What do patients' need?

What do patients' need, right now, at this very given moment? It's an excellent question, which--if we mean to actually achieve--we should narrow down to a few concise and relevant points. Paramount to the deconstruction of patient needs is understanding what the goal for patients is. This goal isn't just to live. It's to live well. It's to contribute to and enjoy the world -- to have hike the mountains, enjoy the breeze of the oceans, bask in the heat, laugh with friends, love our partners, and grow as a person. Now, moving slowly from the goal to the respective problems. We have patients who are ill once or twice and are then relatively healthy, healthy patients who see their doctor once a year for a checkup, chronically ill people who live for the rest of their life with chronic conditions like Crohn's or Hyperthyroidism, and there are terminal patients dealing with life threatening illnesses. Certainly, there are many many more subsections of patients. The needs for each of these segments are varient - varying along a spectrum with healthy patients needing less than the chronic and the chronic needing less than the terminal. Some people happen to be further along this spectrum than others. At the end of this presentation, we announced a survey to enumerate and articulate these needs across these various spectrums. It is our hope that this survey helps illustrate these pieces along the timeline, allowing us to focus on perfecting each piece.

In an effort to expand access to antiretroviral treatment (ART), countries across sub-Saharan Africa have begun to implement and scale up differentiated approaches to ART distribution, particularly at the community level. These distribution models are typically designed to cater to stable adult patients and aim to bring ART closer to where patients live, which, in turn, serves to decongest clinics and reduce the overall strain on a countryās health system. While documentation on the results of these models exists, there is limited information on the specific barriers and enablers that have impacted the implementation of such models. This cross-country study describes various models of differentiated ART distribution and the barriers and enablers associated with their implementation.
The community models of ART distribution that were explored were: community adherence clubs, outreach and community distribution points.
It was concluded that While differentiated models of ART distribution aim to broaden patient access to ART, models that provide these services at the community level have additional challenges and considerations that should be assessed and planned for prior to implementation. Two important barriers seen across all countries assessed included stigma and poor linkage to care. Peer support and education for both patients and providers were noted as important drivers of the success of community models. Intensive education at the front-end of scale-up can help providers effectively market the community-level models and help patients pick a model that would work best for them. Understanding these barriers and enablers will help country programs effectively and efficiently implement differentiated models of ART distribution to fit varying contexts, and allow for rapid scale-up, resulting in expanded access to ART and the ability to better meet the demands of patients on ART on a global level.

This dissertation explores an online health community called BCSM (Breast Cancer Social Media) established on Twitter. Through interviews and analysis of tweets with the #bcsm hashtag, the author finds that the community provides clinical and emotional support, information sharing, and knowledge translation between patients, providers, advocates, researchers, and caregivers. The community exhibits "cross-peer engagement", where individuals of differing statuses interact, and forms "microspurs", new relationships that result from community participation and expand support networks. The research contributes to understanding how social media enables a novel form of patient-centered healthcare through online communities.

The creation of community health promotion and education programs takes into consideration several agencies or parties who help in the achievement of the desired health goals. Each of the partners will address its roles using different approaches depending on their area of expertise. This is an important factor to consider as different institutions address health promotion using different approaches and perspectives. The overall outcome from the contribution of every partner should be able to restore and promote the physical, emotional, spiritual, psychological, and social wellness of the community in relation to the health issue being suffered (Minelli, & Breckon, 2009). Chronic diseases are currently the leading causes of death in the community due to their complexity and the severe effects on human health. The community health promotion and education program will be provided by the āHealth Concerns Coalitionā which will be made up of the following partners; community religious groups, Cancer Supportive Care Foundation, an association of cancer-survivor patients, nutritional organizations, and the local authority.
1. Cancer Supportive Care Foundation ā This is an important part of the coalition as it will offer technical expertise in education and diagnosis of chronic diseases. The foundation team will include medical experts who will diagnose the community members of any chronic illnesses. Examinations for diseases such as breast cancer, prostate cancer, diabetes and blood pressure will be conducted by this partner as they will provide modern machines needed for the diagnosis of chronic illnesses.

Who are the clients in Community Health nursing?
Individuals, families, and groups who live in a specific geographic area and may be at risk for health problems or in need of health services are considered clients in community health nursing. People of all ages, from infants to the elderly, as well as those with physical, mental, or social challenges, may be included. The purpose of community health nursing is to promote the health and well-being of the entire community by addressing the health needs of its individual members (Rector & Stanley, 2021). Community health nurses work with clients to identify and address health risks, provide health and wellness education, and connect clients to suitable health services.
What government resources might they be eligible for?
Individuals and families may be eligible for a variety of government resources depending on their circumstances. Some of these resources are intended specifically for people with low incomes or who are experiencing financial hardship, while others are open to anyone who meets certain criteria. Among the resources available to them are (ISPOR, n.d):
1. Medicaid: It is a federal-state partnership program that provides health insurance to low-income individuals and families. Individuals must meet income and asset limits, as well as other requirements, to be eligible.
2. Children's Health Insurance Program (CHIP): It is a federally funded program that provides health insurance to low-income children that are not eligible for Medicaid but cannot afford private health insurance. It provides coverage for a variety of medical services, such as preventive care, doctor visits, hospital stays, and prescription medications. Eligibility is determined by income and family size.
3. Supplemental Nutrition Assistance Program (SNAP): It is alsoĆĀ known as food stamps and itĆĀ provides financial assistance to low-income individuals and families in order for them to purchase food. Income and assets, as well as other factors, determine SNAP eligibility.
4. Temporary Assistance for Needy Families (TANF): This program helps low-income families with children by providing financial assistance as well as other services such as job training and childcare. Income and assets, as well as other factors, determine TANF eligibility.
5. Low Income Home Energy Assistance Program (LIHEAP): It is program funded by the federal government that provides low-income households with financial assistance to help them pay for home energy costs such as heating and cooling. The Department of Health and Human Services (HHS) administers the program, which is intended to assist households that are struggling to pay their energy bills and may face having their service disconnected. Eligibility is determined by income and family size.

Defining a Culturally Competent Organization Culturally competent health care, broadly defined as services that are respectful of and responsive to the cultural and linguistic needs of patients, is increasingly viewed as essential in reducing racial and ethnic disparities, improving health care quality, and controlling costs. The U.S. government considers cultural competence as a method of increasing access to quality care for all patients. The aim should be to develop systems more responsive to diverse populations. Managed care organizations view cultural competence as driving both quality and business. By embedding cultural competence strategies into quality improvement initiatives to make care more efficient and effective, clinical outcomes are improved while costs are controlled. Those in academic settings agree that cultural competency education is crucial for preparing future health care workers, although appropriate education on the topic is provided in only half of the medical schools in the United States (Betancourt, Green, Carrillo, & Park, 2005). According to the Office of Minority Health, cultural competence refers to the ability of health care providers and organizations to understand and respond effectively to the cultural and linguistic needs of patients (Office of Minority Health, 2001). Cultural competence encompasses a wide range of activities and considerations. It includes providing respectful care that is consistent with cultural health beliefs of the clients and family members. Competent interpreter services and programs to promote staff diversity are other ways in which health care organizations can increase cultural competence (Clancy & Stryer, 2001). Because communication is a cornerstone of patient safety and quality care, every patient has the right to receive information in a manner he or she understands. Effective communication allows patients to participate more fully in their care. Communicating effectively with patients is also critical to the informed consent process and helps practitioners and hospitals give the best possible care. For communication to be effective, the information provided must be complete, accurate, timely, unambiguous, and understood by the patient. Many patients of varying circumstances require alternative communication methods: patients who speak and/or read languages other than English; patients who have limited literacy in any language; patients who have visual or hearing impairments; patients on ventilators; patients with cognitive impairments; and children. The hospital has many options available to assist in communication with these individuals, such as interpreters, translated written materials, pen and paper, communication boards, and speech therapy. It is up to the hospital to determine which method is the best for each patient. Various laws, regulations, and guidelines are relevant to the use of interpreters. These include Title VI of the Civil Rights Act, 1964; Executive Order .

Seriously Ill Patients Access To Experimental Therapies
There are four main methods for terminally or seriously ill patients to access experimental therapies: clinical trials, expanded access programs, compassionate use protocols, and emergency use. However, few patients enroll in clinical trials or access experimental therapies through other means due to various barriers. These barriers include lack of awareness about opportunities, concerns from drug companies about liability and bad publicity, limitations on access to early-stage experimental therapies, and lack of incentives for drug companies to provide access. Efforts are needed to help more patients learn about and navigate options to access potentially life-saving experimental treatments.
Combination Therapies
Combination therapies using multiple drugs or medical treatments promise to more effectively treat and possibly cure diseases like cancer, Alzheimer's, HIV/AIDS, and others. Combination therapies are more effective by attacking the disease through multiple pathways and are less likely to lead to drug resistance. Current drug development usually tests one drug at a time, but combination therapies could shorten development times and better target complex diseases. The organization I&C aims to educate about and help integrate combination therapies and precision medicine approaches into patient care.
