Community Nursing Practice Model
The prevalence of illness among poor urban and rural populations increase the demand for critical care services. However, there is a shortage for physicians who can take up responsibilities in the community health sector. Among the efforts in place to strengthen the human resource is the growing interest to have nurses in advance practices participate inpatient care at the community health level. By applying the community, nursing practice model advanced practice nurses are better prepared to deliver care and outcomes to patients in poor communities.
The Community Nursing Practice Model
The community nursing model plays an essential role in ensuring that less privileged communities can access better healthcare by providing a framework for community nurses to focus on entire populations that have similar health concerns or characteristics. For example, a society where there are reproductive health issues, nurses applying this model in such a community will be able to know what the needs of the community are as far as reproductive health is concerned (Maclaine, 2014).
The model considers all levels of prevention, which include primary prevention whereby the advanced practice nurses promote health and protect against threats to health in the community. For example, carrying out awareness in the community on sexually transmitted diseases and distributing latex condoms in the community (Maclaine, 2014). Another level of prevention is secondary prevention, which involves the community nurses’ practitioners detecting and treating problems at the early stage of detection so that the health problem does not cause serious problems or affect others. The last level of prevention involves the community nurse practitioner preventing existing problems from getting worse.

Clinical prevention and population health are one of the MSN essential that is relevant to the community nursing practice. The underlying notion of this MSN essential is recognizing that masters prepared nurse applies and integrates a good organizational, patient-centered and culturally appropriate idea in planning to deliver and managing of clinical prevention and community care services to individuals and families (AACN, 2011). Planning: priorities are set; goals are formulated; objectives are developed; programmes, strategies, and approaches are specified; and means of evaluation are determined.
Intervention: the action phase of the TR process. Involves the actual execution of the programme plan by the TR specialist and client.
Evaluation: The goals and objectives are appraised. The primary question: How did the client respond to the planned intervention?

Community Nursing Practice Model
The prevalence of illness among poor urban and rural populations increase the demand for critical care services. However, there is a shortage for physicians who can take up responsibilities in the community health sector. Among the efforts in place to strengthen the human resource is the growing interest to have nurses in advance practices participate inpatient care at the community health level. By applying the community, nursing practice model advanced practice nurses are better prepared to deliver care and outcomes to patients in poor communities.
The Community Nursing Practice Model
The community nursing model plays an essential role in ensuring that less privileged communities can access better healthcare by providing a framework for community nurses to focus on entire populations that have similar health concerns or characteristics. For example, a society where there are reproductive health issues, nurses applying this model in such a community will be able to know what the needs of the community are as far as reproductive health is concerned (Maclaine, 2014).
The model considers all levels of prevention, which include primary prevention whereby the advanced practice nurses promote health and protect against threats to health in the community. For example, carrying out awareness in the community on sexually transmitted diseases and distributing latex condoms in the community (Maclaine, 2014). Another level of prevention is secondary prevention, which involves the community nurses’ practitioners detecting and treating problems at the early stage of detection so that the health problem does not cause serious problems or affect others. The last level of prevention involves the community nurse practitioner preventing existing problems from getting worse.
The MSN Essential
Clinical prevention and population health are one of the MSN essential that is relevant to the community nursing practice. The underlying notion of this MSN essential is recognizing that masters prepared nurse applies and integrates a good organizational, patient-centered and culturally appropriate idea in planning to deliver and managing of clinical prevention and community care services to individuals and families (AACN, 2011). Under this essential, it is well elaborated that a master’s degree level nurse should be able to synthesize broad social determinants of health and data from epidemiology to design and deliver clinical interventions to the communities in need while using relevant strategies.
In summary,the model has transcended values of respect, care, and wellness, which are essential in primary health care. In today’s society, certain types of cultural barriers in healthcare have the power to negatively impact outcomes and can lead to expensive consequences for health systems. Cultural competency refers to the ability to interact with people across cultures. When used in healthcare, it focuses on being able to care for patients with diverse values, beliefs, and behaviors. Today, the delivery of healthcare needs to be tailored to patients’ social, cultural and linguistic needs. According to the health care service company Cigna, cultural competency in healthcare can overcome health disparities. Some of these disparities include language barriers, cultural beliefs and practices, medical bias, variations in care access and quality, and low health literacy (Husson University, 2019).
The Purnell Model of Clinical Competence is a mid-range theory that is used for the research examining culture and within the context of culture and values. This theory was created by Larry Purnell and it was developed based on his observations of undergraduate nursing students and how they would interact with patients that are from different cultural backgrounds. In the United States, culturally competency is a key area for nurses and advanced practitioners (Marzilli, 2017).
Having the ability to communicate effectively with patients and their families is key for good patient care. The importance of communicating effectively in cross-cultural encounters is necessary. Cultural competence includes providing effective health care across diverse cultures by working collaboratively and communicating effectively. Advanced practice nurses and physicians who are aware of their own and their patients’ cultural backgrounds, along with the values that are often implicit in current medical models, are better able to achieve mutual understanding within the patient encounter and to focus on culturally appropriate health care interventions (Ladha et al., 2018)
With the Purnell Model, all aspects and domains of this model work by affecting one another, rather than standing alone. With this model there are specific cultural domains that include, heritage, communication, family roles and organization, workforce issues, bicultural ecology, high risk behaviors, nutrition, childbearing, death rituals, spirituality, health care practices and the healthcare practitioner (National Association for School Nurses, 2019).
As nurse practitioners, understanding the impact that cultural competence and interaction with your patients are key in quality patient care and trust. Cultural competency is relevant to healthcare education due to the fact that minorities will constitute 54% of the total United States population by 2050. The Purnell Model helps by evaluating the impact of interventions such as service learning and the change on cultural competence following the specific intervention. One of the essential roles of a community health nurse is to educate and provide best care practices that promote optimal health. They educate the client tailoring to their needs, as the needs of the population have changed. Being a community health nurse is not just tending to the homebound geriatric population. Nurses also visit group homes to provide monthly antipsychotic medications to clients and manage their medications, for example. They are needed to manage cases of patients when it is not easily accessible to travel to hospitals or physician offices.
There are a variety of examples of care they provide are providing hospice care, wound care, assessing the needs of the homeless or addicted population, or a school nurse. Nurse guide and coordinate essential services needed such as social workers, and counselling services.
Community nurses can help promote local resources available to the patient that are not aware such programs are available, such as community youth programs. In order to plan interventions or treatment nurses must be aware of services offered by the community.
An example may be a mental health nurse assisting a patient that has been affected by a death of a family member. Educating the patient on the stages of grief is vital and providing local resources to attend counselling or grief groups will help that patient gain a sense of understanding through members of their community to help them process a great loss with other people that are going through the same thing. A Pastor can help counsel a patient to understand the spiritual reasons of death and help them turn to their church leaders for strength and guidance.

The Purnell model for cultural competency is considered a model to improve comprehension on cultural competence for individuals within the healthcare community. Members of the healthcare field are acquainted to people from distinct backgrounds, cultures, beliefs, and values daily. The population is growing nationwide and is becoming more assorted. Therefore, nurses and other medical professionals need to become more familiar with cultural diversity or it might have a negative impact on the population. Purnell’s model aims at preventing this from happening by making nurses more culturally knowledgeable and catering to their needs regardless of a patient’s culture and background.

chose this topic because I am personally effected by it and so are many millions of Americans. Mental illness is also one of the leading causes of
death in our nation and one life is lost as a result of suicide, abuse or incarceration every 17mins in the United States. Mental illness has been my
area of focus throughout this program and the advocacy and participatory philosophy will be useful for the final project because it suggests that “
that research inquiry needs to be intertwined with politics and a political agenda” (Creswell, p.9). I do believe that mental health has a specific
agenda for a study and that there has been constant aim for reform in healthcare and mental health. This social issue is definitely pertinent right
now and topics that address it such as “empowerment, inequality, oppression, domination, suppression, and alienation” (Creswell, p.9), and are
really the focus of the study. The goal of this project for me, is to provide a voice to participants and give them the ability address the concerns that
will lead to reform.
According to Kemmis and Wilkinson (1998) this philosophy offers four key features of the advocacy/participatory framework of inquiry:
1. Participatory actions are focused on bringing about change, and at the end of this type of study, researchers create an action agenda for change.
2. It is focused on freeing individuals from societal constraints, which is why the study begins with an important issue currently in society.
3. It aims to create a political debate so that change will occur.

The establishment of a perinatal mental health service in Cairns was found to increase referrals to consultation liaison services in the region.
