For the millions of Americans that are affected by sleep apnea or even COPD, their CPAP
or Bi-level Positive Airway Pressure systems are important to them. Obstructive sleep
apnea can lead to major health problems that include high blood pressure, heart
problems, and have an overall negative impact on our well-being. Those suffering from
obstructive sleep apnea (OSA) have problems with breathing and sleeping every night.
The best way to treat sleep apnea or COPD is continuous positive airway pressure
therapy or CPAP. CPAP is used to treat mild, moderate and even severe cases of sleep
apnea. By using CPAP therapy, the risk of serious health problems can be avoided. These
systems consist of a machine that can pump the air at the prescribed pressure, hoses,
and masks. For complicated cases of central apnea, mixed apnea or Cheyne-
Stokes respirations, the treatment requires the use of high level technology devices,
such as bi-level PAP device (BiPAP) and auto-adjusting device (APAP).

People suffering from sleep apnea understand the impact OSA has on breathing as
well as night’s sleep. After being diagnosed with obstructive sleep apnea, there is

open and allow oxygen to flow freely. Your CPAP machine increases the pressure
in the back of the throat so that your airway does not collapse while you inhale.
The wearing of a facial apparatus (nasal or full face mask) may take some getting
used to, but compliance to this therapy is necessary to be effective.

There are two major types of CPAP machines-
Fixed Pressure CPAP - A fixed-pressure machine stays constantly at one pressure
level all the time and delivers air at the same pressure throughout the night. Fixed
pressure pumps run at a pressure that you set for your specific needs. They have a
ramp function which allows this pressure to increase slowly to this level after it is
put on, if the user prefers so. They are cheaper than auto-adjusting devices and
are highly acceptable to many patients, particularly for average pressure
requirements.
Auto – Adjusting CPAP – Also known as an APAP it automatically titrates, or tunes,
the amount of pressure delivered to the patient to the minimum required to
maintain an unobstructed airway on a breath-by-breath basis by measuring the
resistance in the patient's breathing, thereby giving the patient the precise
pressure required at a given moment and avoiding the compromise of fixed
pressure. Auto-adjusting CPAP, these respond to subtle changes in your upper
airway and provide an adequate pressure response on a breath-by-breath basis.
The flexibility in pressure can make for a more natural breathing sensation when
wearing CPAP. 7

Bi-level pressure (BPAP) – While a CPAP machine will provide pressure prescribed
by your doctor or sleep therapist at a constant rate, BiPAP therapy uses two
different pressure levels for inhalation and exhalation. With COPD it is common to
have a harder time to exhale rather than inhale, so the use of a BiPAP machine may
be more appropriate than the standard CPAP device. The Respironics BiPAP AutoSV
Advanced Sleep Therapy System monitors the patient’s airflow in the circuit and
adjusts to assist in inhalation and exhalation. For those suffering from COPD, the
system can help by lowering pressure during exhalation while maintaining pressure
to keep the airway open. Respironics also offer the Respironics BiPAP System
Replacement Accessories so your therapy won’t be interrupted. While BiPAP
machines are solely manufactured by Respironics, some standard CPAP machines
also offer Bi-Level Positive Airway Pressure. One example is the Devilbiss IntelliPAP
Bilevel S CPAP System with SmartCode which can be operated in both CPAP and bi-
level modes. A heated humidifier can be added for patient comfort and the unit
automatically adjusts for altitude up to 8500 feet. In some instances patients may
require a higher pressure to maintain an unobstructed airway. These patients can
use the Devilbiss IntelliPAP AutoBilevel System with SmartCode, which
automatically adjusts to the patient’s needs during inhalation and exhalation.

Choose the right CPAP machine for you after consultation with your doctor. There
are various factors to consider before purchasing the CPAP machine such as quality,
portability, additional features, and type of CPAP machine best suited for your
medical condition.
CPAP machine quality- Your CPAP machine must work very efficient all night and
should have high shelf life. It must also deliver a consistent level of air pressure
even if you have a leak around the mask. Choose quality CPAP machine from well
known brands such as Devilbiss and Respironics.
Noise- While almost all CPAP machines from top manufacturers are low noise and
made whisper quiet (below 30 db.), some are quieter than others. If sound is an
issue for you, make sure to check the decibel levels of machine you purchase.
Portability- If you travel frequently, go for a small, lightweight and compact CPAP
machine that is easy to fit in a travel bag. You can also buy a separate machine for
travel purposes. It's also crucial to make sure that your machine comes with
multiple plug-in adapters such as a DC power supply or international plugs, as well
as an option to use portable battery.
Ramp- For people with moderate to severe sleep apnea, relatively high pressure is
required to maintain the airway open. The ramp feature will allow a gradual
increase of the air pressure from a low level to the final prescribed air pressure,
making compliance more comfortable. It also makes it easier to adapt to the
immediate airflow of pressure settings, while wearing mask initially.

Exhalation pressure relief- This feature makes it comfortable to exhale your breath
against the incoming CPAP pressurized air, making breathing feel more natural and
easier. The machine maintains the prescribed air pressure settings during
inhalation, but reduces a bit during exhalation.
Heated tubing- A small heating coil helps keep the air in the tube at a constant
temperature. It is placed in the tubing that connects the CPAP machine to the mask.
Heated tubing feature aids in reducing condensation build-up in the tube and mask,
thus preventing water drops to form and fall on the face through the mask.
Mask on/off alert system- People who tend to make a lot of movements during
sleep risk mask coming off or losing mask seal. Some CPAP machines comes with an
alert system that beeps to wake the sleeper and remind them that their mask has
come loose.
Leak Compensation- CPAP machines with leak compensation feature can
compensate the pressure loss due to mask leak by increasing the airflow to ensure
that the patient is getting the prescribed pressure at all times.
Data Recording- This feature is used by some CPAP machines to collect data
regarding patient’s CPAP therapy such as pressure data, leak data, etc. It is used to
record the types of breathing events one may have when they use CPAP in sleep. A
memory card is used to record the information from patient’s sleep. It records the
necessary data for doctors to analyze the sleep quality.

Since each patient is different, the choice of a nasal apparatus or facemask is uniquely
different for each and every case. What works for one person may not be comfortable
for another and since therapy compliance is quite important, so is comfort. The
largest complaint about CPAP therapy is the discomfort of wearing a breathing
apparatus while you sleep. If you opt for a nasal mask, the Drive Extreme Comfort

softer, more comfortable silicon material. Some masks are available with quick

comfort while sleeping. To minimize the occurrence of leaks, the Fisher & Paykel
FlexiFit 431 Full Face CPAP Mask automatically adjusts to a wide range of nasal and
facial contours. Combining comfort and a great seal, the Invacare Twilight Full Face
CPAP Mask uses breathable headgear as head support to increase comfort and
reduce irritation. Patients who have facial hair, dentures, facial irregularities, or those
suffering from claustrophobia may have a tough time finding a mask that is suited for
them. The Respironics FitLife Total Full Face CPAP Mask with Headgear is designed
with these patients in mind. It provides a great seal around the less pressure sensitive
areas of the face and presents an un-obstructed view minimizing any claustrophobic
feeling. There are many solutions available to patients today that will allow for
greater compliance therefore making any CPAP therapy more effective.
Humidifier-
Humidifier makes CPAP therapy so much more comfortable by providing warm and
moist air instead of cool dry air. Most CPAP machines now come with an added
humidifier rather than as an optional feature. Heated humidifier is the most popular
choice. Most CPAP machines also have the option of either separating the CPAP from
the humidifier, or turning off the humidifier if needed. This feature come in handy
while traveling, when you don't want to take the entire machine.

By keeping all components of CPAP machines clean you can extend the longevity
of these parts and therefore keep the cost down.
Scrub-A-Dub Dub
The simplest way to extend the life of your CPAP supplies is soaking them in a
warm soapy solution for about five minutes. Using a small amount of Ivory liquid
or mild dishwashing detergent will suffice. Some helpful tips for drying CPAP
include:
• Hang your tubing over a shower/towel rod
• Place your mask/headgear on a towel in order to dry
• Hanging your headgear on a hook or a clothes hanger will expedite drying
• Do your cleaning in the morning so everything will be dry by night-time
• Wipe your CPAP/BiPAP machine weekly with a damp cloth
It is recommended to disinfect your CPAP mask weekly. Using a solution of two
parts vinegar and three parts water is perfect. After rinsing thoroughly, be sure to
air dry completely. You can also clean your machines filter weekly. Just rinse the
filter underneath tap water squeezing it to make sure all dirt is removed. Blotting
it with a towel will help speed up drying. It’s also a good idea to change your filter
every six months or so. Some machines may use disposable filters, so be sure
which one you have.
Cleaning your mask or headgear with alcohol is not recommended and they
should never be put in the dishwasher. You should disinfect your tubing once a
week by soaking it for 30 minutes in the same two parts vinegar three parts water
solution. This solution should be discarded after every use and not used for
multiple parts of your CPAP system.

For those that use a humidifier, it’s important to make sure you empty any and all
liquid in between uses to avoid any type of mold growth. They can also be soaked
for 30 minutes in the same vinegar/water solution listed above.
Replacement Time
You should replace your mask and headgear every 6-12 months, tubing every one-
two years, and if you use a humidifier change the chamber as necessary (usually
every one-two years). The great thing is there are numerous options available to
CPAP patients to help maintain their supplies as well as replacing them.
The Respironics SleepEasy Replacement Chamber Kit includes the chamber as well
as all the necessary seals and is intended to be used with the Respironics Sleep
Easy Sleep Therapy System with Humidifier. Sunset Healthcare offers replacement
filters for different styles and brands of CPAP machines. For instance, the Sunset

Required length and whether or not a smooth bore tube fits your fancy. The AG

length. It allows for excellent airflow and comes with 22 mm rubber or integral
cuffs, is grey in color and is latex free. Another replacement tube that is

  • 3. Introduction CPAP Machines For the millions of Americans that are affected by sleep apnea or even COPD, their CPAP or Bi-level Positive Airway Pressure systems are important to them. Obstructive sleep apnea can lead to major health problems that include high blood pressure, heart problems, and have an overall negative impact on our well-being. Those suffering from obstructive sleep apnea (OSA) have problems with breathing and sleeping every night. The best way to treat sleep apnea or COPD is continuous positive airway pressure therapy or CPAP. CPAP is used to treat mild, moderate and even severe cases of sleep apnea. By using CPAP therapy, the risk of serious health problems can be avoided. These systems consist of a machine that can pump the air at the prescribed pressure, hoses, and masks. For complicated cases of central apnea, mixed apnea or Cheyne- Stokes respirations, the treatment requires the use of high level technology devices, such as bi-level PAP device (BiPAP) and auto-adjusting device (APAP). Benefits of CPAP- • Decrease daytime sleepiness • Improves memory and cognitive functions • Improves the time spent in deep sleep • Treat hypertension in sleep apnea patients • Prevents the risk for myocardial infarction • Prevents the risk for diabetes • Helps control GERD (Gastroesophageal reflux disease) • Prevents excessive urination during the Night • Helps Improve the depression symptoms Using CPAP might have side effects such as dry mouth and nose irritation from air pressure, swallowing air and bloating, chest discomfort and sometimes even chest pain. Some users might feel claustrophobia. These CPAP problems are common, and learning how to use and manage CPAP machines can improve the success rate. 3
  • 5. CPAP Working Mechanism CPAP Machines People suffering from sleep apnea understand the impact OSA has on breathing as well as night’s sleep. Types of CPAP Machines CPAP Machines There are two major types of CPAP machines- Fixed Pressure CPAP - A fixed-pressure machine stays constantly at one pressure level all the time and delivers air at the same pressure throughout the night. Fixed pressure pumps run at a pressure that you set for your specific needs. They have a ramp function which allows this pressure to increase slowly to this level after it is put on, if the user prefers so. They are cheaper than auto-adjusting devices and are highly acceptable to many patients, particularly for average pressure requirements. Auto – Adjusting CPAP – Also known as an APAP it automatically titrates, or tunes, the amount of pressure delivered to the patient to the minimum required to maintain an unobstructed airway on a breath-by-breath basis by measuring the resistance in the patient's breathing, thereby giving the patient the precise pressure required at a given moment and avoiding the compromise of fixed pressure. Auto-adjusting CPAP, these respond to subtle changes in your upper airway and provide an adequate pressure response on a breath-by-breath basis. The flexibility in pressure can make for a more natural breathing sensation when wearing CPAP. 7

Types of CPAP Machines CPAP Machine Bi-level pressure (BPAP) – While a CPAP machine will provide pressure prescribed by your doctor or sleep therapist at a constant rate, BiPAP therapy uses two different pressure levels for inhalation and exhalation. With COPD it is common to have a harder time to exhale rather than inhale, so the use of a BiPAP machine may be more appropriate than the standard CPAP device. A heated humidifier can be added for patient comfort and the unit automatically adjusts for altitude up to 8500 feet. In some instances patients may require a higher pressure to maintain an unobstructed airway.   • 10. Choosing the CPAP Machine CPAP Machine Choose the right CPAP machine for you after consultation with your doctor. There are various factors to consider before purchasing the CPAP machine such as quality, portability, additional features, and type of CPAP machine best suited for your medical condition. CPAP machine quality- Your CPAP machine must work very efficient all night and should have high shelf life. It must also deliver a consistent level of air pressure even if you have a leak around the mask. Choose quality CPAP machine from well known brands such as Devilbiss and Respironics. Noise- While almost all CPAP machines from top manufacturers are low noise and made whisper quiet (below 30 db.), some are quieter than others. If sound is an issue for you, make sure to check the decibel levels of machine you purchase. Portability- If you travel frequently, go for a small, lightweight and compact CPAP machine that is easy to fit in a travel bag. You can also buy a separate machine for travel purposes. It's also crucial to make sure that your machine comes with multiple plug-in adapters such as a DC power supply or international plugs, as well as an option to use portable battery. Ramp- For people with moderate to severe sleep apnea, relatively high pressure is required to maintain the airway open. Mask on/off alert system- People who tend to make a lot of movements during sleep risk mask coming off or losing mask seal. Some CPAP machines comes with an alert system that beeps to wake the sleeper and remind them that their mask has come loose. Leak Compensation- CPAP machines with leak compensation feature can compensate the pressure loss due to mask leak by increasing the airflow to ensure that the patient is getting the prescribed pressure at all times. Data Recording- This feature is used by some CPAP machines to collect data regarding patient’s CPAP therapy such as pressure data, leak data, etc. It is used to record the types of breathing events one may have when they use CPAP in sleep. A memory card is used to record the information from patient’s sleep. It records the necessary data for doctors to analyze the sleep quality. 11

What works for one person may not be comfortable for another and since therapy compliance is quite important, so is comfort. The largest complaint about CPAP therapy is the discomfort of wearing a breathing apparatus while you sleep.   • 22. Cleaning the CPAP MAchine CPAP MAchines By keeping all components of CPAP machines clean you can extend the longevity of these parts and therefore keep the cost down. Scrub-A-Dub Dub The simplest way to extend the life of your CPAP supplies is soaking them in a warm soapy solution for about five minutes. Using a small amount of Ivory liquid or mild dishwashing detergent will suffice. Some helpful tips for drying CPAP include: • Hang your tubing over a shower/towel rod • Place your mask/headgear on a towel in order to dry • Hanging your headgear on a hook or a clothes hanger will expedite drying • Do your cleaning in the morning so everything will be dry by night-time • Wipe your CPAP/BiPAP machine weekly with a damp cloth It is recommended to disinfect your CPAP mask weekly. Using a solution of two parts vinegar and three parts water is perfect. After rinsing thoroughly, be sure to air dry completely. You can also clean your machines filter weekly. Just rinse the filter underneath tap water squeezing it to make sure all dirt is removed. Blotting it with a towel will help speed up drying. It’s also a good idea to change your filter every six months or so. Some machines may use disposable filters, so be sure which one you have. Cleaning your mask or headgear with alcohol is not recommended and they should never be put in the dishwasher. You should disinfect your tubing once a week by soaking it for 30 minutes in the same two parts vinegar three parts water solution. This solution should be discarded after every use and not used for multiple parts of your CPAP system. 22
  • 23. Cleaning the CPAP MAchine CPAP MAchines For those that use a humidifier, it’s important to make sure you empty any and all liquid in between uses to avoid any type of mold growth. They can also be soaked for 30 minutes in the same vinegar/water solution listed above. Replacement Time You should replace your mask and headgear every 6-12 months, tubing every one- two years, and if you use a humidifier change the chamber as necessary (usually every one-two years). The great thing is there are numerous options available to CPAP patients to help maintain their supplies as well as replacing them.
