- There are more than thirty causes of pneumonia. These causes are divided into three major categories. The categories include bacterial, viral, and mycoplasma, which is also known as atypical pneumonia.
- Extrinsic causes of pneumonia include irritants, causative, and pulmonary injury. Irritants include smoke inhalation and paint fumes. Causative includes toxins or toxic chemicals. Direct pulmonary injury can be caused by falls or car accidents for example.
- Intrinsic causes : loss of upper airway protective mechanism. Stokes, throat cancer, and other disease processes can cause a loss of the gag reflex. Without the gag reflex aspiration of food and fluids in to the lungs occurs which will lead to aspiration (bacterial) pneumonia.
- When the inflammatory process begins neutrophils move into the extracellular space were they attack the bacterial and stop the reproductive cycle of the bacteria.
- The continued reproduction of the bacteria causing infection/ inflammation of the lungs leads to ineffective gas exchange. The patient’s lungs are incapable of exchanging CO2 for O2. This leads to hypoxia. Patient’s also tend to develop an ineffective breathing pattern in an effort to expel the CO2 and take in O2.
- Zosyn combines two medications, Piperacillin and Tazobactam.
- The Zosyn effectively binds to penicillin receptors and works to stop the reproduction of the targeted bacteria. Zosyn is effective at all stages of bacterial reproduction. Zosyn has a half life of 0.7 to 1.2 hrs from administration. Sixty-eight percent of piperacillin is excreted in urine and another ten to twenty percent is excreted in feces.
- For patient safety it is very important that the five right of administration is followed. Be aware of the compatibility or incompatibility of any fluids that may have already been ordered as a continuous infusion. Zosyn is not compatible with lactated ringers. Always assess for drug to drug compatibility, for example Zosyn interacts with Vancomycin. When Zosyn and Vancomycin are given at the same time or back to back it puts the patient at risk for nephrotoxicity. Being aware of patient allergies is also important. Zosyn is a penicillin based antibiotic, so if the patient is allergic to penicillin they would be at risk of an adverse reaction to the Zosyn.
