The Seniors' WrapAround program helps immigrant and Francophone seniors live independently in their own homes. It is led by the London InterCommunity Health Centre and funded by the South West Local Health Integration Network. The program works to identify seniors' needs across different life domains and brings together natural and professional supports to create support plans. It also aims to increase accessibility of services for these seniors and engage internationally trained professionals. A case study describes how the program helped address the needs of a 78-year-old isolated immigrant man experiencing anxiety and lack of socialization.
