 Health Registration and Action Plans
 Register if you’ve made mention of Asthma,
Diabetes, Seizure or Severe Allergy requiring Epi
Pen we encourage you fill out an Emergency
Action Plan.

 Medications to send in:
 Back up supply (daily meds if miss)
 Emergency/rescue meds (epi pens)
 Original Bottles/consents (found on webpage
under downloads and links)

without medication

Health
 Please provide basic diagnosis so can track (fever,
cold)

• NOT a peanut free campus
• Bringing food into building
– Avoid nut products when making items to
share with students (sports travel meals, XPX food
requests)
• Special diet requests (lactose, gluten)
– Diet Modification Request form found online
