The instruction is sufficient. But it is better to write “
Complete the sentence with simple past or past perfect
form of the verbs.”

3. • Choose the correct modal or modal form to fit the sentence.

The instruction is confusing. Because any student can be
confused with “modal and modal form”. Instead, it can be
written only “modal”.
