Examples of a Near Miss
• Employee driving forklift
through the warehouse,
rounds a corner, and slams on
the brakes to avoid hitting a
pedestrian.
• Employee bringing down a
skid of material when a 50-lb
pail falls off and lands on the
floor 15 feet below.
• Employee cleaning equipment
while not locked out. The
equipment moved suddenly
during the cleaning, but no
one was hurt.

Near Miss vs Injury
• The only difference between an injury and a near miss is in the outcome.
• And the outcome is only a matter of luck!

Why Report a Near Miss?
• You can learn just as much from a near miss as you can from a serious
injury if you take the time to look.
• Since we have more near-misses than injuries, we can analyze the
incidents to spot trends or problem areas.
• Employees can see what not to do and share lessons learned with
others.
• Remember, we can’t fix what we don’t know about!

Definitions
• First Aid: a minor injury which may or may not require treatment on site
such as band aids or ice packs
• Recordable Accident: injury resulting in treatment that cannot be
provided on site. Any incident that results in prescription medication,
stitches, or treatment only a doctor can provide would be classified as an
accident.
• Property Damage: damage to equipment, building structure or resulting
in loss of production.

If seeking medical attention
• You must inform your supervisor
• If during work hours, you will be taken to the hosbital by the supervisor,
taxi, a
• ambulance or a co-worker. Never drive yourself to the hospital!
• If you decide to go during non-work hours, you must call work as soon
as possible to inform HR/Safety so a Form 7 can be initiated. OSI Forms
available.
