• Fill out the forms – used for emergency action plans
– Freshmen complete dental form
• Food Allergies – inform us (NOT peanut-free campus)
– Diet Modification Request form found online
• Prescription Meds – original container & labeled
• Discourage frequent flyers
• Call in sick = communicate illness
• Flu Shots (fall) and Hearing Screening (spring)
• Blood drives 3x/Year

without medication
 Track illnesses through Linn County Public
Health
 Please provide basic diagnosis so can track (fever,
cold)

share with students (sports travel meals, XPX food
requests)
