Pneumoperitoneum is the abnormal presence of air in the peritoneal cavity. It can be caused by perforated hollow organs due to conditions like peptic ulcer disease, bowel obstruction, or appendicitis. It can also be caused iatrogenically due to medical procedures like laparoscopy or peritoneal dialysis. On a chest x-ray, free air under the diaphragm or the cupola sign may be seen. An abdominal x-ray may show signs like the double wall sign or gas outlining organs. Ultrasound can also detect free air in the peritoneal cavity. Large amounts of pneumoperitoneum can cause continuous diaphragm or football signs on imaging.
