ CTหรือ MRI scanจึงควรกระทําในผูปวยที่มีอาการสมองเสื่อมอยางรวดเร็ว
หรือ ผูปวยมีอายุนอยกวา60 ป เพื่อแยกโรคทางประสาทศัลยศาสตรออกไป เชน chronic subdural
hematoma ซึ่งมักพบในผูสูงอายุโดยอาจมีหรือไมมีประวัติอุบัติเหตุทางศีรษะรวมดวย ในผูปวยที่มี
ปจจัยเสี่ยงทางหลอดเลือดแดงแข็ง ควรสงสัยโรคหลอดเลือดสมองตีบ ซึ่งเปนไดทั้งสาเหตุ และ
ภาวะแทรกซอนของสมองเสื่อม สวนผูที่ไดรับยาตานการแข็งตัวของเลือด หรือมีความผิดปกติของ
เลือด ควรสงสัยภาวะเลือดออกในเนื้อสมอง (intracerebralhemorrhage)หรือ subdural hematoma
ผูปวยที่มีอาการอื่นรวมดวยในชวงแรกของภาวะสมองเสื่อม เชน กลั้นปสสาวะไมอยู เดินผิดปกติโดย
ทรงตัวไมอยู ควรสงสัยโรค normal pressure hydrocephalusซึ่งใน CT/MRI scanจะพบวาขนาดของ
ventricle ใหญขึ้นมากเมื่อเทียบกับเนื้อสมองที่ยังมีปริมาณปกติ แพทยควรทําการเจาะหลังเพื่อตรวจน้ํา
ไขสันหลัง และวัดความดันน้ําไขสันหลังดวย ซึ่งผูปวยมักมีอาการดีขึ้นหลังจากเจาะหลัง หากแพทย
