Playa del Rey Care & Rehabilitation Center, Playa del Rey, CA
Administrator, 2011 - 2015
Responsible for the day-to-day operation of a 99-bed Skilled Nursing and Rehabilitation Facility in the highly competitive Los
Angeles Westside environment. Facility rehabilitation specialties included orthopedic, stroke, and cardiac rehabilitation.
Supervised over 100 employees. Reinstituted staff recognition programs and improved employee morale. Passed all surveys
with no major deficiencies. Upgraded dietary equipment to maintain compliance and improve kitchen operations. Reorganized
nursing and management staff including the Nursing Director, staff nurses, and all but two department head staff to improve
facility operations, communications, and facility customer service environment. Exceeded annual facility budgetary projections
after arrival. Consistently maintained labor and expenses at or below budgeted levels. Improved facility amenities by installing
televisions and telephones at every bed and installing WiFi throughout the facility. Put in place procedures to resolve ongoing
linen and central supply shortages. Assisted in the re-designing of the company’s website, brochures, and marketing materials.
Organized marketing program to network with community hospitals, case managers, and other community referral sources In

Woodland Care Center, Reseda, CA
Administrator, 2008 - 2011
Responsible for the day-to-day operation of a 157-bed Skilled Nursing Facility in the San Fernando Valley. Facility contained a
62-bed Express Recovery Unit devoted to short-stay residents (orthopedic, stroke, cardiac problems.) Supervised approximately
165 employees. Reinstituted several programs to improve employee morale. Passed surveys with only minor deficiencies.
Exceeded annual facility budgetary projections since arriving. Consistently maintained expenses at or below budgeted levels.
Increased Medicare census to historic highs. After inheriting a deficit of $100,000 in February 2008, presided over a budget turn
around resulting in a year-end $500,000 over-budget profit. Maintained acquired decubitus levels at an average 1.5%, facility
overtime averages at an average 2%, and weight loss average at less than 3%. Assisted in the re-designing of the company’s
website and brochures. In 2009, directed the facility in winning the Step 1 AHCA National Quality Award. Successfully
trained 3 AIT’s all of whom presently are successfully operating skilled nursing facilities in this area.

Hy-Pana House Care Center - Fresno, CA
Executive Director, 2005 – 2008
Worked with company executives in turn-key preparations for the acquisition of a 112-bed skilled nursing and rehabilitation
hospital. Responsible for the successful takeover and transition of the center including inventory control, vendor operations,
physician relations, and community relations and marketing. Acted as transition between old and new ownership on date of
turnover. Organized and stabilized staff with no loss of key department management personnel. Managed existing operations
and set up systems to find and repair weak internal systems. Internalized Housekeeping and Laundry operations markedly
reducing costs and improving production. Supervised the changeover of computerized clinical medical records system software.
Led facility to a successful change-of-ownership survey. With the approval of the owner, responsible for all facility repair and
improvement projects including maintenance repairs and improvements, dietary equipment assessment and replacement, facility
street sign design and replacement, grounds improvements, and furniture replacement. Also played a key role in the planning
and direction of the reorganization and renovation of the facility. Designed and produced all marketing brochures, promotional
strategies, and sales materials.
2. Responsible for the ongoing operations of the facility since the transition. Instituted management programs to increase staff
morale and resident care. Increased Medicare and overall census to historic highs. Controlled costs within budgetary
allowances. The facility has shown a considerable profit since the first month of assuming operations. In the first full year of
operations (2006), the facility produced the highest profit contribution in the company.
Consulted with Director of Operations on various projects and assignments. Prepared to preside over the acquisition of Hy-Pana
House Care Center in Fresno.
Bishop Care Center – Bishop, CA
Executive Director, 2003 – 2005
Took over the turn around operations of a 99-bed, Skilled Nursing Facility under threat of fast-track decertification. Upon entry
into the facility, adjusted staffing load and staffing patterns to proper levels; reduced staffing shortages and call-offs retaining
many long term employees ready to leave; and instituted administrative systems to stabilize facility operations. Instituted team
meetings and goal directed projects to raise facility morale and staff cohesiveness. Reduced pressure sores from 14% to 2%.
Reduced weight loss levels to 1%. Reduced staff turnover, overtime, and expenditures to historic lows. Increased census and
revenue to all time facility highs. Passed final re-survey with zero deficiencies. Passed subsequent annual standard survey in
June 2004 with excellent results.
Candlewood Care Center, Long Beach, CA
Executive Director, 2000 – 2002
Responsible for the day-to-day operation of a 120-bed, unionized Skilled Nursing Facility in the competitive Long Beach
environment. Facility also contains a 42 bed, secured Alzheimer's/Dementia unit. Supervised approximately 110 employees.
Upon entry into facility, initiated a cleanup and repair program to bring the facility into survey-ready condition. Instituted a QA
program to bring facility into operational condition. Expanded the "Weekend Manager Program" to better serve the residents
and visitors. Passed all three surveys with only minor deficiencies. Increased Medicare census to historic highs. Presided over
first profitable year for the facility since company ownership ($230,000 turnaround from FY 2000 to FY 2001). Maintained
acquired decubitus levels at an average 1%, facility overtime averages at an average 3%, and weight loss average at less than
1%. Reduced receivables over 120 days old and reduced facility overtime by close monitoring and close controls along with
regular Administrative review. Redesigned facility brochures and marketing program. Consistently maintained expenses at
budgeted levels.
SunBridge Care Center of Santa Monica - 17th Street, Santa Monica, CA
Administrator and Area Administrator, 1999 – 2000
Operated a 72-bed Skilled Nursing Facility in the highly competitive West Los Angeles environment. Supervised
approximately 80 employees. Upon entry into facility, initiated a repair program to bring the facility into satisfactory physical
condition. Maintained budgetary and labor hour management controls. Instituted a "Department Head Rounds" program to
bring facility into "crisp" physical and operational condition. Improved the "Weekend Manager Program" to better serve the
residents and visitors. Exceeded overall census and quality mix goals. Annual expenses held at budgeted levels. Achieved
profitability for the first time in many years. Annual 1999 net profits exceeded budgetary expectations by $350,000.
Maintained acquired decubitus levels at 2%, facility overtime at less than 3%, and weight loss at 3% or less. Psychotropic drug
use and fall incidents both held to less than 10%. Maintained census, staffing, and facility care integrity throughout the period
of Sun Healthcare Company's bankruptcy. Served as Area Administrator for several months (and through two annual surveys)
for two facilities until a permanent Administrator was appointed.
