with MetLife for Dental and accepting the proposals
with 5 Star for Life and AD&D, Symetra for Long Term
Disability and Davis Vision for Vision Insurance and
authorizing the City Manager to execute all necessary
agreements for the provision of such insurance.

2. BACKGROUND
FBS obtained renewals for Dental, Vision,
Life, AD&D, and long term disability.
The following companies submitted bids:
Dental: MetLife & UHC
Vision: Avesis, Davis Vision, UHC, Superior Vision
Basic Life: MetLife, 5 Star, OneAmerica,
Symetra
Long Term Disability: MetLife, Symetra,
OneAmerica
3. Recommendations for Employer Paid Coverage
Dental:
MetLife PPO is proposing a 4% increase and was the lowest
proposed rate
MetLife DHMO is proposing a 5.5% increase
Vision:
Davis Vision proposed a plan that includes significant benefits as
compared to Avesis
Rates guaranteed for 4 years
Basic Life with AD&D:
5 Star offered the lowest proposed rate with a savings of 29%, rates
are guaranteed for 3 years
LTD:
Symetra offered the same plan as MetLife but with a savings to the
city of 21% and the rates are guaranteed for 3 years
4. FISCAL IMPACT
Dental PPO increase from $21.47 to $22.32
per employee.
Dental DHMO increase from $11.53 to
$12.16 per employee.
Vision from $4.38 to $4.82 per employee.
LTD savings of 21% with a 3 year rate
guarantee
Basic Life with AD&D savings of 29%.
